BACKGROUND: Choosing to breastfeed is a decision with far-reaching benefits; strengthened immune systems, all-encompassing nutrition, and fostering healthy attachment. For these and other reasons, some mothers are reluctant to wean their child due to a subsequent pregnancy. Mothers may breastfeed throughout their second pregnancy, give birth to their second child, and simultaneously breastfeed two children. This practice is known as tandem breastfeeding.
PURPOSE: We explore the biological and emotional considerations of tandem breastfeeding and offer practical suggestions for nurses.

BACKGROUND: The number of child adoptions from abroad is increasing, but the adverse living conditions of these children prior to the adoption raise questions on their medical and neurodevelopmental status, particularly since there are no guidelines for pre- or post-adoption medical evaluation.
OBJECTIVES: To describe the condition of a cohort of young children who were candidates for adoption in East European orphanages and foster homes, and to determine those attributes associated with a family's decision to adopt or refuse a particular child.
METHODS: Eighty-two young children, median age

This article examines Lucy LaFarge's "The Imaginer and the Imagined" through the lens of Winnicottian play. LaFarge's detailed clinical material shows how primitive fantasies about communication-beliefs that minds can omnipotently control or annihilate one another-become part of the analytic field and can get enacted bidirectionally to produce stalemates in the analytic process. This commentary compares Ms. O's adult analysis to a child case in which imaginative play opened access to fears of dependency and aggression. It underscores play's paradoxical, transitional quality as a therapeutic me

BACKGROUND: Engagement is essential for fostering learning and development in young children with disabilities, including those with autism spectrum disorder (ASD) and intellectual disability (ID).
METHODS: This study used a non-concurrent multiple-baseline across participants design to examine the effects of embedded learning opportunities (ELOs) on engagement and learning in four preschool-aged children diagnosed with ASD and ID.
RESULTS: Results showed a consistent increase in engagement for each child following the introduction of ELOs by teachers, with children generalising this engagem

The World Psychiatry Exchange Program offers opportunities overseas for early career psychiatrists (ECPs), fostering immersion in clinical and cultural contexts. In this article, we present the experiences of two Tunisian ECPs in India. Activities included observing interviews, and attending courses and webinars. Challenges and opportunities in perinatal psychiatry and in child psychiatry were observed, emphasising cross-cultural nuances. Language barriers were overcome through translation. Notably, collaboration and proximity between departments countered mental illness stigma among medical p

This study evaluated methods for training community-based clinicians to deliver a mentalization-based parenting intervention in an addiction treatment setting. Mothering from the Inside Out (MIO) targets psychological deficits associated with early stages of addiction recovery by fostering improvement in parental reflective functioning, the capacity to make sense of strong emotions in oneself and the child. Fifteen addiction counselors were randomized to training in MIO versus a Parent Education comparison, and completed eight training sessions and a clinically-supervised 12-session training c

Qualitative research methods were used to examine transition experiences of (a). 9 young adults with severe disabilities during their last year of high school, (b). their parents, and (c). professionals from schools and adult service agencies. Students were remarkably articulate about plans following graduation but had few opportunities to meaningfully fulfill them. Parents hoped their child's talents and abilities would allow them to achieve fulfilling adult lives, but faced uncertain outcomes and unfamiliar procedures. Professionals managed the process and approached transition by matching n

Young children removed from institutions and placed into foster care or adoptive homes have been shown to experience significant gains in IQ relative to children who remain in institutions. Less is known about the long-term impact of severe early deprivation on development in late childhood. Data are presented from a follow-up of children at 12 years of age in the Bucharest Early Intervention Project, a randomized clinical trial of foster care for institutionally reared children. Of the original 136 children in the study, 107 were tested with the Wechsler Intelligence Scale for Children-IV. Re

BACKGROUND: Mobility is a human right and play a key role for children's development and participation. Children with limited independent mobility do not have the same opportunities to explore their surroundings and interact in play. Research has shown that powered mobility can enable independent mobility for children as young as 12 months.
AIMS/OBJECTIVES: To explore occupational therapists experience with powered mobility for children under three years of age.
MATERIAL AND METHOD: Data was collected through three focus groups. A total of ten occupational therapists from ten different munic

BACKGROUND: Several studies have explored the mechanisms of intergenerational transmission of child maltreatment (ITCM), which also involves care-experienced parents; however, what is less explored is their direct experience, especially regarding resilience processes.
OBJECTIVE: Developing the theoretical framework of ITCM through an exploration of the perspectives of those who appear to have interrupted it.
PARTICIPANTS AND SETTING: A sample of 27 Italian parents - with experience in foster and/or residential care - who have broken the ITCM, completed an in-depth interview between May 2021

Children in foster care have exceptional needs due to their histories of abuse, neglect, and increased exposure to violence. The rates of psychiatric symptoms and disorders, such as attention-deficit/hyperactivity disorder, posttraumatic stress disorder, and reactive attachment disorder, are much higher in children in foster care; furthermore, the rate of these children receiving psychotropic medications is 3 times that of children who are not in foster care. Pediatricians, in their role of providing a medical home, play a central role in safeguarding the physical and mental health of these ch

IMPORTANCE: Severe neglect in early life is associated with compromises in brain development and associated behavioral functioning. Although early intervention has been shown to support more normative trajectories of brain development, specific improvements in the white matter pathways that underlie emotional and cognitive development are unknown.
OBJECTIVE: To examine associations among neglect in early life, early intervention, and the microstructural integrity of white matter pathways in middle childhood.

Physically neglected youth are at increased risk of mental health problems, but there are few interventions that have demonstrated efficacy in reducing mental health symptoms for this vulnerable population. The Fostering Healthy Futures (FHF) program, which consists of mentoring and skills groups, was developed for preadolescent youth in foster care. In a published randomized controlled trial with 156 youth, FHF demonstrated positive impacts on mental health functioning. The current study sought to determine whether FHF might be particularly effective in ameliorating the impact of neglectful f

This study replicated the Child Behavior Checklist factor structure of traumatic sequelae in maltreated children that was established by A. C. Hulette and colleagues (in press; see also A. Cholankeril et al., 2007). The factors represent dissociation and posttraumatic stress disorder symptomatology. The present study also examined the extent to which these 2 factor scores varied depending on specific maltreatment experiences. Results indicated that children who experienced both physical and sexual abuse in addition to neglect had significantly higher levels of dissociation than children who ex

This paper describes the support systems available for orphans in a rural Luo community in Nyang'oma sub-location in Bondo District of Western Kenya. Qualitative data were collected through in-depth interviews with orphaned children and their caretakers as well as key informants, and through focus group discussions with orphaned children, widows and community elders. Quantitative data were obtained by questionnaires administered to 100 caretakers of orphaned children. The most serious problem was inability of the orphan households to afford school fees, although lack of food, medicare and clot

Sleep problems in toddlers occur in ∼40% of children and increase the likelihood of postnatal depression. Most sleep training in toddlers requires contact with a trained professional, and requires a parent to ignore their child's cries, causing distress to many children and parents, increasing attrition and leaving families untreated and at risk. This case study reports success in significantly ameliorating sleep reluctance and bedtime fears in a sleep disturbed toddler with a history of trauma. It uses a novel use of bedtime behaviour management with some positive reinforcement techniques, ca

This study examined associations between alcohol misuse and childhood maltreatment and out-of-home placement among urban lesbian, gay, and bisexual (referred to as two-spirit) American Indian and Alaska Native adults. In a multi-site study, data were obtained from 294 individuals who consumed alcohol during the past year. The results indicated that 72.3% of men and 62.4% of women engaged in hazardous and harmful alcohol use and 50.8% of men and 48.7% of women met criteria for past-year alcohol dependence. The most common types of childhood maltreatment were physical abuse among male drinkers (

Previous research has established that child sexual abuse (CSA) and other forms of child maltreatment can have lasting and profound implications for survivors in terms of externalizing symptomatology. Few studies, however, have examined long-term consequences of CSA and maltreatment among adopted children. Guided by a polyvictimization framework, the present study investigated: (a) rates of co-occurrence of pre-adoptive CSA and maltreatment among adopted children, and (b) the relative impact of pre-adoptive CSA and maltreatment on externalizing behaviors at 14 years post-adoption. Analyses wer
