Effects of L-carnitine supplementation on lipid profiles in patients with coronary artery disease.

BACKGROUND: L-carnitine (LC) plays an important physiologic role in lipid metabolism. To date, no clinical study has been performed to examine the effect of LC supplementation on the lipid status of coronary artery disease (CAD) patients. The aim of this study was to investigate the lipid lowering effects of LC supplementation (1000 mg/d) in CAD patients. METHODS: CAD patients were identified by cardiac catheterization as having at least 50 % stenosis of one major coronary artery. Forty-seven subjects were recruited and randomly assigned to the placebo (n = 24) and to the LC (n = 23) groups. …

Contractile function and energy metabolism of skeletal muscle in rats with secondary carnitine deficiency.

The consequences of carnitine depletion upon metabolic and contractile characteristics of skeletal muscle remain largely unexplored. Therefore, we investigated the effect of N-trimethyl-hydrazine-3-propionate (THP) administration, a carnitine analog inhibiting carnitine biosynthesis and renal reabsorption of carnitine, on skeletal muscle function and energy metabolism. Male Sprague-Dawley rats were fed a standard rat chow in the absence (CON; n = 8) or presence of THP (n = 8) for 3 wk. Following treatment, rats were fasted for 24 h prior to excision of their soleus and EDL muscles for biochemi…

The Carnitine Palmitoyl Transferase (CPT) System and Possible Relevance for Neuropsychiatric and Neurological Conditions.

The carnitine palmitoyl transferase (CPT) system is a multiprotein complex with catalytic activity localized within a core represented by CPT1 and CPT2 in the outer and inner membrane of the mitochondria, respectively. Two proteins, the acyl-CoA synthase and a translocase also form part of this system. This system is crucial for the mitochondrial beta-oxidation of long-chain fatty acids. CPT1 has two well-known isoforms, CPT1a and CPT1b. CPT1a is the hepatic isoform and CPT1b is typically muscular; both are normally utilized by the organism for metabolic processes throughout the body. There is…

Effects of L-carnitine and pentoxifylline on carbohydrate distribution of mouse testicular sperm membrane.

BACKGROUND: The glycoconjugate content of sperms indicates their physiological and fertility properties. Lectin reactivity is indicative of intact, capacitated, and acrosome-reacted sperms. In the epididymis, sperms experience maturation, glycoconjugate modification, and simultaneously, higher L-carnitine (LC) concentrations. The aim of this project was to evaluate the effects of LC and Pentoxifylline (PF) on the integrity, capacitation, and acrosomal reaction of sperms by studying their lectin reactivity. METHODS: Mouse testicular sperm samples were divided into three parts. Each sample was …

[Carnitine deficiency with valproate sodium therapy--the difference by normal diet and enteral nutrition].

OBJECTIVE: Valproate sodium (VPA) treatment is known to reduce blood carnitine levels; however, some enteral nutrition formulas contain carnitine. The objective of this study was to verify the hypothesis that blood carnitine levels are lower after VPA treatment in patients on enteral nutrition than in those on normal diet. METHODS: Forty-five epilepsy patients under medical treatment in our hospital were classified according to their alimentation type into normal diet group (n = 31) and 14 on enteral nutrition group (n = 14). The differences in the blood free carnitine levels between the two …

Novel mutations in myopathic form of carnitine palmitoyltransferase II deficiency in a Chinese patient.

BACKGROUND: Carnitine palmitoyltransferase II (CPT II) deficiency is one of the most common disorders of oxidative fatty acid metabolism. In this disorder, long-chain acylcarnitines cannot be converted to acyl CoA and there is impairment of β-oxidation of fatty acids. RESULTS: In the 3 distinct clinical subtypes of CPT II deficiency, adult onset myopathic form shows mild clinical manifestations, characterized by recurrent rhabdomyolysis after intense physical stress. In this study, we report a case with adult myopathic form of CPT II deficiency presenting recurrent exercise-induced myoglobinu…

Important role of ventromedial hypothalamic carnitine palmitoyltransferase-1a in the control of food intake.

Carnitine palmitoyltransferase-1 (CPT-1) liver isoform, or CPT-1a, is implicated in CNS control of food intake. However, the exact brain nucleus site(s) in mediating this action of CPT-1a has not been identified. In this report, we assess the role of CPT-1a in hypothalamic ventromedial nucleus (VMN). We stereotaxically injected an adenoviral vector containing CPT-1a coding sequence into the VMN of rats to induce overexpression and activation of CPT-1a. The VMN-selective activation of CPT-1a induced an orexigenic effect, suggesting CPT-1a in the VMN is involved in the central control of feeding…

Concentrations of carnitine, glutamate and myo-inositol in epididymal fluid and spermatozoa from boars.

This study examines the concentrations of carnitine, glutamate and myo-inositol in fluid and spermatozoa from six epididymal regions. Samples were taken from six post-pubertal boars, and the sperm concentration, the protein concentration in epididymal fluid and the concentrations of carnitine, myo-inositol and glutamate in the epididymal fluid and spermatozoa were analysed. In epididymal fluid the concentration of myo-inositol decreased in a proximo-distal direction, whereas intraluminal concentrations of L-carnitine and L-glutamate increased distally. As changes in the concentration of these …

A variable myopathy associated with heterozygosity for the R503C mutation in the carnitine palmitoyltransferase II gene.

Adult-onset carnitine palmitoyltransferase II (CPT II) deficiency is an autosomal recessive disease characterized by muscle pain and stiffness with rhabdomyolysis and myoglobinuria in severe cases. Exercise, fasting, viral infection, anesthesia, or extremes in temperature may trigger symptoms. A 54-year-old woman exhibited a 35-year history of progressive weakness and myopathic symptoms. CPT II activity in the patient's lymphoblasts, cultured skin fibroblasts, and skeletal muscle was reduced to 47, 43, and 13% of normal, respectively. Respiratory chain enzymes were also reduced in muscle rangi…

Labour analgesia in a patient with carnitine palmityl transferase deficiency and idiopathic thrombocytopenic purpura.

We report a case of a woman with carnitine palmityl deficiency (CPT) and idiopathic thrombocytopenic purpura, presenting in active labour at 38 weeks gestation. We discuss different anaesthetic factors involved with both diseases, and we propose an optimal management of such cases. Neuraxial analgesia with minimal motor blockade is indicated in early labour because it is necessary to alleviate stress in order to avoid rhabdomyolisis associated with CPT deficiency. Neuraxial analgesia is also needed because the theoretical risk of performing a caesarean section is higher than in a normal popula…

Effect of acute acetyl-l-carnitine treatment on daytime melatonin synthesis in the rat.

The effects of acute treatment of young male rats with 350 mg/kg of acetyl-l-carnitine (ALCAR) at 15.00 hr on the synthesis and secretion of melatonin by the pineal gland and the retinas were studied 1 hr after injection. The pineal N-acetyltransferase (NAT) activity was unchanged when compared with the control. However, there was a non-significant but slight increase in the melantonin content of the pineal glands in the ALCAR-treated rats. The serum level of melantonim in the ALCAR-treated group was significantly higher (P < 0.05) when compared with the control. The melatonin content of the r…
