Transforming GP practices into multiprofessional primary care centers: The role of nursing professionals (PRIMA) - study protocol for a multicenter non-randomized, controlled, parallel-group intervention study.

BACKGROUND: Germany faces increasing challenges in primary care due to demographic change, workforce shortages, and the growing prevalence of chronic conditions. Integrating nursing professionals into primary care teams may improve continuity, comprehensiveness, and efficiency of care, but evidence for the German setting remains limited. The PRIMA project (DRKS00039104) implements a new care model that transforms general practices into multiprofessional primary care centers integrating specially trained nursing professionals. The objective is a productivity- and quality-oriented reorganization

Report on the Second International Workshop on interim positron emission tomography in lymphoma held in Menton, France, 8-9 April 2010.

One hundred and fifty hemato-oncologists and nuclear medicine specialists from more than 20 countries joined in April 2010 the 2-day Second International Workshop on interim PET in lymphoma. During the nuclear medicine session the advantages of the five-point scale Deauville criteria for interim PET reporting over the other sets of visual criteria were presented. The specific problems of PET reporting in escalation/de-escalation trials in Hodgkin lymphoma (HL) were addressed as well as the limitations of visual analysis for early PET evaluation in non-Hodgkin lymphoma (NHL). The applicability,

Muscle imaging in facioscapulohumeral muscular dystrophy research: A scoping review and expert recommendations.

Clinical trial readiness is an important topic in the field of facioscapulohumeral muscular dystrophy (FSHD). As FSHD is a slowly progressive and clinically heterogeneous disease, imaging biomarkers have been proposed to complement clinical outcome measures. Muscle magnetic resonance imaging (MRI), ultrasound and dual energy X-ray absorptiometry (DEXA) have been used to measure disease severity, activity and progression. We conducted a scoping review of the literature on these imaging modalities to assess gaps in knowledge and subsequently collaborated with a panel of neuromuscular imaging exp

Does mobile phone instructional video demonstrating sputum expectoration improve the sputum sample quality and quantity in presumptive pulmonary TB cases? Protocol for a prospective pragmatic non-randomised controlled trial in Karnataka state, India.

INTRODUCTION: Sputum smear microscopy is the cornerstone of tuberculosis (TB) diagnosis under the Revised National Tuberculosis Control Programme (RNTCP) in India. Instructions on how to produce a good sputum sample are a part of RNTCP training manuals, but its assessment is not emphasised. Healthcare provider's instruction to expectorate a good sputum sample has limitations. Presumptive TB patients often submit inadequate (in quantity and/or quality) sputum samples, which may result in false-negative results. Objectives of the study are, among the selected RNTCP designated microscopy centres

Prospective observational cohort study of factors influencing trial participation in people with motor neuron disease (FIT-participation-MND): a protocol.

INTRODUCTION: Motor neuron disease (MND) is a rapidly progressive and fatal neurodegenerative disorder with limited treatment options. The Motor Neuron Disease Systematic Multi-Arm Randomised Adaptive Trial (MND-SMART) is a multisite UK trial seeking to address the paucity in effective disease-modifying drugs for people with MND (pwMND). Historically, neurological trials have been plagued by suboptimal recruitment and high rates of attrition. Failure to recruit and/or retain participants can cause insufficiently representative samples, terminated trials or invalid conclusions. This study inves

AmBisome in the treatment of fungal infections: the UK experience.

Despite a number of recent advances in diagnosis and treatment strategies, fungal infections remain an important cause of morbidity and mortality in neutropenic and immunosuppressed patients. The main fungal infections are candidosis and aspergillosis, although an increased incidence of rarer fungal infections including Fusarium spp., Aurebasidium pullulans and Mucor spp. has been noted. Mortality may be decreased with earlier diagnosis and aggressive treatment with lipid-associated amphotericin B. Invasive pulmonary aspergillosis (IPA) in the neutropenic host presents particular challenges in

Red-hot chili receptors: A systematic review of TRPV1 antagonism in animal models of psychiatric disorders and addiction.

Transient Receptor Potential Vanilloid 1 (TRPV1) channels are non-selective cationic polymodal receptors gated by several different chemical and physical stimuli. TRPV1 receptors are distributed in several brain areas and interact with important neurotransmitter systems linked to mental disorders, such as endocannabinoid and opioid systems. The increasing number of results obtained in this field has recently attracted growing attention to these receptors as potential targets for the treatment of different psychiatric conditions. To review the available results on this topic, we searched on Pub

Psychiatric Case Management in the Emergency Department.

PURPOSE: The care of the mentally ill has reached a real crisis in the United States. There were more than 6.4 million visits to emergency departments (EDs) in 2010, or about 5% of total visits, involved patients whose primary diagnosis was a mental health condition or substance abuse (). That is up 28% from just 4 years earlier, according to the latest figures available from the Agency for Healthcare Research and Quality in Rockville, MD. Using a method called scoping, the purpose of this article is to examine the range, extent, and evidence available regarding case management as an intervent

TRansfusion strategies in Acute brain INjured patients (TRAIN): a prospective multicenter randomized interventional trial protocol.

BACKGROUND: Although blood transfusions can be lifesaving in severe hemorrhage, they can also have potential complications. As anemia has also been associated with poor outcomes in critically ill patients, determining an optimal transfusion trigger is a real challenge for clinicians. This is even more important in patients with acute brain injury who were not specifically evaluated in previous large randomized clinical trials. Neurological patients may be particularly sensitive to anemic brain hypoxia because of the exhausted cerebrovascular reserve, which adjusts cerebral blood flow to tissue

A double-blind, placebo-controlled, randomised, multicentre, proof-of-concept and dose-finding phase II clinical trial to investigate the safety, tolerability and efficacy of adrecizumab in patients with septic shock and elevated adrenomedullin concentration (AdrenOSS-2).

INTRODUCTION: Sepsis remains a major health problem with an increasing incidence, high morbidity and high mortality. Apart from treatment with antibiotics and organ support, no approved specific adjunct therapies currently exist. Adrenomedullin (ADM) is a vasoactive peptide. High plasma concentrations of ADM correlate with worse outcome in sepsis patients. Preclinical work with the non-neutralising ADM-binding antibody adrecizumab showed promising effects in animal models of septic shock, including improved vascular barrier function, reduced vasopressor demand and organ dysfunction and increas
