[Surgical treatment of chronic constrictive pericarditis].

From April, 1969 to December, 1988, partial pericardiectomy was performed in 78 patients with chronic constrictive pericarditis. The long-term follow-up study in 60 patients showed a satisfactory functional result. The authors considers that the median sternotomy is a better approach, the effectiveness of operation should be gauzed by dropping the vena cave pressure below 140 mmH2O, resection of pericardium from left to right is logical, and rational use of cardiotonic agents is important.

[Isolated mitral valve insufficiency in comparison with mitral-tricuspid insufficiency: various mechanisms of compensating for the defect and functional status of the myocardium].

A combined analysis of ventricular contractility and intracardiac hemodynamic compensatory mechanisms was carried out, on the basis of angiocardiographic findings, in 37 patients with rheumatic mitral incompetence. Atrial fibrillation aggravates essentially the defect's hemodynamics, while added tricuspid incompetence is accompanied by a certain off-loading in the lesser circulation network. A grossly perversed phasic structure of intramyocardial stress was noted, apparently being a compensatory mechanism. Reduced specific coronary flow and diastolic perfusion gradient in intact coronary arter

Haemodynamic changes with blood transfusion in chronic severe anaemia.

Circulatory behavior in chronic, severely anaemic patients on volume loading is not precisely known. Twenty young male subjets with hook-worm anaemia, (Hb 2 to 5 gm %), without any complications were transfused with 300 or 600 ml of whole blood at 3 6 or ml/mt. Haemodynamic study was done before and immediately after. Blood volume was low, intracardiac pressures normal or minimally abnormal, cardiac output raised and vascular resistances low. After transfusion, there was a small but significant rise in arterial and mixed venous oxygen content, oxygen transport, heart rate, pulmonary wedge and

How can assessing hemodynamics help to assess volume status?

In critically ill patients, fluid infusion is aimed at increasing cardiac output and tissue perfusion. However, it may contribute to fluid overload which may be harmful. Thus, volume status, risks and potential efficacy of fluid administration and/or removal should be carefully evaluated, and monitoring techniques help for this purpose. Central venous pressure is a marker of right ventricular preload. Very low values indicate hypovolemia, while extremely high values suggest fluid harmfulness. The pulmonary artery catheter enables a comprehensive assessment of the hemodynamic profile and is par

Systemic and renal hemodynamics in oliguric hepatic failure: effect of volume expansion.

Systemic and renal hemodynamics were studied by indicator dilution techniques before and after infusion of 500 ml of dextran 40 in 21 patients with renal failure developing in the course of decompensated cirrhosis. Cardiac index was directly correlated with total blood volumes. Renal blood flow was low and renal vascular resistance elevated in 13 of 15 patients. Renal vascular resistance was directly related to total systemic nonrenal vascular resistance. Two patients with the highest cardiac outputs in the group were oliguric with high renal blood flow. Plasma volume expansion increased cardi

Effect of Exercise on Coronary Hemodynamics Under the Earth's Gravity and Microgravity Conditions: A CFD Study.

Exposure to microgravity affects the cardiovascular system and fluid distribution in the vasculature system of the human body depending on the short term or long term exposure. Literature suggests that in the initial few days in the microgravity environment, a short term exposure, the cardiac output and plasma volume are altered. Gradually, after about a week of long exposure to the microgravity environment, these parameters return to the same levels as those under the Earth's gravity. In this computational fluid dynamics (CFD) study, we investigate the effect of microgravity environment on co

Cardiovascular pathophysiology of essential hypertension: a clue to therapy.

Arterial hypertension is by definition a haemodynamic disorder. At least 3 different subsets of cardiovascular pathophysiological features can be identified in so-called essential hypertension: The young lean patient characterised by an elevated cardiac output and renal blood flow, elevated plasma renin activity and circulating catecholamine levels, as well as symptoms and signs of hyperadrenergic hypertension. The elderly patient characterised by a low cardiac output often with left ventricular hypertrophy, elevated total peripheral resistance, nephrosclerosis, and symptoms and signs of targe

Ventilation-perfusion distributions in the adult respiratory distress syndrome.

The distribution of ventilation-perfusion ratios, as determined by the multiple inert gas elimination technique, was measured in 16 patients with the adult respiratory distress syndrome. In each case, the distribution of ventilation-perfusion ratios was bimodal. The upper mode consisted of units with normal ventilation-perfusion ratios that received a mean of 52% of the cardiac output. The lower mode, which received the remainder of the cardiac output, consisted of either pure shunt or shunt plus a small number of units with very low ventilation-perfusion ratios (less than 0.01). The measured

Perioperative Mammillary Body Injury on MRI in Neonates with Severe Congenital Heart Disease.

BACKGROUND: Neonates with critical congenital heart disease (CCHD) are at risk for long-term cognitive impairments, including memory deficits. We aimed to assess the incidence and timing of mammillary body (MB) injury, critical memory structures, in neonates with CCHD and explore its association with cognitive outcome at 2 years of age. METHODS: We included neonates who underwent surgery with cardiopulmonary bypass in the first 6 weeks of life. Pre- and postoperative cerebral magnetic resonance imaging were reviewed for MB injury, defined as signal change on T1-, T2-, or diffusion-weighted im

Differential diagnosis of alterations in arterial flow and tissue oxygenation on venoarterial extracorporeal membrane oxygenation.

BACKGROUND: Venoarterial extracorporeal membrane oxygenation (VA-ECMO) may be life-saving in several clinical situations, but it is also one of the most invasive therapeutic procedures, with significant potential for life-threatening complications. Pulse pressure waves are typically very small or even absent at the onset of ECMO therapy, and will reappear with the improvement of cardiac function. A low pulse pressure may indicate low cardiac output due to heart failure during sustained ECMO support. A sudden loss of pulse pressure during ECMO therapy, however, may reveal complications like per

Postoperative arrhythmias in open-heart surgery, A study on fifty cases.

50 consecutive patients undergone open heart surgery were analyzed regarding postoperative arrhythmias in the first postoperative 3 days. Disturbances of rhythm occurred in each case of our group, serious or not serious (100%). Ventricular premature beats were the most frequent type of arrhythmia in the first and second postoperative days (80%). Two cases expired postoperatively. In one of them complete atrioventricular block developed after double valvular replacements (mitral and tricuspid). The other died of low cardiac output syndrome. Etiology of the arrhythmias after open heart surgery i

Cardiopulmonary performance at rest and during exercise in seven patients with increased transradiancy of one lung ("Macleod's syndrome").

A group of seven patients with increased transradiancy of one lung included four with chronic bronchitis. The function of the two lungs combined was assessed by measurement of the lung volumes, airway resistance, and carbon monoxide uptake at rest, and by measurement of pulmonary ventilation, gas exchange, blood gases, and cardiac output at rest and during steady state exercise. The contribution of the hypertransradiant lung to the overall functional abnormalities at rest was estimated using a radioactive gas technique. All the patients had airway obstruction. Pulmonary gas exchange was only m

Advanced hemodynamic monitoring during post-resuscitation care after out-of-hospital cardiac arrest: a scoping review.

AIM: Advanced hemodynamic monitoring may provide clinical information during early post-resuscitation care after out-of-hospital cardiac arrest (OHCA), but practices, treatment targets, and responses to abnormal values remain incompletely defined. We aimed to map the available evidence on advanced hemodynamic monitoring after OHCA. METHODS: We conducted a scoping review in accordance with a prespecified protocol and the PRISMA-ScR methodology. Eligible studies included hospitalized adults after OHCA and reported advanced hemodynamic variables beyond routine arterial blood pressure monitoring.

Experimental analysis of the onset mechanism of TdP reported in an LQT3 patient during pharmacological treatment with serotonin-dopamine antagonists against insomnia and nocturnal delirium.

Torsade de pointes (TdP) occurred in a long QT syndrome type 3 (LQT3) patient after switching perospirone to blonanserin. We studied how their electropharmacological effects had induced TdP in the LQT3 patient. Perospirone hydrochloride (n = 4) or blonanserin (n = 4) of 0.01, 0.1, and 1 mg/kg, i.v. was cumulatively administered to the halothane-anesthetized dogs over 10 min. The low dose of perospirone decreased total peripheral vascular resistance, but increased heart rate and cardiac output, facilitated atrioventricular conduction, and prolonged J-Tpeakc. The middle dose decreased mean blood

Severe ischemic early liver injury after cardiac surgery.

BACKGROUND: The epidemiology, pathogenesis and prognosis of severe ischemic early liver injury (SIELI) after cardiac surgery are poorly understood. Accordingly, we studied patients whose alanine transaminase (ALT) concentration acutely increased above 500 IU/l in the immediate postoperative period and compared these patients to two control groups matched for preoperative and immediate postoperative characteristics. METHODS: We used a prospective database of 1,800 consecutive cardiac surgical cases to identify the study groups. Group I was made up of 20 patients with ALT levels above 500 IU/L

[Volume expansion during septic shock].

At its very early phase, septic shock is characterized by severe hypovolemia related to abundant fluid losses, increased venous compliance and maldistribution of extracellular fluid (interstitial edema, splanchnic pooling). It results in a drop in venous return and cardiac output. Volume expansion, with crystalloids or colloids produces a hyperdynamic state with high cardiac output and low peripheral resistances. The quantity of liquid administered rather than its composition is the main determinant. Clinical examination and hemodynamic monitoring (Swan-Ganz catheter) appear to provide the mos

Constant-flow ventilation during experimental left ventricular failure.

The efficacy of constant-flow ventilation (CFV) was investigated in dogs with normal heart function (control phase, n = 8) and after development of left ventricular failure (LVF phase, n = 8). Heated, humidified and oxygen-enriched air (inspired oxygen fraction (Fio2) = 0.4) was continuously delivered via two catheters positioned within each mainstem bronchus at two flow rates (1.2 and 1.6 l/kg/min). Conventional mechanical ventilation (CMV) with positive end-expiratory pressure (PEEP) of 0.5 kPa was used as reference ventilation. During control, neither CMV with PEEP nor CFV revealed severe i

Evaluation of levosimendan as treatment option in a large case-series of preterm infants with cardiac dysfunction and pulmonary hypertension.

Levosimendan as a calcium-sensitizer is a promising innovative therapeutical option for the treatment of severe cardiac dysfunction (CD) and pulmonary hypertension (PH) in preterm infants, but no data are available analyzing levosimendan in cohorts of preterm infants. The design/setting of the evaluation is in a large case-series of preterm infants with CD and PH. Data of all preterm infants (gestational age (GA) < 37 weeks) with levosimendan treatment and CD and/or PH in the echocardiographic assessment between 01/2018 and 06/2021 were screened for analysis. The primary clinical endpoint was

Prolapse of aortic intimal flap into the left ventricle: a rare cause of global myocardial ischemia in acute type A aortic dissection.

A 62-year-old woman experienced an acute type A aortic dissection complicated with profound shock caused by acute myocardial ischemia. Intraoperative transesophageal echocardiography (TEE) identified a circumferentially dissected intimal flap at 5.5 cm above the aortic valve, prolapsing into the left ventricle through the aortic valve during diastole and obstructing both coronary ostia. Acute aortic dissection must be kept in mind when presented with myocardial ischemia and TEE is the most useful method for detecting a prolapsing cylindrical intimal flap.
