Reaction of superficial bursae in response to specific disease stimuli.

Although microscopic studies have shown similarities between bursal and joint membranes, little is known about bursae and their response to disease states. Eighty-six cases of superficial bursitis due to trauma, bacterial infection, or gout were reviewed and compared with cases of joint inflammation due to the same etiologies. In traumatic bursitis the bursal fluid mucin test was more abnormal and the viscosity lower than that of joint fluid in traumatic arthritis. The bursal fluid total leukocyte count of septic bursitis was less than 20,000/mm3 in 8 of 13 cases but in only 1 of 21 synovial f…

Localized BIA identifies structural and pathophysiological changes in soft tissue after post-traumatic injuries in soccer players.

Localized bioimpedance (BIA) was measured with a single frequency phase-sensitive analyzer at 50 kHz in three post-traumatic types of injuries on four professional soccer players: (1) myositis ossificans, (2) intramuscular seroma and (3) trochanteric (hip) bursitis. Normal reference value (no injury) was obtained from the contra lateral not injured limb at a mirror-like location of the injury. The relative variations resistance (R) and reactance (Xc) at the time of injury was confronted with the not injured values. Relative variations between acute measurements and post medication ones on intr…

Shoulder ultrasonography in the diagnosis of polymyalgia rheumatica: a case-control study.

OBJECTIVE: Magnetic resonance imaging (MRI) showed that subacromial/subdeltoid bursitis is the most frequent shoulder lesion in polymyalgia rheumatica (PMR). We evaluated whether shoulder ultrasonography (US) was as effective as MRI in the detection of this lesion and assessed the sensitivity and specificity of bilateral subacromial/subdeltoid bursitis in the diagnosis of PMR. METHODS: A case-control study of 57 consecutive case patients with untreated PMR and 114 controls seen over a 6 month period in 3 secondary referral rheumatology centers. Control patients consisted of the next 2 consecu…

Arthroscopic resection of olecranon and prepatellar bursae.

This article presents a new method of excision of olecranon and prepatellar bursae using an arthroscopic technique. Six cases, three in each anatomic location, are retrospectively reviewed with an average follow-up of 6.25 months (1.5-15 months). When the etiology of the bursitis was traumatic, all four cases did well. Unsatisfactory results were obtained, for both a patient with CREST syndrome (Calcinosis, Raynaud's phenomenon, Esophageal hypomobility, Sclerodactyl, Telangietasia) and a patient with gouty bursitis. The results from this limited series would suggest reserving this technique fo…

Tibial collateral ligament bursa: MR imaging.

The bursa of the tibial collateral ligament (TCL) may be visualized at magnetic resonance (MR) imaging when it becomes distended with fluid. In the authors' experience, this finding signifies a pathologic condition either in the medial capsuloligamentous complex or in the bursa itself. Such a finding may indicate TCL bursitis. TCL bursitis can be suspected in the setting of isolated pain in the medial joint line in the absence of mechanical symptoms. Prolonged relief of symptoms after injection of steroid into the bursa is supportive of the diagnosis. Seven cases are presented in which a fluid…

Localized Pigmented Villonodular Synovitis of the shoulder: a rare presentation of an uncommon pathology.

Pigmented Vilonodular Synovitis is a rare clinical entity characterized as a synovial membrane benign tumour, despite possible aggressive presentation with articular destruction. The localized variant is four times less frequent and the shoulder involvement is uncommon. We present the case of a Caucasian 59 year-old patient, who presented with left shoulder pain, of uncharacteristic quality, with local swelling and marked functional limitation of 1 month duration. Shoulder ultrasonography showed subacromial bursitis. An ultrasound-guided aspiration was performed: synovial fluid was citrine-col…

Endoscopic bursal resection: the olecranon bursa and prepatellar bursa.

We treated 31 cases of olecranon bursitis and 19 cases of prepatellar bursitis. The average duration of symptoms before surgery was 1.1 years with a range of 3 months to 4 years. All patients had had preoperative aspiration and injection of cortisone. Patients underwent an arthroscopic bursal resection, removing all the bursal sack that could be seen. The results indicated that 86% of patients after olecranon bursectomy had no pain whatsoever. In the patients with prepatellar bursitis, 66% had no pain whatsoever, but we did note some residual tenderness in 24% of the patients, and 10% had pain…

Comparison of Shoulder Ultrasonographic Assessments between Polymyalgia Rheumatica and Frozen Shoulder in Patients with Bilateral Shoulder Pain: A Comparative Retrospective Study.

This study aimed to assess and compare the ultrasonographic (US) pathologic findings in patients with polymyalgia rheumatica (PMR) and bilateral frozen shoulder (FS). We included 19 patients with clinically diagnosed PMR and 19 patients with stage II bilateral FS. The US evaluation included the assessment of subacromial-subdeltoid (SASD) bursitis, long head of biceps (LHB) tenosynovitis, and posterior and inferior glenohumeral (GH) synovitis. Unilateral SASD bursitis was noted significantly more frequently in PMR patients than in bilateral FS patients (p = 0.001). There were no significant dif…

[Periarticular pathology of the hip].

The hip region is the seat of a rich and common periarticular pathology, with a variable involvement, depending of the location, and needs better knowledge. Anyway the clinical examination takes the major place in the diagnostic. Trochanteric bursitis and tendinitis are the more common clinical syndrome in that regional pathology. Local injections of corticosteroid are still the mean modality of the conservative treatment but are not sufficient to avoid recurrence or chronicity which may lead to tendon tears. A better knowledge of the physiopathology will lead to a better and early recovery. A…

[Corpora oryzoidea--rice bodies or melon seed-like bodies].

Two cases of bursitis complicated by rice bodies (C.O., corpora oryzoidea) are described. One case occurred after local elbow trauma, in the other case tuberculous bursitis was found. C.O. is a rare manifestation of fibrinous inflammation, seen not only in TB but also, perhaps more frequently, in rheumatoid conditions. Removal of the bursa and C.O. followed by microscopic investigation is recommended.

Intrabursal pressures in the olecranon and prepatellar bursae.

Intrabursal pressures were studied with a water manometer in 7 patients with olecranon bursitis and in 1 with prepatella bursitis. Baseline pressures in the olecranon bursa occurred from maximal extension to approximately 60 degrees of elbow flexion. Further degrees of flexion led to an exponential rise in pressure. In the prepatellar bursa a linear pressure rise was observed throughout the range of knee flexion.

Rheumatoid and other diseases of the cervical interspinous bursae, and changes in the spinous processes.

Bursal spaces between the cervical interspinous processes were found at necropsy in 14 out of 27 "normal' adult necks, especially when the spines were close together. In this random series they were the seat of crystallopathic disease in 2 instances out of 14 cases. In spines from 9 cases of adult-onset rheumatoid arthritis, rheumatoid bursitis was seen in 2 and banal bursitis in 2. In juvenile-onset chronic arthritis inflammatory bursal changes of rheumatoid nature were found in 2 out of 5 cases, and are compared with the "normal'. A third case showed crystallopathic destruction. In one insta…
