Cluster of wound botulism in California: clinical, electrophysiologic, and pathologic study.

Over a period of 15 months we have seen 6 patients with long-standing history of subcutaneous heroin injections who experienced acute blurred vision, dysphagia, dysarthria, and generalized weakness. Decreased or absent deep tendon reflexes, pupillary abnormalities, incremental responses to fast repetitive nerve stimulation, and positive serology for Clostridia botulinum toxin A were found, but not in all cases. Muscle biopsies showed variable signs of neurogenic atrophy. In vitro electrophysiology studies revealed decreased end-plate potentials quantal content, confirming the presynaptic natur

Production pressure in the work environment. California anesthesiologists' attitudes and experiences.

BACKGROUND: Pressure to put efficiency, output, or continued production ahead of safety has caused catastrophic accidents in various industries. The authors assessed the attitudes and experiences of anesthesiologists concerning production pressure. METHODS: A random, repeated-mailing survey was conducted among 647 members of the American Society of Anesthesiologists residing in California. Questions were asked about attitudes toward production pressure and other patient safety issues, frequency of occurrence of various operating room events, encounters with situations involving unsafe actions…

Serological surveillance of plague in dogs and cats, California, 1979-1991.

Following natural infection both cats and dogs develop antibodies to plague which can be measured for several months after infection. Besides being helpful in the diagnosis of plague in domestic carnivores, the antibody titer has important implications in outbreak investigation and surveillance programs. We report the first serological survey for plague in domestic carnivores conducted in California between 1979 and 1991 in five different settings or programs. A total of 4115 dogs and 466 cats were tested for plague antibody by the passive hemagglutination test. 86 dogs (2.09%) and 15 cats (3.…

Investigation of cryptosporidial infection in calves in San Bernardino County, California.

Cryptosporidium is a protozoan parasite that inhabits the intestinal epithelium of calves, lambs, foals, pigs, and poultry. Of the 500 calves studied, 95 calves had diarrhea and 20 (21.05%) of those were infected with Cryptosporidium; only 8 (1.9%) of the remaining 405 nondiarrheic calves had Cryptosporidium oocysts in their feces.

Thyroid storm. A review of cases at University of California, San Francisco.

Retrospective study of the diagnosis and management of the 8 cases of thyroid storm in a series of 400 hyperthyroid patients led to conclusion that thyroid storm is a clinical diagnosis based on a life-endangering illness in a hyperthyroid patient whose hyperthyroidism has been severely exacerbated by a serious precipitating illness, and that storm is manifest by the symptoms of hyperpyrexia, tachycardia and striking alterations in consciousness. No laboratory tests were diagnostic of storm, and the underlying precipitating cause of thyroid storm was the major determinant of survival. Vigorous

Phenylketonuria. Experience at one center in the first year of screening in California.

One year's experience with phenylketonuria during the calendar year 1966, the first year for compulsory newborn screening in California, was reviewed. The over-all prevalence rate from reported cases in California during this period was one case per 19,500 persons tested. Fifty-seven persons suspected of having pku were evaluated, and 25 of them were determined to be phenylketonuric. Eleven of the 25 were infants in whom the abnormality was detected through the newborn screening program or because it was detected in a sibling through a screening program. All the newborn phenylketonuric patient…

Control of diarrheal diseases in California state hospitals for retarded children.

The three California state hospitals for mentally retarded persons have been having a severe problem with amebic and bacillary dysentery and with other infectious diseases of the gastrointestinal tract. At Sonoma, the oldest of these hospitals, this problem is known to have existed for many years. Improved medical and nursing staffing and the use of antibiotics and other effective drugs developed in recent years have greatly lowered morbidity and mortality rates in these institutions. However, in themselves these measures have not been effective in lowering the incidence of infection. Studies …

Influenza Vaccine and Associated Infection and Death in California, 2024 to 2025.

IMPORTANCE: Effectiveness of seasonal influenza vaccine must be assessed annually as influenza viruses evolve, requiring updated vaccine components. Evidence is limited for direct vaccine benefit in lowering risk of death following influenza virus infection. OBJECTIVE: To evaluate the association between current-season influenza vaccination and laboratory-confirmed influenza virus infection and, among persons with laboratory-confirmed influenza, the association with influenza-associated death. DESIGN, SETTING, AND PARTICIPANTS: A case-control analysis of California residents aged 6 months or…

Impact of Wildland Fire Smoke PM2.5 on Birth Weight in California.

The increase in the frequency, duration, and intensity of wildland fires is a significant source of air pollution that can impact perinatal outcomes. This study assessed associations between wildfire fine particulate matter <2.5 μm (PM2.5) and adverse birth weight outcomes among singleton term births in California for 2007-2018. Exposure was assessed using bias-corrected Community Multiscale Air Quality Model, linked to residence at delivery. Logistic and linear regression models estimated associations between average daily wildfire PM2.5 and birth weight outcomes, adjusting for individual-lev…

Associations of PM2.5 exposure with diabetes-related mortality for California residents.

BACKGROUND: Emerging evidence links air pollution exposure to metabolic dysfunction; however, few studies have examined diabetes-related mortality in relation to ambient air pollutants using high-resolution exposure data at the population level. In the United States, particularly in large and geographically diverse states such as California, exposure contrasts and population heterogeneity provide an important setting to evaluate these associations. METHODS: We conducted a matched case-control analysis using California Department of Public Health (CDPH) Vital Records (2010-2021). Diabetes-rela…

Interstitial pneumonias of undetermined etiology in foals in California, 1990-2020.

Interstitial and bronchointerstitial pneumonias of undetermined etiology in young foals are relatively common in autopsy services with an equine focus. Unknown viruses, toxins, hyperthermia, surfactant or alveolar macrophage function deficiency, certain antibiotics, and aberrant responses to Rhodococcus equi or other bacteria have been proposed as causes. We performed a retrospective study of autopsies on foals with a diagnosis of interstitial or bronchointerstitial pneumonia with an unidentified etiology. Forty-one foals (median age: 3-mo-old) were included. Most were received in summer (n =

Trends and Racial and Ethnic Disparities in Maternal Cardiovascular Health in California.

BACKGROUND: To address the maternal health crisis in the United States, it is important to closely examine the epidemiologic trends of and racial and ethnic disparities in maternal cardiovascular health. METHODS: We used statewide birth hospitalization and vital statistics records from California (2007-2019; N=6 117 886) to examine the prevalence, trends, and racial and ethnic disparities of hypertensive disorders of pregnancy overall (ie, chronic hypertension, gestational hypertension/preeclampsia, or eclampsia), and chronic hypertension and gestational hypertension/preeclampsia separately. …
