Surgical management of thoracic outlet syndrome: a 10-year experience.

BACKGROUND: Thoracic Outlet Syndrome (TOS) refers to compression of the neurovascular structures in the region between the scalene muscles and the first rib, or by anatomical abnormalities such as cervical rib, fibrous bands and other variations in the scalene musculature. METHODS: Our experience with 63 consecutive operations for TOS, over a period of 10 years, has been reviewed. Preoperative symptoms and signs, investigations, surgery done, complications and the outcome of surgery are analysed. RESULTS: A total of 60 patients underwent 63 operations for decompression of TOS. All the 63 fir…

Addition of McKenzie Exercises to a Multimodal Physical Therapy Approach for Cervical Radiculopathy: A Case Report.

Cervical radiculopathy is a common subset of neck pain involving cervical nerve root irritation potentially resulting in numbness, radicular pain, and/or upper extremity weakness. The Neck Pain Clinical Practice Guidelines published by the American Physical Therapy Association does not support McKenzie exercises in isolation when treating cervical radiculopathy, but endorses a multimodal physical therapy approach for the management of cervical radiculopathy. The purpose of this case report is to exemplify the treatment of a patient with cervical radiculopathy utilizing McKenzie centralization

Characterization of a first thoracic rib ligament: anatomy and possible clinical relevance.

STUDY DESIGN: Cadaver dissection and measurement. OBJECTIVE: To describe a previously undocumented intracostal ligament that limits the potential space through which the T1 ventral ramus passes before joining the C8 ventral ramus. SUMMARY OF BACKGROUND DATA: Preclavicular entrapment of the T1 ventral ramus can lead to radiculopathy, neurogenic thoracic outlet syndrome, or both, the so called "double crush" phenomenon. The usual sites of entrapment include the neural foramen, the interscalene interval, an aberrant cervical rib, the first rib itself, or an apical thoracic mass. METHODS: A tot…

Klippel-Feil syndrome associated with persistent trigeminal artery.

STUDY DESIGN: In the present report, radiologic findings of the patient with the Klippel-Feil syndrome associated with persistent trigeminal artery are reviewed. In same case, there was also spina bifida occulta, cervical spondylosis, atlanto-occipital assimilation, bilateral cervical ribs, and lack of flow signal in the C4 segment of contralateral vertebral artery. The persistent trigeminal artery and Klippel-Feil syndrome can be seen together and with the other anomalies. It should be considered that they might be related with the insufficiency or dysfunction of the embryologic developmental…

Paget-Schrotter syndrome and complications of management.

An 18-year-old man presented to the emergency department with an acutely swollen right upper limb having spent the previous day canoeing. Venography confirmed right subclavian venous thrombosis at middle one-third of right clavicle with no evidence of cervical rib or other structural abnormalities. Following heparinisation, catheter directed thrombolysis was performed which restored luminal flow. Postprocedure the patient developed spontaneous small-volume haemoperitoneum and acute renal failure. The patient was admitted to the intensive care unit for haemodialysis and supportive management. T…

Tracheostomy in trauma patients with rib fractures.

PURPOSE: Patients with rib fractures (RF) may require prolonged mechanical ventilation and tracheostomy. Indications for tracheostomy in trauma patients with RF remain debatable. The goal was to delineate characteristics of patients who underwent tracheostomy due to thoracic versus extra-thoracic causes, such as maxillofacial-mandibular injury (MFM), traumatic brain injury (TBI), and cervical vertebrae trauma (CVT), and to analyze clinical outcomes. The predictive values of chest trauma scoring systems for tracheostomy were also evaluated. We hypothesized that tracheostomized patients were mor

Imaging of cervicothoracic junction anatomical variation in neurogenic thoracic outlet syndrome: A scoping review protocol.

OBJECTIVE: The objective of this scoping review is to systematically map the extent and nature of the literature on the use of imaging for assessing cervicothoracic junction anatomical variation in patients with neurogenic thoracic outlet syndrome. INTRODUCTION: Neurogenic thoracic outlet syndrome is a complex, disputed diagnosis that lacks a definitive gold-standard test, despite being the most common thoracic outlet syndrome subtype. Imaging is essential for identifying anatomical variations, such as cervical ribs, and excluding competing diagnoses. This review aims to clarify how imaging m…

Axial skeletal morphological studies on the helmeted Guinea fowl (Numida meleagridis).

This study on the morphological features of bones of the helmeted guinea fowl (Numidia meleagridis) specifically the axial skeleton was conducted for documentation and valuable information on differences when compared with other species of the same order. Ten (10) Helmeted guinea fowls (5 females and 5 males) with an average weight of 3.0 kg were euthanized via jugular vein severance with bone preparation achieved via the use of insect larvae. The skull presented an elongated structure made up of paired and unpaired flat, short and long bones with distinctive features. The frontal bone project

The thoracic outlet syndromes.

The term thoracic outlet syndrome (TOS) refers to a heterogeneous group of disorders, all of which have in common compression of one or more neurovascular elements at some point within the thoracic outlet. Of the five disorders comprising this group, four have all of the features expected of a syndrome-a recognized constellation of clinical features; an anatomic derangement accounting for these features; and a method of testing that identifies the anatomic derangement. Consequently, their recognition and management are relatively straightforward. Conversely, one of these five disorders (nonspe

[Fibrous-muscular and vessels anomalies revealing during surgery of the scalenus anticus syndrome].

One hundred and twenty five operations on 100 patients were performed due to the symptoms suggesting presence of the pressure on the neurovascular supraclavicular structures, although X-ray showed no evidence of cervical rib. In 15 cases of patients with so-called scalenus anticus syndrome as one of the forms of thoracic outlet syndrome (TOS) seemed to be compressing the neurovascular bundle. Scalenus mediums muscle appeared to be producing more compression of the nerve roots than did the scalenus anterior in 5 patients. Pressure from Rudimentary First Rib--in 2 cases in our series malformatio…

Diagnostic importance of 3D CT images in Klippel-Feil Syndrome with multiple skeletal anomalies: a case report.

We present here the case of a 12-year-old boy who had Klippel-Feil syndrome with renal, cardiac and multiple skeletal anomalies, and we show the relevent three-dimensional computed tomography images. Our patient had a triple renal pelvis, mitral valve prolapsus, multiple cervical vertebrae fusions, cervical ribs, hypoplasia of the right thumb, spina bifida of L5, lumbalization at the right side of S1 and a sacral curved defect. In this study, we discuss the atypical clinical features and the diagnostic value of three-dimensional CT for evaluating the skeletal anomalies of the Klippel-Feil synd…

Arterial thoracic outlet syndrome secondary to hypertrophy of the anterior scalene muscle.

Thoracic outlet syndrome (TOS) was first introduced in literature by Peet et al. in 1956. Since then is has been studied extensively and sub … astly arterial. Arterial TOS comprises ~1% of all TOS cases, they are caused largely by presence of cervical rib and/or anomalies of first rib. Here we describe arterial TOS caused by hypertrophy of the anterior scalene muscle in a 65-year-old male. Patient underwent successful open surgical repair with Dacron interposition graft. Postoperative course was unre …

Pediatric cervical kyphosis in the MRI era (1984-2008) with long-term follow up: literature review.

OBJECTIVE: Cervical kyphosis is rare in the pediatric population. It may be syndromic or acquired secondary to laminectomy, neoplasia, or trauma. Regardless, this should be avoided to prevent progressive spinal deformity and neurological deficit. Long-term follow-up is needed to evaluate fusion status, spine growth, potential instability, and neurological function. METHODS AND MATERIALS: A retrospective review of 27 children (6 months to 16 years) with cervical kyphotic deformity was performed and limited to the MRI era until 2008, to provide a long-term follow-up after which complex instrume

Diagnostic possibilities for the nuchal evoked potential.

As a response to stimulation of the arm nerves an evoked potential can be recorded in the posterior neck region in man. This nuchal evoked potential (NEP) was recorded in three selected patients with A) cervical rib, B) syringomyelia, and C) prolapse of cervical intervertebral disc. Patients A and C had low-amplitudic NEPs with increased latencies when the affected side was stimulated and "normal" NEPs when the unaffected side was stimulated. Patient B had grossly abnormal, polyphasic NEPs (regardless of the side stimulated) but the simultaneously recorded somatosensory evoked potential was "n…

Defective neural tube closure and anteroposterior patterning in mice lacking the LIM protein LMO4 or its interacting partner Deaf-1.

LMO4 belongs to a family of transcriptional regulators that comprises two zinc-binding LIM domains. LIM-only (LMO) proteins appear to function as docking sites for other factors, leading to the assembly of multiprotein complexes. The transcription factor Deaf-1/NUDR has been identified as one partner protein of LMO4. We have disrupted the Lmo4 and Deaf-1 genes in mice to define their biological function in vivo. All Lmo4 mutants died shortly after birth and showed defects within the presphenoid bone, with 50% of mice also exhibiting exencephaly. Homeotic transformations were observed in Lmo4-n
