Clinical framework to guide operative decision making in disconnected left pancreatic remnant (DLPR) following acute or chronic pancreatitis.

BACKGROUND: Disconnected left pancreatic remnant (DLPR) presents clinically as a pancreatic fistula, pseudocyst, or obstructive pancreatitis. Optimal operative treatment, either distal pancreatectomy (DP) or internal drainage (ID), remains unknown. This paper critically evaluates our operative experience in patients with DLPR. METHODS: A retrospective analysis of a consecutive case series from a single, high-volume institution was carried out. A total of 76 patients with radiographic-confirmed DLPR (computed tomography + endoscopic retrograde cholangiopancreatography or magnetic resonance cho

[Clinical thinking and decision making in practice. A 7-year old boy with rapid growth and pubic hair].

A 7-year-old boy was tall, with a developing penis and initial growth of pubic hair. Due to the accelerated growth there was no premature adrenarche and due to the increased testicular volume there must have been a central cause for the production of androgens. Further specific investigations revealed an astrocytoma of the hypothalamus. After radiotherapy, the tumour exhibited no growth. The pubertal development was inhibited and the growth later stimulated by means of medication. In the case of pubertas praecox the growth chart and pubertal stages including testicular volume can help to diffe

Assessing Clinical Decision-Making Aided by a RAG-Based Dialog in Pre-Examination on Non-Odontogenic Tooth Pain.

Differentiating Non-Odontogenic Tooth Pain, a potential symptom of life-threatening conditions like Ischemic Heart Disease, is a critical challenge for dentists, as existing AI tools fail to support their decision-making. To address this, we developed a chatbot using a Retrieval-Augmented Generation (RAG) enhanced Large Language Model (LLM). This study protocol outlines a trial to evaluate the impact of the AI's outputs on dentists' diagnostic accuracy. The study employs a three-arm, within-subjects crossover trial where 128 dentists evaluate simulated patient scenarios under three conditions:

Causal Machine Learning for Personalized Surgical Decision-Making in Aortic Dissection: A National Fairness-Aware Clinical AI Framework.

BACKGROUND: Most machine learning (ML) models for aortic dissection predict mortality but do not address treatment-effect heterogeneity, fairness, or bedside implementation. We developed a causal-inference-informed framework integrating prediction, heterogeneous treatment-effect estimation, fairness auditing, and individualized risk-benefit profiling. METHODS: Using 2016-2022 Healthcare Cost and Utilization Project National Inpatient Sample discharge data, we analyzed 45,112 aortic dissection hospitalizations. Five ML models, plus a weighted ensemble, were trained on 2016-2019 data (n = 24,47

Reliability and device objectivity in oscillatory blood pressure measurement - a measurement error analysis to inform clinical decision making.

The implementation of different hypertension thresholds (e.g. ESC/ESH: 140:90 or ACC/AHH: 130:80) substantially increased hypertension prevalence donation of pharmacological treatment across various populations. Under narrow margins of only 10mmHg, measurement accuracy and reliability become decisive for clinical decision-making. However, detailed information on systematic and random errors naturally occurring in clinical practice under repeated testing conditions remain widely limited. To provide practically relevant insights, 65 participants were recruited for visited the lab four times with …

Model-Informed Decision Making from Drug Discovery to Reimbursement: A Roadmap for Clinical Pharmacology in Value-Based Healthcare.

The transition to value-based healthcare demands more efficient, evidence-driven approaches to drug discovery, development and medical use. Clinical pharmacology is uniquely positioned to support this shift through model-informed drug development (MIDD) decision-making, integrating quantitative methods across the product lifecycle. This commentary outlines a roadmap for embedding MIDD from early drug discovery through regulatory evaluation and ultimately into reimbursement and real-world use. We highlight how pharmacokinetic/pharmacodynamic modeling, mechanistic modeling, disease progression m

"A Transformational redesign of the global clinical workforce decision-making processes: Novel cognitive schemes embracing modern healthcare paradigms".

BackgroundGlobal healthcare systems continue to face a persistent quality gap driven by fragmented and inconsistent clinical decision-making. Although competency-based medical education (CBME) frameworks define clinician roles, they provide limited practical guidance and tools for real-time decision-making. Additionally, contemporary literature failed to incorporate modern healthcare paradigms into everyday clinical decision-making.ObjectiveTo develop a structured, competency-aligned, seven-step cognitive roadmap that standardizes clinical decision-making primarily for high-stakes, acute care.…

Patellar management in primary total knee arthroplasty: A narrative review and evidence-based framework for clinical decision-making.

Patellar management in primary total knee arthroplasty (TKA) remains one of the most debated and variable aspects of contemporary arthroplasty practice. Patellar resurfacing, non-resurfacing, and selective resurfacing each offer distinct advantages, but no universally superior strategy has emerged. Although global functional outcomes are generally comparable across techniques, differences persist in anterior knee pain, survivorship, patellofemoral degeneration, and the likelihood of secondary patellar procedures. This narrative review synthesizes current clinical, radiographic, biomechanical,

Artificial Intelligence impact on decision-making regarding short-term ADT with prostate radiotherapy: ASTuTE clinical trial interim results.

This interim analysis of a multicentre implementation trial (n = 200) evaluated a multimodal artificial intelligence biomarker's impact on shared short-term androgen deprivation therapy (ST-ADT) decisions in intermediate-risk prostate cancer (IR-PC). Biomarker testing altered 27.5% (CI: 21.4-34.2%) of final decisions. ST-ADT use significantly decreased: 70.3% (CI: 58.5-80.3%) of patients initially electing ST-ADT changed to 'NO', versus a 2.4% (CI: 0.5-6.8%) shift to 'YES' (p < 0.001). Decision changes were significantly greater in unfavourable versus favourable IR-PC (35% v 4%; p < 0.001). Th

Cognitive bias in veterinary clinical decision-making: Antemortem misdiagnosis of suspected giant kidney worm (Dioctophyme renale) in Australia.

A 10-year-old male neutered Rottweiler was referred for evaluation of suspected Dioctophyme renale infection following haematuria, lethargy, coughing and passage of a worm-like structure in the urine. The presumptive diagnosis was strongly influenced by the referring veterinarian's prior experience with D. renale in an endemic region. Diagnostic imaging identified left renal abnormalities including hydronephrosis, a dilated ureter containing a tubular intraluminal structure and multiple tubular structures within the urinary bladder; these observations further reinforced the initial clinical im

Combined Assessment of HPV and p16 Optimizes Clinical Decision-Making in Head and Neck Cancer of Unknown Primary.

BACKGROUND: Human papillomavirus-associated oropharyngeal squamous cell carcinoma (HPV+ OPSCC) is increasing in Sweden with implications for both the incidence and management of HPV+ head and neck cancer of unknown primary (HNCUP). In OPSCC, the combined presence of HPV DNA (HPV DNA+) and p16INK4a (p16+) overexpression has been shown to provide superior prognostic information compared with either marker alone. In this study, we therefore aimed to assess whether the prognostic impact of HPV DNA and p16 concordance/discordance in HNCUP mirrors that observed in OPSCC, within a high HPV-prevalent

Predicting major amputation risk in diabetic foot ulcers using comparative machine learning models for enhanced clinical decision-making.

It is to develop a predictive model utilizing machine learning techniques to promptly identify patients with diabetic foot ulcers (DFU) who may require major amputation upon their initial admission. A total of 598 DFU patients were admitted to a tertiary hospital in Beijing. We employed synthetic minority oversampling technique to address the class imbalance of the target variable in the original dataset. A Lasso regularization analysis identified 17 feature variables for inclusion in the model: age, diabetes duration, wound size, history of peripheral neuropathy, history of atrial fibrillatio

This study compared individuals with Restrictive Anorexia Nervosa (R-AN; n = 40), Healthy Controls (HCs; n = 45), and individuals at risk for eating disorders (RI; n = 38) using a Reinforcement Learning (RL) paradigm. Participants completed a Probabilistic Reversal Learning (PRL) task involving food-related and neutral contexts. The study examined whether RL impairments in R-AN are context-specific and whether they reflect maintaining factors or preclinical markers. R-AN participants showed reduced learning rates in food-related contexts compared to HC and RI but performed similarly in neutral
