Even though fractures of the clavicle are very common but fracture of the shaft of clavicle associated with sternoclavicular joint dislocation is extremely rare. This is a case report of a 50-year old woman who met with a road accident. Radiographs revealed right mid shaft clavicle fracture with inferior angulation of fracture fragments, anterior dislocation of sternoclavicular joint. The sternoclavicular joint was stabilized with sutures whereas the midshaft fracture was managed non-operatively. In postoperative period the sternoclavicular joint was found stable whereas the shaft clavicle uni…

We report a case of a 28-year-old female who presents with painless, progressively increasing left upper chest swelling of 6 months duration. Examination revealed a 20 by 10 cm firm mass arising from the clavicle. She was investigated and a diagnosis established based on the histological report. The tumour was excised with a wide margin and reconstruction done with an autogenous fibula graft. Full functional ability was restored in limb with the radiological union. After 5 years of regular follow-up, there was no evidence of recurrence and shoulder function remained excellent. The successful m…

BACKGROUND: Intraoperative neurovascular complications with clavicle fracture fixation are often due to far cortex penetration by drills and screws, but could be avoided using a unicortical construct. The objective of this study was to compare the bending and torsional strength of a unicortical locking screw plate construct and a hybrid (with central locked and outer non-locked long oblique screws) unicortical plate construct for clavicle fracture fixation with that of a conventional bicortical locking screw construct of plate fixation.
METHODS: Twenty-four human clavicle specimens were harve…

INTRODUCTION: Risks associated with drill plunging are well recognised in clavicle osteosynthesis. To date no studies have described plunge depth associated with clavicle osteosynthesis.
PRIMARY AIM: To determine whether plunge depth associated with clavicle osteosynthesis is great enough to penetrate neurovascular structures and whether surgical experience reduces the risk of neurovascular injury METHOD: Cadaveric clavicles were pressed into spongy phenolic foam to allow measurement of drill bit penetration beyond the far cortex (plunge depth). 15 surgeons grouped according to experience wer…

A 57-year-old woman with Marfan syndrome had undergone the surgical treatment for pectus excavatum at 15 years of age. She had since been screened regularly by computed tomography(CT) to detect any vascular diseases. CT demonstrated a left subclavian arterial aneurysm at 55 years of age. It enlarged to a diameter of 32 mm, and surgical treatment was performed. The subclavian artery and the aneurysm were identified by echography in order to locate the exact sites of supraclavicular and subclavicular skin incisions. The subclavian artery aneurysm was resected without dividing the clavicle, and r…

Retrosternal displacement of the medial clavicular metaphysis after a medial physeal fracture is a rare and potentially fatal injury because of possible damage of neurovascular structures, trachea and oesophagus. As the medial clavicular epiphysis does not ossify until 18-20 years of age and the medial clavicular physis is the last to close, around 22-25 years of age, this injury is difficult to demonstrate with plain radiographs in younger patients and is often mistaken for a sternoclavicular joint dislocation. We report on a 16-year-old boy with a fracture of the medial clavicular physis wit

Traditionally, the belief is that the clavicle heals readily and with nonoperative treatment. Since evidence suggests that many clavicles benefit from operative care, the aim of this study was to find the incidence of complications after operative treatment of clavicle fractures. This retrospective study includes 56 patients (41 male and 15 female) who were treated operatively for clavicle fractures between 2010 and 2012. The indications were displacement more than 20 mm, comminution, tenting of the skin, floating shoulder, and open fractures. Seventy-five percent of the fractures occurred in …

BACKGROUND: Clavicle fractures are common, accounting for 2.6% to 4% of all fractures. Eighty per cent of clavicle fractures are located in the middle third of the clavicle. Although treatment of these fractures is usually non-surgical, displaced clavicle fractures may be considered for surgical treatment because of their greater risk of non-union.
OBJECTIVES: To assess the effects (benefits and harms) of surgical versus conservative interventions for treating middle third clavicle fractures.
SEARCH METHODS: We searched the Cochrane Bone, Joint and Muscle Trauma Group Specialised Register (t…

OBJECTIVE: The aim of this study was to assess the outcome of coracoclavicular screw fixation in displaced distal clavicular fractures.
METHODS: Sixteen patients with Neer Type 2 displaced distal clavicle fractures were treated with open reduction and internal fixation. Fixation was performed using a coracoclavicular screw with washer application. After union the screw was removed under local anesthesia. Radiographic evaluation was performed and the clinical outcome was assessed using the Constant score.
RESULTS: All patients achieved painless osseous unions. However, one patient showed sign…

From 1995 to 2003, 39 semi-professional athletes (34 men and 5 women) aged 17-34 years (mountain bike racers, soccer players, swimmers and cyclists) with a displaced fracture of the middle-third of the clavicle were treated operatively using rigid plate fixation within one week after the initial trauma. In 90% of the athletes, radiographic union was achieved after 12 weeks. At 6 weeks post-operatively the mean Constant score was 88 points, the mean visual analogue pain score was 3 out of 10 and the average time for return to sports was 45 days. Seventy-five percent of the patients were very sa…

INTRODUCTION: Osteochondroma, a frequent benign tumor of the bone, is in most cases asymptomatic. Localization in the clavicle is extremely rare. The authors report an isolated case occurring in the coraco-clavicular area responsible for a painful shoulder syndrome evoking rotator cuff tendinitis.
METHODS: A 47 years female patient consulted for painful shoulder syndrome. Pain occurred following an effort and at night. On clinical examination, the area around the coracoid process was tender as was the supraspinatus fossa. Passive anterior elevation of the shoulder was limited to 150 degrees. …

We analyzed clavicular radiographs of 26 patients with a history of trauma. The apical oblique projection of the clavicle was obtained with the injured side of the patient angled 45 degrees towards the X-ray tube and a 20 degrees cephalad angulation of the X-ray beam. This view proved to be more informative than the routine apical anteroposterior projection. It is especially effective in detecting nondisplaced fractures of the middle third of the clavicle in neonates and children. To verify our findings, we obtained apical anteroposterior and oblique radiographs of a specimen adult clavicle. O…

We have reported the case of a nonunion of a clavicle fracture in which the Dacron had eroded the bone and acted as a stress riser contributing to the fracture. The Dacron then became interposed in the fracture site preventing union.

A new operative approach is presented for treatment of fractured distal end of clavicle associated with recurrent anterior shoulder subluxations. One case is presented in which open reduction of the fractured clavicle and stabilizing the anterior shoulder joint were accomplished by transfer of the coracoid process with its attached muscles under the subcapularis muscle onto the clavicle. The operation was followed by complete reduction of the fracture and a stable shoulder joint.

Surgery of acromioclavicular luxations and lateral fractures of the clavicula with the Balser plate is described as a reliable low-complication method on the basis of the authors' own experiences in 13 patients. This treatment method offers significant advantages over the other conventional methods of surgery. Early mobilization of the patients is possible if exercise stability is present. Costly and complicated immobilization can be omitted, and so can reconstruction of the coracoclavicular ligaments or syndesmoplasty. The period of disability is shortened.

During four years, the authors have observed ten lesions of the sub-clavian artery associated with a fracture of the clavicle. The clinical picture in one case out of two was that of an acute ischemia of the upper limb. The surgical repair has been performed in 8 cases. The thoracic approach was necessary three times. The clavicle was resected in most cases. The prognosis of these lesions depends on the rapidity of their recognition, and of their treatment, and on the fact whether the brachial plexus is involved or not
