Shoulder capsule shrinkage and consequences on shoulder movements.

A study was designed to find landmarks to avoid restriction of range of motion or insufficient shortening of the ligaments after thermal shoulder capsule shrinkage. In 15 nonembalmed shoulders from cadavers, shrinkage was done in three steps: (1). the middle glenohumeral ligament, (2). the anterior part of the inferior glenohumeral ligament, and (3). the posterior part of the glenohumeral ligament. Before and after each step passive range of motion of the glenohumeral joint was studied. Results indicated that flexion and the external rotation were not affected. External rotation in the scapula

Diagnostic yield and clinical outcomes of capsule endoscopy.

BACKGROUND: Capsule endoscopy is proving to be superior to push enteroscopy and barium contrast radiography for evaluation of the small bowel. However, its impact on clinical outcome has not been thoroughly investigated. This study assessed changes in therapy based on capsule endoscopy findings and on the impact of such changes on patient outcomes. METHODS: Forty-four consecutive capsule endoscopies in 43 patients were reviewed. Data were collected by systematic review of patient records and included indication, results of prior diagnostic tests, and capsule endoscopy findings. Specific inter

A new model of posterior capsule opacification in rodents.

PURPOSE: To describe a new model of posterior capsule opacification (PCO) in rodents METHODS: An extracapsular lens extraction (ECLE), by continuous curvilinear capsulorrhexis and hydrodissection, was performed in 42 consecutive Brown Norway rats. Animals were killed at 0, 6, and 24 hours and 3, 7, and 14 days after surgery. Eyes were enucleated and processed for light microscopy and immunohistochemistry. RESULTS: In 34 (81%) of the animals the operated eye appeared well healed before death, with a clear cornea and a well-formed anterior chamber. In eight (19%) there was no view of anterior

Painting technique for staining the anterior lens capsule.

We describe a technique of capsule staining called capsule painting. Using a Khokhar capsule painting cannula, the dye is spread directly over the anterior capsule but to no other part of the anterior chamber. There is minimal turbulence in the anterior chamber as there is no need to replace the injected viscoelastic material. The use of minimal dye and viscoelastic material reduces the cost of surgery.

Diabetic cervical radiculopathy with adhesive capsulitis of the shoulder.

The common form of diabetic neuropathy is symmetrical peripheral polyneuropathy, which involves the distal part of the lower extremities whereas diabetic amyotrophy is seen in the proximal part of the lower extremities. Although other regions may also be affected, the presence of upper extremity involvement has rarely been emphasized. Diabetic radiculopathy may involve the cervical region before, after, or concurrently with lumbosacral radiculopathy. We report 2 rare cases of diabetic radiculopathy which involves the cervical region without involving the lumbosacral region. To our knowledge, t

Vaccine development for capsulate bacteria causing pneumonia.

Pneumonia strikes the extremes of the age spectrum, causing maximal death and disability in children and the elderly. Despite its worldwide impact, there is a paucity of epidemiologic data regarding its incidence and the causative organisms. The two leading causes of bacterial pneumonia in childhood are Streptococcus pneumoniae (SP) and Haemophilus influenzae type b (Hib). SP is the major cause of pneumonia beyond the newborn period. In neonates, Group B Streptococcus (GBS) remains a major cause of sepsis and pneumonia despite recent reductions due to targeted perinatal antibiotic prophylaxis.

True exfoliation of the lens capsule following uveitis.

PURPOSE: To report a rare case of true exfoliation of the lens capsule following panuveitis, with slit-lamp photographs before and following the development of the true exfoliation. METHODS: Case report. Review of the history and clinical features of a 70-year-old woman who developed true exfoliation during the treatment for panuveitis. RESULTS: After 7 months of treatment for bilateral panuveitis with topical steroid and tropicamide, the patient developed a thin, transparent membrane arising on the lens surface in the right eye. A diagnosis of true exfoliation of the lens capsule was made.

Ciliary body detachment caused by capsule contraction.

A 74-year-old woman developed capsule contraction associated with hypotony and choroidal effusion 18 months after uneventful phacoemulsification with 3-piece poly(methyl methacrylate) intraocular lens implantation. Ultrasound biomicroscopy revealed ciliary body detachment and stretched zonules. A radial neodymium: YAG anterior capsulotomy was performed, resulting in the resolution of the ciliary body detachment and choroidal effusion as well as in normal intraocular pressure over 4 days.

Rupture of the anterior lens capsule in Alport syndrome.

Alport syndrome is an inherited disorder of type IV collagen, a major constituent of basement membranes. Eighty-five percent of cases are transmitted through X-linked dominant inheritance, although autosomal dominant and autosomal recessive inheritance has also been reported. Clinical manifestations of Alport syndrome include progressive glomerulopathy, sensorineural deafness, anterior lenticonus, posterior corneal dystrophy, and abnormal retinal pigmentation. Anterior lenticonus may lead to loss of vision because of progressive myopia or cataract formation. We report 2 cases of unusual catara

Capsular opacification after cataract surgery and capsule.

The lens capsule, the keystone of modern cataract surgery, has been the subject of some important and interesting investigations over the past year. This review addresses 1) inhibition of posterior capsule opacification (surgical techniques, intraocular lens design, and pharmacologic methods); 2) treatment of posterior capsule opacification; and 3) characteristics of the capsule. Of special interest are techniques for inhibiting posterior capsule opacification in pediatric patients by optic capture through a posterior capsulorhexis, the concept that sharp optic edges markedly decrease posterio

Prevention of posterior capsule opacification with the CO2 laser.

BACKGROUND AND OBJECTIVE: Posterior capsule opacification (PCO) is a common complication after cataract extraction, despite the modern surgical techniques and lenses being used for this procedure. Its prevention challenged many investigators, because the current treatment of choice, capsulotomy with Nd:YAG laser, is associated with sight-threatening complications. In the present study, the authors investigated two approaches of preventing PCO using the CO2 laser. MATERIALS AND METHODS: A 15-W CO2 laser with a 17- or 18-gauge hollow probe was used on 20 sheep eyes and 14 rabbit eyes. Lens extr

The ultrastructure of the lens capsule abnormalities in Alport's syndrome.

The ultrastructure of lens capsule abnormalities in Alport's syndrome is reported. An anterior lens capsule from a 29-year-old patient with lenticonus who was affected by Alport's syndrome was obtained at the time of surgery. The histopathologic findings showed the thickness of the anterior lens capsule was decreased and there were many vertical capsular dehiscences localized at the inner part of the lens capsule. Almost every dehiscence was limited to the inner two thirds of the capsule. One should be cautious in attempting intraocular lens implantation into the lens capsule of patients with

Cinearthrography of the Bankart lesion and anterior capsule elongation.

Diagnostic cinearthrography was performed on 3368 patients with shoulder pain during the period of 1982 through 1992. Cinearthrography identified a Bankart lesion in 128 shoulders on which surgical stabilization was performed for recurrent anterior dislocation. The operative pathologic condition for each of these patients was correlated and analyzed with the measurements of the Bankart lesion identified on the cinearthrogram. A small Bankart lesion was seen in patients with anterior capsular elongation. A larger Bankart lesion was observed in patients who did not have anterior capsular elongat

Lens capsule opacification in aphakic and pseudophakic eyes.

BACKGROUND: Posterior capsule opacification (PCO) is the most common complication of lens extraction. Although intraocular lenses (IOLs) are thought to inhibit capsule opacification, the mechanisms by which they do this are poorly understood. This study was done to determine the effects of pseudophakia on secondary cataract and PCO in experimentally lentectomized dogs. METHODS: Twenty-four normal dogs were bilaterally lentectomized by phacoemulsification and unilaterally implanted with a plano-convex polymethylmethacrylate IOL. Secondary cataracts and capsule opacification were evaluated at w

[Insertion of the Tenon capsule onto the rectus muscles].

The localisation and definition of the capsule of Tenon insertion on the rectus muscles is of prime importance to the strabismus surgeon. The depth of the superior sub-Tenon space was measured on 22 adult human eyes after fixation in formalin. Although the measurements revealed major differences between the eyes, the following results were found: inferior rectus muscle = 7.8 mm +/- 1.79 (5.2-13.0), medial rectus = 12.6 mm +/- 2.15 (8.4-17.2), superior rectus muscle = 16.7 mm +/- 2.39 (12.7-21.5), and lateral rectus muscle = 19.0 mm +/- 2.19 (14.5-22.0). Histological studies of 2 other human ey

Autonomous telemetric capsule to explore the small bowel.

An intestinal telemetric capsule has been developed to study the small bowel in man. It consists of a cylinder (11 mm in diameter and 39 mm in length) containing a location detector, a radiotransmitter, a lithium battery and an interchangeable tip. After having been swallowed by the patient, the capsule passes through the whole gut and is recovered in the stools. During the transit through the small bowel, the information provided by the radiotransmitter allows continuous monitoring of the distance covered from the pylorus, the direction and the velocity of progression. Moreover, according to

[What becomes of the interposed capsule in Colonna's arthroplasty?].

A total hip replacement had to be performed 25 years after a Colonna arthroplasty. The acetabulum has been cut off for examination (articular cartilage and subchondral bone). The evolution of the case is detailed. Histological examination shows that the capsular tissue became articular cartilage, but not a normal one. This observation confirms that Colonna arthroplasty can give a good joint for more than 20 years but not longer.
