[Suboccipital Pott's disease].

A new case of sub-occipital Pott's disease is reported: a 26 year old, coloured male patient from Senegal was admitted for: cervical pain of 6 month duration, neck stiffness, dysphagia, left XII paralysis and left hemiparesis. The radiological study found an anterior atlanto-axial subluxation, basilar impression, and increased thickness of the retropharyngeal soft tissue, lysis of the left occipital condyle and the left lateral mass. CT scan study revealed a retropharyngeal mass and an epidural contrast-enhanced lesion at the C2 C3 C4 levels. Chest-X ray showed one tuberculous lesion at the ri

Airway management in a displaced comminuted fracture of the mandible and atlas with a vertebral artery injury: A case report.

Complex cervical spine fractures are a serious complications of maxillofacial trauma and associated with high mortality and neurological morbidity. Strict vigilance in preventing further insult to the cervical spine is a crucial step in managing patients who are at risk for neurologic compromise. We report a rare case of a right transverse process of atlas fracture with right-sided vertebral artery injury that was associated with a comminuted fracture of the body and angle of the mandible, which restricted mouth opening. Airway management was performed by an awake fiber-optic nasotracheal intu

Posterior screw placement on the lateral mass of atlas: an anatomic study.

STUDY DESIGN: An anatomic study evaluating the feasibility of posterior screw fixation on the lateral mass of the atlas and comparing the pullout force of C1 posterior lateral mass screws with that of occipital and C2 pedicle screws. OBJECTIVES: To study the feasibility, anatomy, and biomechanics of posterior screw fixation within the lateral mass of the atlas. SUMMARY OF BACKGROUND DATA: Occipitocervical fusion is a common choice for atlantoaxial dislocations. After that the movement of occipitocervical region will be restricted. If screws could be placed in the lateral mass of the atlas, 1

Multiple Fractures of Cervical Vertebrae Combined with Arcuate Foramen and Vertebral Artery Occlusion: A Case Report and Literature Review.

BACKGROUND: The arcuate foramen is a complete or partial bony bridge over the vertebral artery groove of atlas. The mechanism of the arcuate foramen is not clearly understood. Omission of the arcuate foramen sometimes causes lethal iatrogenic injury during spinal surgery. CASE PRESENTATION: We describe a patient who was diagnosed with multiple fractures of the cervical vertebrae, arcuate foramen, and right vertebral artery occlusion based on clinical and radiological exams. After conservative treatment, he resumed a normal and productive life. CONCLUSIONS: Arcuate foramen is a common variati

Plough fracture of the anterior arch of the atlas: a biomechanical investigation.

PURPOSE: "Plough" fracture, in which the odontoid ploughs through and causes a high-energy shear fracture of the anterior arch of the atlas, has been documented in clinical case studies and classified as clinically unstable. Our objectives were to develop a biomechanical model to simulate atlantal plough fracture and investigate injury mechanisms. METHODS: Horizontally aligned head impacts into a padded barrier were simulated using a human upper cervical spine specimen (occiput through C3) mounted to a surrogate torso mass on a sled and carrying a surrogate head. We divided 13 specimens into

Development of dysphagia and trismus developed after c1-2 posterior fusion in extended position.

Cervical misalignment after upper cervical fusion including the occipital bone may cause trismus or dysphagia, because the occipito-atlanto joint is associated with most of the flex and extended motion of the cervical spine. There are no reports of dysphagia and trismus after C1-2 fusion. The purpose of this paper is to demonstrate the potential risk of dysphagia and trismus even after upper cervical short fusion without the occipital bone. The patient was a 69-year-old man with myelopathy caused by os odontoideum and Klippel-Feil syndrome, who developed dysphagia and trismus immediately after

Traumatic spondylolisthesis of the axis: a biomechanical comparison of clinically relevant anterior and posterior fusion techniques.

OBJECT: Surgical management of unstable traumatic spondylolisthesis of the axis includes both posterior and anterior fusion methods. The authors performed a biomechanical study to evaluate the relative stability of anterior fixation at C2-3 and posterior fixation of C-1 through C-3 in hangman's fractures. METHODS: Fresh-frozen cadaveric spine specimens (occipital level to T-2) were subjected to stepwise destabilization of the C1-2 complex, replicating a Type II hangman's fracture. Intact specimens, fractured specimens, and fractured specimens with either anterior screw and plate or posterior

Transmandibular approach for upper cervical pathologies: report of 2 cases and review of the literature.

In routine surgical practice, anterior approaches are not often used to treat upper cervical pathologies. Such lesions can be difficult to access surgically. This article describes 2 cases in which the transmandibular approach was used to address anterior upper cervical pathology. One case was a chordoma invading the C2-C3 vertebrae and the other case was atlanto-axial instability. Neurological examination revealed myelopathy in both cases. Each patient had already undergone occipito-cervical fusion at a different center and, thus, had limited neck extension and mouth-opening ability. In the f

Posterolateral dislocation of the C1-C2 articulation associated with fracture of the anterior arch of C1: a case report.

STUDY DESIGN: Case report. OBJECTIVE: To report a rare traumatic C1-C2 dislocation associated with fracture of the anterior arch of the atlas in a neurologically intact patient. SUMMARY OF BACKGROUND DATA: Isolated fractures of the anterior arch of C1 are very rare. There have been reports of horizontal fractures of the anterior arch thought to occur secondary to hyperextension injuries with subsequent avulsion of the anterior tubercle of the atlas. To our knowledge, however, there are no previously reported cases of isolated anterior arch fractures of C1 associated with posterolateral dislo

Management of combination fractures of the atlas and axis in adults.

STANDARDS: There is insufficient evidence to support treatment standards. GUIDELINES: There is insufficient evidence to support treatment guidelines. OPTIONS: Treatment of atlas-axis combination fractures based primarily on the specific characteristics of the axis fracture is recommended. External immobilization of most C1--C2 combination fractures is recommended. C1--Type II odontoid combination fractures with an atlantodens interval of 5 mm or more and C1--hangman's combination fractures with C2--C3 angulation of 11 degrees or more should be considered for surgical stabilization and fusion

Traumatic atlantoaxial distraction injury: a case report.

STUDY DESIGN: Case report. OBJECTIVES: Description of a rarely reported variant of traumatic C1-C2 dislocation and discussion of a favorable outcome in a nonsurgical treatment approach. SUMMARY OF BACKGROUND DATA: Atlantoaxial dislocation most commonly involves an anterior movement of C1 in relation to C2. Often, the integrity of the transverse ligament or odontoid process is compromised, and the atlantodental interval changed. The described patient sustained a purely craniocaudal atlantoaxial distractive lesion secondary to injury sustained in a high-speed motor vehicle accident. RESULTS:

Reconstruction of C-1 lateral mass with titanium mesh cage after resection of an aneurysmal bone cyst of the atlas.

Aneurysmal bone cysts (ABCs) are rare benign tumors with a prevalence of 0.14 cases per 100,000 people. A majority of cases arise during adolescence, and there is a female predominance. This lesion accounts for 1.4% of all primary bone tumors. Aneurysmal bone cysts occur mainly in the long bones, with spinal involvement in 10-30% of cases. Cervical spine ABCs account for about one-third of spinal ABCs, and atlas involvement occurs in 1% of cases. Resection of ABCs at the atlas is difficult because of the location and the lack of proper instrumentation for reconstruction of C-1. The authors pre

Anatomical variations of the atlas arches: prevalence assessment, systematic review and proposition for an updated classification system.

OBJECTIVE AND BACKGROUND: This study focuses on the atlas, a pivotal component of the craniovertebral junction, bridging the cranium and spinal column. Notably, variations in its arches are documented globally, necessitating a thorough assessment and categorization due to their significant implications in clinical, diagnostic, functional, and therapeutic contexts. The primary objective is to ascertain the frequency of these anatomical deviations in the atlas arches among a Colombian cohort using cone-beam computed tomography (CBCT). METHODOLOGY: Employing a descriptive, cross-sectional approa

Defining hyoplasia of the atlas: a cadaveric study.

STUDY DESIGN: Cadaveric study. OBJECTIVE: To define congenital hypoplasia of the atlas. SUMMARY OF BACKGROUND DATA: Little has been written about hypoplasia of the atlas and it is usually described in the setting of other skeletal dysplasias or syndromes. METHODS: A total of 543 cervical spine specimens were randomly selected from the Hamann-Todd collection. Sagittal and coronal diameters of the atlas, axis, and C3 (when available), and the dens diameter were measured using digital calipers. Correction for modern size and radiographical magnification was performed. Hypoplasia of the atlas w

[Cervical involvement in rheumatoid arthritis (author's transl)].

The authors studied 577 records of rheumatoid arthritis examined radiologically in the Department of Radiology B of Cochin Hospital: 165 patients showed evidence of lesions of the cervical spine. The risk of cervical involvement in rheumatoid arthritis is thus not negligeable: 28%. This involvement may occur at any time during the course of the disease and presents as pain or by rare and more or less slight neurological manifestations. Radiologically emphasis must be placed upon the orientation points of straight and lateral films, and the value of lateral films with the neck in flexion. Atlan

Dysplasia of the odontoid process in Morquio's syndrome causing quadriparesis.

Eleven patients with documented Morquio's syndrome who had dysplasia of the odontoid process and resulting atlanto-axial instability were reviewed. They were found to be at risk for acute traumatic quadriparesis, chronic myelopathy of a variable and often rapid rate of progression, and sudden death by respiratory arrest. The evidence suggests that early prophylactic posterior cervical fusion is indicated for patients with this malformation and that once quadriparesis is established, recovery of function is limited.
