A Complex Case of Systemic Sclerosis with Concurrent Breast Malignancy and Treatment-related Complications.

Systemic sclerosis (SSc) is a complex autoimmune disease marked by vasculopathy, fibrosis, and multisystem involvement. Interstitial lung disease (ILD) and pulmonary hypertension (PH) are major mortality contributors. Importantly, SSc is associated with an increased risk of malignancy. This report presents a challenging case of SSc with severe Raynaud's phenomenon (RP), digital gangrene, calcinosis cutis, extensive ILD, severe PH, and right heart failure. The patient underwent mesenchymal stem cell therapy (MST), following which she developed breast cancer and chemotherapy-related complication

Calcified vocal cord nodule - a unique case report.

Vocal cord nodules are benign neoplastic lesions which occur due to submucosal oedema and haemorrhage, leading to fibrosis and hyalinization. Calcification in vocal cord nodules has not been reported so far in literature. It is thought to be a laryngeal counterpart of idiopathic calcinosis cutis. Here, we are reporting a case of a 38-year-old male patient who presented with a change in voice, which had a duration of one month. Laryngoscopic examination revealed a globular, yellowish white, sessile mass which arose from anterior commissure region of right vocal cord. Micro laryngeal excision wa

A Case of a Subepidermal Calcified Nodule on the Sole without Trauma.

Subepidermal calcified nodule is an uncommon form of calcinosis cutis, which most commonly occurs in children. It usually presents as an asymptomatic, solitary verrucous nodule on the head and neck region, but occasionally as multiple lesions. Serum calcium and phosphorus levels are usually normal. Histopathology shows well-formed homogeneous eosinophilic material and granules in the upper dermis. Material in the dermis stained with von Kossa was positive. We report on an unusual case of a subepidermal calcified nodule occurring on the sole. A 21-month-old male presented with an oval-shaped, w

[Indirect immunofluorescence method using anticentromere antibodies and its application in the study of chromosome aberrations in tumor cells].

The discovery that individuals with CREST-syndrome scleroderma (calcinosis cutis, Raynaud phenomenon, eosinophilia, telangiectasia) have anticentromere antibodies (ACA) in their peripheral blood has made it possible to visualize centromeres using indirect immunofluorescence staining. We have studied two tumor cells lines (k562 and SP2/0) using ACA and immunofluorescence techniques. In SP2/0 cells, two abnormal chromosomes were observed: One had a Robertsonian translocation and the other had a tandem translocation. Both were dicentric chromosomes, and the centromeres were functionally active. C

Critical Limb Ischaemia in a Patient With Subcutaneous Nodules: Clues to a Hidden Systemic Disease.

Dermatomyositis most commonly presents with characteristic rash or proximal muscle weakness; however, specific autoantibody subsets can produce markedly atypical phenotypes. Anti-NXP2 dermatomyositis is clinically significant for its strong association with malignancy, extensive calcinosis, and presentations that may be amyopathic or minimally symptomatic. We report a rare case of anti-NXP2-positive dermatomyositis manifesting without classical cutaneous or muscular features, instead presenting with acute limb ischemia and long-standing subcutaneous nodules. Amyopathic anti-NXP2 disease is unc

[Skin manifestations in patients with renal chronic renal failure on regular hemodyalysis].

The authors studied the skin disorders in 50 hemodialyzed patients. Pruritus appears to be the main dermatological feature by the frequency of its occurrence and by its unpleasant and even intolerable effects. This sign is more frequent among long time dialyzed patients and seems to be due to 2 factors: high urea blood concentration and secondary hyperparathyroidism. Calcinosis cutis is less frequent than pruritus. It seems to have the same origin and can be considered in the more general picture of the metastatic calcinosis in patients with renal failure and secondary hyperparathyroidism. Hyp

Six cases positive for anti-centromere antibodies with ulcer and gangrene in the extremities.

We report six cases that were positive for anti-centromere antibodies, with ulcer and gangrene in the extremities but mild or no skin thickening. The patients were five women and one man, and the mean age at onset of gangrene was 56 yr. Raynaud's phenomenon was found in five patients and calcinosis cutis in two patients. Three patients did not satisfy the criteria for systemic sclerosis and CREST syndrome in this study. Ulcer and gangrene occurred in the fingers in three patients, and in the fingers and toes in two patients. The gangrene was refractory to treatment, and amputation of fingers o

Case report 620. Progressive systemic sclerosis (PSS) with paraspinous and intraspinous calcifications.

Progressive systemic sclerosis (PSS) is a connective tissue disorder of unknown etiology and pathogenesis characterized by fibrosis of the skin (scleroderma) and variable involvement of several organ systems including the gastrointestinal tract, lungs, heart, and kidneys. Calcinosis cutis is a frequent finding, commonly seen in the soft tissues of the fingertips and over the knees and elbows. Calcifications may also occur in areas of muscle necrosis and have been reported in articular synovium. This report describes a patient with PSS in the form of subcutaneous calcifications and scleroderma

Progressive osseous heteroplasia.

BACKGROUND: Primary heterotopic ossification beginning in childhood is quite rare but occurs in several well-described conditions, such as Albright hereditary osteodystrophy, fibrodysplasia ossificans progressiva, and platelike osteoma cutis. Recently, a new disorder called progressive osseous heteroplasia (POH) has been described in the orthopedic literature. Primary cutaneous calcification and ossification beginning in infancy are presenting signs of this progressive and potentially debilitating disorder. OBSERVATIONS: We describe 2 children with POH who were recently seen at Children's Hos

Cutaneous gangrene secondary to metastatic calcification in end stage renal failure--a case report.

Metastatic calcification is a frequent association of chronic renal failure but skin involvement is accepted as a rare, albeit well recognised, complication accompanying the secondary hyperparathyroidism that results from renal impairment, although the aetiology remains unknown. Skin involvement can take the form of metastatic calcinosis cutis or cutaneous gangrene resulting from vascular calcification. There have been reported cases which describe the healing of gangrenous areas following parathyroidectomy as well as use of dietary restriction of phosphorus and phosphate binding antacids(1-6)

Extraosseous 99mTc Methylene Diphosphonate Uptake in Perianal Dystrophic Calcification: Incremental Value of Single-photon Emission CT/CT.

A 57-year-old woman underwent 3-phase 99mTc methylene diphosphonate bone scintigraphy for persistent right hip pain after cannulated screw fixation of a femoral neck fracture. The flow and pool phases were unremarkable, with delayed uptake around the right femoral neck screws. Incidentally, intense focal tracer uptake was seen in the right perianal region. Single photon emission CT/CT localized this activity to coarse subcutaneous perianal calcifications, corresponding clinically to chronic pruritic, discharging, chalky perianal plaques. The appearance favoured dystrophic calcinosis cutis/post

Isolated rheumatoid nodules: a diagnostic dilemma.

We present a 27-year-old male with multiple nonpainful soft tissue masses over several metacarpals, bilateral elbows, the left wrist, and both knees since the age of 4. Physical exam was significant for firm, nonmobile, nodular growths over the extensor surfaces of bilateral elbows and knees and on the 2nd and 5th metacarpal phalangeal joints. Laboratory studies revealed an unremarkable rheumatoid factor, negative ANA screening and normal joint radiographs. Differential diagnosis included subcutaneous granuloma annulare (SGA), seronegative rheumatoid nodule, and calcinosis cutis. Biopsy is the

[Normophosphatemic familial tumoral calcinosis: description of a case with phosphate lowering therapy].

Normophosphatemic familial tumoral calcinosis (NFTC) is characterized by cutaneous calcifications with no metabolic disorders; it manifests in the first years of life; associated with mutations in SAMD9 gene. Chronic trauma and transient hyperphosphatemia play a leading role in calcification formation.Surgical treatment can remove calcifications, but it does not prevent recurrence and can cause complications. Various conservative therapy is used. Lowering of blood phosphate levels is considered a promising pathogenetic treatment method. We present a clinical case of a patient with NFTC associat

Poikiloderma With Neutropenia due to Novel USB1 Mutation.

Poikiloderma with neutropenia (PN) is a rare autosomal recessive disorder characterized by skin abnormalities, chronic neutropenia, and an increased risk of infections and malignancies. Patients typically present with poikiloderma, which includes hypopigmented and hyperpigmented macules, telangiectasia, atrophy, as well as nail thickening and palmoplantar hyperkeratosis. The condition is caused by pathogenic variants in the USB1 gene, which affects neutrophil function and immune response. Endocrine involvement, such as hypogonadism, may also occur. We present a 17-year-old male with a novel US

Intravascular Large B-Cell Lymphoma Presenting as Systemic Capillary Leak Syndrome With Immunological Phenomena: A Case Report.

Intravascular large B-cell lymphoma (IVLBCL) has a high mortality rate, partly due to its heterogeneous presentation and rarity. We present a case of a 73-year-old woman who came into the emergency room in need of fluid resuscitation, interpreted as septic shock. However, broad-spectrum antibiotics gave no resolution, and no causative agent was found. Further physical examination showed proximal muscle weakness, Raynaud's phenomenon, and calcinosis cutis. During 3 weeks of admission, vasopressor support was required continuously due to a capillary leak syndrome. The patient passed away. The un

Dermatologic Manifestations of Endocrine Disorders.

Endocrine disorders are known to involve all organ systems of the body, including the skin. The cutaneous manifestations of endocrine disorders can range from common findings such as acanthosis nigricans, pretibial myxedema, acne, hirsutism, hyper or hypopigmentation to rare cutaneous findings such as miliaria rubra, calciphylaxis, lentigines, and calcinosis cutis. These cutaneous symptoms can sometimes be the presenting symptoms or can even be pathognomonic of the underlying endocrine condition. In some cases, the cutaneous symptoms from an underlying endocrine abnormality may be the most pro

[Therapeutic management in early disease stages of systemic sclerosis : early diagnosis - early symptoms - early problems].

Increasing knowledge about the rare disease systemic sclerosis and improved diagnostic methods in the course of recent decades has led to the possibility of diagnosing systemic sclerosis at earlier disease stages. However, earlier diagnosis has an impact on routine clinical care of affected patients, and rheumatologists need to know about early symptoms, their diagnosis, and clinical management. In this review, the therapeutic management of early disease stages is described. In particular, we focus on diagnostic tools which should be included in a "basic assessment" of systemic sclerosis and d

Mineralization of collagen and elastic fibers in superficial dystrophic cutaneous calcification: an ultrastructural study.

The ultrastructural morphology of localized skin calcifications without associated diseases and with normal serum calcium and phosphate ion values is still unknown. In a case of superficial dystrophic calcinosis cutis (DCC), the role of collagen, elastin and ground substance in the process of calcification and the organization of the apatite crystals could be studied by light and electron microscopy despite technical difficulties in sectioning the hard tissue. Ultrastructural investigation revealed the nucleation of calcification being related to collagen and elastic fibers. No intracellular c

Serum samples from 539 subjects were screened for the presence of the anticentromere antibody on a human laryngeal carcinoma (HEp-2) cell line (Antibodies, Inc.). The antibody was present in 61 patients (11%), most of whom had features of limited scleroderma or the CREST syndrome (calcinosis cutis, Raynaud's phenomenon, esophageal dysfunction, sclerodactyly, and telangiectasia), either independently or in association with primary biliary cirrhosis. The antibody was rarely found in patients with rapidly advancing or diffuse scleroderma. The anticentromere antibody is therefore a useful prognost
