Decision quality and regret with treatment decisions in women with breast cancer: Pre-operative breast MRI and breast density.

PURPOSE: We evaluated self-report of decision quality and regret with breast cancer surgical treatment by pre-operative breast MRI use in women recently diagnosed with breast cancer. METHODS: We conducted a survey with 957 women aged 18 + with stage 0-III breast cancer identified in the Breast Cancer Surveillance Consortium. Participants self-reported receipt of pre-operative breast MRI. Primary outcomes were process measures in the Breast Cancer Surgery Decision Quality Instrument (BCS-DQI) (continuous outcome) and Decision Regret Scale (dichotomized outcome as any/none). Generalized estimat…

Density of breast: An independent risk factor for developing breast cancer, a prospective study at two premium breast centers.

BACKGROUND: Breast cancer is the leading cause of death among women worldwide. Studies have identified breast density as a controversial risk factor of breast cancer. Moreover, studies found that breast density reduction through Tamoxifen could reduce risk of breast cancer significantly. To date, no study on the association between breast density and breast cancer has been carried out in Malaysia. If breast density is proven to be a risk factor of breast cancer, intervention could be carried out to reduce breast cancer risk through breast density reduction. PURPOSE: To determine if density of…

Breast Biopsy Intensity and Findings Following Breast Cancer Screening in Women With and Without a Personal History of Breast Cancer.

IMPORTANCE: There is little evidence on population-based harms and benefits of screening breast magnetic resonance imaging (MRI) in women with and without a personal history of breast cancer (PHBC). OBJECTIVE: To evaluate biopsy rates and yield in the 90 days following screening (mammography vs magnetic resonance imaging with or without mammography) among women with and without a PHBC. DESIGN, SETTING, AND PARTICIPANTS: Observational cohort study of 6 Breast Cancer Surveillance Consortium (BCSC) registries. Population-based sample of 812 164 women undergoing screening, 2003 through 2013. EX…

Automated Volumetric Mammographic Breast Density Measurements May Underestimate Percent Breast Density for High-density Breasts.

RATIONALE AND OBJECTIVES: The purpose of this study was to evaluate discrepancy in breast composition measurements obtained from mammograms using two commercially available software methods for systematic trends in overestimation or underestimation compared to magnetic resonance-derived measurements. MATERIALS AND METHODS: An institutional review board-approved, Health Insurance Portability and Accountability Act-compliant retrospective study was performed to calculate percent breast density (PBD) by quantifying fibroglandular volume and total breast volume derived from magnetic resonance ima…

Contralateral breast cancer: a clinico-pathological study of second primaries in opposite breasts after treatment of breast malignancy.

BACKGROUND: Breast cancer is by far the most frequent cancer of women (23 % of all cancers), ranking second overall when both sexes are considered together. Contralateral breast cancer (CBC) is becoming an important public health issue because of the increased incidence of primary breast cancer and improved survival. The present communication concerns a study to evaluate the role of various clinico-pathological factors on the occurrence of contralateral breast cancer. MATERIALS AND METHODS: A detailed analysis was carried out with respect to age, menopausal status, family history, disease sta…

Loss of heterozygosity in normal breast epithelial tissue and benign breast lesions in BRCA1/2 carriers with breast cancer.

Loss of heterozygosity (LOH) of the wild-type BRCA1/2 allele is a reproducible event in breast tumors of BRCA1/2 mutation carriers, but it is unknown if this allelic loss occurs only in association with recognizable histopathologic abnormalities. We evaluated the early genomic changes that occur in the mammary glands of patients with increased predisposition to breast cancer due to germline mutations in the BRCA1/2 genes. We tested the hypothesis that these genomic changes may be detected, not only in histologically abnormal and malignant breast tissues, but also in morphologically normal tiss…

Management of the axilla in operable breast cancer treated by breast conservation: a randomized clinical trial. Edinburgh Breast Unit.

BACKGROUND: In the treatment of operable breast cancer by breast conservation, the extent of axillary dissection, the need for radiotherapy to the axilla and the morbidity associated with these procedures have not been assessed adequately. METHODS: Patients with operable breast cancer were randomized to have level III axillary node clearance (232 patients) or axillary node sample (234 patients). Radiotherapy to the axilla was given selectively. Radiotherapy was not given to those who had an axillary clearance. In the early part of the study all patients who had node sample were treated by rad…

Breast cancer risk associated with abnormal cytology in nipple aspirates of breast fluid and prior history of breast biopsy.

The authors previously reported an increased risk of breast cancer in women from the San Francisco Bay Area first enrolled between 1973 and 1980 with proliferative cytologic findings (hyperplasia, moderate hyperplasia, and atypia) compared with women with normal cytologic findings in breast fluids obtained by nipple aspiration and with women from whom breast fluid could not be obtained. To look for factors which might modify the risks associated with cytology, the authors examined several standard breast cancer risk factors: parity, age at first pregnancy, age at menarche, family history of br…

Reproductive history, breast-feeding and risk of triple negative breast cancer: The Breast Cancer Etiology in Minorities (BEM) study.

Few risk factors have been identified for triple negative breast cancer (TNBC) which lacks expression of estrogen receptor (ER), progesterone receptor (PR) and human epidermal growth factor receptor 2 (HER2). This more aggressive subtype disproportionately affects some racial/ethnic minorities and is associated with lower survival. We pooled data from three population-based studies (558 TNBC and 5,111 controls) and examined associations of TNBC risk with reproductive history and breast-feeding. We estimated odds ratios (ORs) and 95% confidence intervals (CIs) using multivariable logistic regre…

Family history of breast cancer and all-cause mortality after breast cancer diagnosis in the Breast Cancer Family Registry.

Although having a family history of breast cancer is a well established breast cancer risk factor, it is not known whether it influences mortality after breast cancer diagnosis. We studied 4,153 women with first primary incident invasive breast cancer diagnosed between 1991 and 2000, and enrolled in the Breast Cancer Family Registry through population-based sampling in Northern California, USA; Ontario, Canada; and Melbourne and Sydney, Australia. Cases were oversampled for younger age at diagnosis and/or family history of breast cancer. Carriers of germline mutations in BRCA1 or BRCA2 were ex…

Excessive breast self-examination among first-degree relatives of newly diagnosed breast cancer patients. High-Risk Breast Cancer Consortium.

First-degree relatives (FDRs) of women with breast cancer may have heightened anxiety about their personal risk for developing breast cancer. Breast self-examination (BSE) is an important component of risk surveillance for all women. In this study, the authors describe a subset of FDRs who appear to excessively (> or = once per day) perform BSE. These women, who constituted 8% of 1,053 FDRs in this study, were compared with women who did not examine excessively. The excessive self-examiners were older, more frequently African American, and less educated. They were more likely to have an affect…

Ultrasound-guided Finger Compression with Breast Stabilization for Postbiopsy Breast Pseudoaneurysm.

Breast pseudoaneurysm is an uncommon vascular complication after vacuum-assisted breast biopsy. Ultrasound-guided probe compression is typically recommended as first-line conservative management; however, technical failure may occur when probe slippage and breast parenchymal mobility prevent sustained focal occlusion of the feeding artery at the pseudoaneurysm neck. This Technical Note describes a modified ultrasound-guided compression technique that combines targeted finger compression of the pseudoaneurysm neck with concurrent breast stabilization by a second operator. A 63-year-old woman de…

Predicting "Ideal" Breast Proportionality to Support Implant Selection in Breast Augmentation.

BACKGROUND: The 45:55 upper-to-lower pole breast proportion described by Mallucci and Branford is widely cited as an aesthetic reference in breast augmentation. Anticipating whether this proportional relationship will be achieved preoperatively remains challenging, as postoperative breast shape is influenced by implant geometry, tissue characteristics, and lower pole deformation rather than implant volume alone. FRAMEWORK AND MODEL DEVELOPMENT: A regression-informed, proportion-based decision-support framework was developed using standard preoperative breast measurements and implant specifica…

Breast Envelope Complications After Revision Breast Implant Surgery: A Systematic Review.

BACKGROUND: Revisional surgery for cosmetic breast augmentation involves many factors like ptosis, tissue thinning, and a history of prior surgeries, which can increase the risk of reduced blood flow. There is limited research on breast vascularity in women who have undergone previous breast augmentation. This study reviews current literature on envelope-related complications in patients undergoing revisional aesthetic breast augmentation. METHODS: Between October and December 2024, a literature review was carried out in accordance with the PRISMA guidelines, using databases such as Cochrane,…
