Treatment of hypoparathyroidism usually requires the use of pharmacological doses of parent vitamin D or near physiological amounts of the hydroxylated metabolites, calcitriol or alphacalcidol. Vitamin D intoxication and hypercalcaemia may be a problem but can be minimised by the use of small doses of vitamin D or its metabolites combined with large amounts of oral calcium. The response to treatment can be easily monitored by measuring serum and urinary calcium and creatinine concentrations. This allows the derivation of two simple indices reflecting calcium load presented to the kidney (calci…

The components of calcium and magnesium balance and the factors responsible for the maintenance of the serum concentration of these cations are reviewed. Within this framework, the causes and treatment of disturbances of the serum concentration are discussed. Hypercalcemia is usually a reflection of increased bone resorption and/or gut absorption with the kidney playing a secondary role. Hypocalcemia is usually due to either a disturbance in the parathyroid hormone-adenylate cyclase system or a disturbance in vitamin D metabolism. As vitamin D is required for expression of the action of PTH at…

Calcium and phosphorus disturbances in childhood are discussed in vitamin D deficiency rickets, neonatal tetany and beta-homozygous thalassaemia. The nutritional, epidemiological, hereditary and other factors facilitating the development of the disease in these three groups of children are illustrated. The level of culture and sociability of the community must be taken into consideration for the success of the preventive medicine services, especially with regard to vitamin D deficiency rickets. The administration of phosphorus and vitamin D or metabolites of vitamin D to thalassaemic children …

Aequorin was microinjected into squid giant axons, the axons were stimulated, and the change in light emission was followed. This response was compared with that found when the axon, in addition to being microinjected with aequorin, is also injected with the dye phenol red. Large concentrations of phenol red injected into axons result in a high probability that photons emitted by aequorin, when it reacts with Ca in the core of the axoplasm, will be absorbed before they escape from the axon; photons produced by the aequorin reaction at the periphery of the axoplasm are much less likely to be ab

Serum calcium and magnesium were studied in 70 leprosy patients and 25 normal healthy individuals. An attempt has been made in this study to find out if there is any correlation between the clinical and pathological status of the disease and serum calcium and magnesium levels in the blood. Serum calcium was found to be significantly decreased in lepromatous leprosy (Ca=8.42+/-0.7 mg%, P less than 0.001) and dimorphous leprosy (Ca=8.68+/-0.94 mg%, P less than 0.05) while it was normal in tuberculoid leprosy (Ca=9.14+/-2.12, P less than 0.05). The decrease in serum magnesium level was highly sig…

In a retrospective study, based on a biochemical survey of people aged 65 and over in a general practice, subjects taking a barbiturate preparation for indications other than epilepsy had a significantly lower serum calcium concentration than did those taking nitrazepam or diazepam. Ordinary doses of barbiturate may adversely affect vitamin D metabolism in the elderly.

Calcium metabolism in pregnancy is a complex process involving calcium, phosphorus, vitamin D, parathyroid hormone (PTH), and calcitonin (CT). Calcium absorption is enhanced in pregnancy, and increased storage in the maternal skeleton probably occurs as well. Adequate amounts are provided by the current Recommended Dietary Allowance of 1,200 mg. daily which can be met readily by natural foods, specifically milk. If supplemental calcium is given, a nonphosphate salt is probably advisable, since some evidence suggests that excessive phosphate intake may be related to leg cramps in pregnancy. Vit…
