Opportunities and challenges for cholera control in India.

The Indo Gangetic delta is homeland for cholera for almost two centuries now and there is evidence of global spread from this area. With migration of people to more urban areas within the country and increase in international travel, it is time for action against cholera in India, given its capacity to present itself in epidemic proportions. The Global roadmap to end cholera by 2030 was launched by the WHO Global Task force for Cholera Control which has led to renewed vigor and convergence of stakeholders across the world against the disease. There is also an emphasis on cleanliness and improv…

Mitigating Cholera in the Aftermath of Cyclone Idai.

Catastrophic damage and floods followed the deadliest cyclone on record for the Southern Hemisphere. In the aftermath of Cyclone Idai, a cholera outbreak was detected. The global stockpile of oral cholera vaccine was rapidly deployed to counter this fast-growing epidemic. We urge the international community to continue to highlight the importance of water, sanitation, and hygiene as the long-term goal for controlling cholera and meeting the 2030 Sustainable Development Goals.

Spatiotemporal Patterns of Cholera Hospitalization in Vellore, India.

Systematically collected hospitalization records provide valuable insight into disease patterns and support comprehensive national infectious disease surveillance networks. Hospitalization records detailing patient's place of residence (PoR) can be utilized to better understand a hospital's case load and strengthen surveillance among mobile populations. This study examined geographic patterns of patients treated for cholera at a major hospital in south India. We abstracted 1401 laboratory-confirmed cases of cholera between 2000-2014 from logbooks and electronic health records (EHRs) maintained…

Risk structured model of cholera infections in Cameroon.

Since 1991, Cameroon, a cholera endemic African country, has been experiencing large cholera outbreaks and cholera related deaths. In this paper, we use a "fitted" demographic equation (disease-free equation) to capture the total population of Cameroon, and then use a fitted low-high risk structured cholera differential equation model to study reported cholera cases in Cameroon from 1987 to 2004. For simplicity, our model has no spatial structure. The basic reproduction number of our fitted cholera model, R0, is bigger than 1 and our model predicted cholera endemicity in Cameroon. In addition,…

Micro-Hotspots of Risk in Urban Cholera Epidemics.

Targeted interventions have been delivered to neighbors of cholera cases in major epidemic responses globally despite limited evidence for the impact of such targeting. Using data from urban epidemics in Chad and Democratic Republic of the Congo, we estimate the extent of spatiotemporal zones of increased cholera risk around cases. In both cities, we found zones of increased risk of at least 200 meters during the 5 days immediately after case presentation to a clinic. Risk was highest for those living closest to cases and diminished in time and space similarly across settings. These results pr…

Large cholera outbreak in Brong Ahafo Region, Ghana.

BACKGROUND: A nationwide outbreak of Vibrio cholerae occurred in Ghana in 2014 with Accra, the nation's capital as the epi-center. The outbreak spread to the Brong Ahafo Region (BAR) which is geographically located in the middle of the country. In this region a review of data collected during the outbreak was carried out and analyzed descriptively to determine the hot spots and make recommendations for effective response to future outbreaks. METHODS: A review of patient records and line lists of cases of cholera reported in all hospitals during the period of the outbreak (July-December 2014) …

Cholera - the new strike of an old foe.

Cholera is an acute bacterial gastrointestinal infection caused by ingestion of water or food containing the pathogen Vibrio cholerae. The incubation period can be very short and it takes between several hours and 5 days. During the 19th century, cholera was spreading from India across the world. Its original reservoir was located in the Ganges delta. So far, there have been six epidemics of cholera; the current outbreak is the seventh. It started in Asia, attacked Africa and then the Americas. Cholera causes thousands of illnesses and deaths annually, mostly in South Asia and Sub-Saharan Afri…

Factors associated with cholera in Kenya, 2008-2013.

INTRODUCTION: Kenya experienced widespread cholera outbreaks in 1997-1999 and 2007-2010. The re-emergence of cholera in Kenya in 2015 indicates that cholera remains a public health threat. Understanding past outbreaks is important for preventing future outbreaks. This study investigated the relationship between cholera occurrence in Kenya and various environmental and demographic factors related to water, sanitation, socio-economic status, education, urbanization and availability of health facilities during the time period 2008-2013. METHODS: The primary outcome analyzed was the number of cho…

Updated global burden of cholera in endemic countries.

BACKGROUND: The global burden of cholera is largely unknown because the majority of cases are not reported. The low reporting can be attributed to limited capacity of epidemiological surveillance and laboratories, as well as social, political, and economic disincentives for reporting. We previously estimated 2.8 million cases and 91,000 deaths annually due to cholera in 51 endemic countries. A major limitation in our previous estimate was that the endemic and non-endemic countries were defined based on the countries' reported cholera cases. We overcame the limitation with the use of a spatial …

Global stability of an age-structured cholera model.

In this paper, an age-structured epidemic model is formulated to describe the transmission dynamics of cholera. The PDE model incorporates direct and indirect transmission pathways, infection-age-dependent infectivity and variable periods of infectiousness. Under some suitable assumptions, the PDE model can be reduced to the multi-stage models investigated in the literature. By using the method of Lyapunov function, we established the dynamical properties of the PDE model, and the results show that the global dynamics of the model is completely determined by the basic reproduction number R0: i…

John Snow and cholera--the bicentenary of birth.

The bicentenary of John Snow's birth, a legend in his field, for his research on epidemiology and the prevention ofcholera, constitutes a unique opportunity to commemorate this iconic figure. In the article, his spectacular achievements in this discipline are presented, including his epidemiological investigation during cholera epidemic and the well-known Broad Street intervention in Soho, in 1854, as well as his methodologically elegant experiment "on the grandest scale" in which he compared the cholera fatality rates in households served by two different water supply companies. Having referr…
