Hemorrhagic events in cancer patients treated with aflibercept: a meta-analysis.

Aflibercept (Ziv-aflibercept, VEGF Trap, AVE005) is an engineered protein that functions as a decoy receptor to bind vascular endothelial growth factor A (VEGF-A). Hemorrhagic events, including epistaxis, gastrointestinal bleeding, and pulmonary bleeding, is one of its major adverse effects, but the incidence rate and overall risk has not been systematically studied. Therefore, we conducted a meta-analysis of published clinical trials to investigate the incidence and relative risk of hemorrhagic events in cancer patients treated with aflibercept. Electronic databases including PubMed, Embase,

Long-term safety assessment of live attenuated tetravalent dengue vaccines: deliberations from a WHO technical consultation.

Dengue is a rapidly growing public health threat with approximately 2.5 billion people estimated to be at risk. Several vaccine candidates are at various stages of pre-clinical and clinical development. Thus far, live dengue vaccine candidates have been administered to several thousands of volunteers and were well-tolerated, with minimal short-term safety effects reported in Phase I and Phase II clinical trials. Based on the natural history of dengue, a theoretical possibility of an increased risk of severe dengue as a consequence of vaccination has been hypothesized but not yet observed. In O

Is there a palliative benefit of gemcitabine plus fluoropyrimidines in patients with refractory colorectal cancer? A review of the literature previously presented: poster at the 2008 Gastrointestinal Cancer Symposium (Abstract No. 512).

INTRODUCTION: Fluoropyrimidines with oxaliplatin or irinotecan plus bevacizumab is the standard chemotherapy combination in patients with advanced colorectal cancer (CRC). Gemcitabine acts synergistically with fluoropyrimidines to enhance the binding of thymidylate synthase and increase inhibition of DNA synthesis. The objective of this review is to evaluate the literature for evidence of efficacy and safety of fluoropyrimidine plus gemcitabine (FG) in patients with advanced CRC. METHODS: Relevant studies were identified in PubMed, Ovid, Cochrane database and the American Society of Clinical

Simeprevir with peginterferon/ribavirin for treatment of chronic hepatitis C virus genotype 1 infection: pooled safety analysis from Phase IIb and III studies.

This pooled analysis of five Phase IIb and III studies evaluated the safety and tolerability of simeprevir, a once daily, oral hepatitis C virus (HCV) NS3/4A protease inhibitor. Data were summarised for patients who received simeprevir 150 mg once daily (n = 924) or placebo (n = 540) plus pegylated interferon-α/ribavirin for 12 weeks. During the first 12 weeks of treatment, few patients discontinued simeprevir or placebo due to adverse events (AEs) (both 2.2%). Pruritus (23.8% vs 17.4%), rash (any; 22.9% vs 16.7%) and photosensitivity (3.2% vs 0.6%) [Correction added on 16 January 2015, after

Bevacizumab for advanced colorectal cancer.

Bevacizumab is a recombinant humanized monoclonal antibody that targets vascular endothelial growth factor (VEGF). It is thought that bevacizumab inhibits the formation of new blood vessels. Two clinical trials show that the addition of bevacizumab to a regimen of either fluorouracil plus leucovorin (FL) or FL combined with irinotecan (IFL), significantly improves response rate and time to tumour progression and increases overall survival for patients with advanced colorectal cancer (ACC). Thromboembolic events are the most clinically significant adverse events, but hypertension, hemorrhage an

Very Low-Carbohydrate Breakfast Intervention for Adults with Type 2 Diabetes and Persistent Hyperglycemia: Protocol for a Digital, Nonrandomized Pre-Post Study.

BACKGROUND: More than 15% of US adults with type 2 diabetes have persistent hyperglycemia. Adults with persistent hyperglycemia and type 2 diabetes have an elevated health risk of a variety of outcomes, including amputation and mortality from cardiovascular disease and from all causes. Nutrition-focused interventions can be effective for improving glycemic control, reducing antihyperglycemic medications, and reducing body weight, all of which are critical outcomes for adults with type 2 diabetes. Carbohydrate intake impacts postprandial glycemia more than any other dietary factor. The American

TITE-LOCRM12: A Local Continual Reassessment Method for Drug Combination Optimization Based on Late-Onset Toxicity and Efficacy Outcomes.

The primary goal of Phase I/II combination trials is to identify the optimal biological dose combination (OBDC), defined as the dose combination that achieves the highest efficacy while maintaining an acceptable toxicity rate. Several designs have been proposed for such trials. Among them, the local continual reassessment method for Phase I/II trials (LOCRM12) performs well when compared with other model-based methods in most of the scenarios studied. However, LOCRM12 assumes that both toxicity and efficacy outcomes are fully observed before enrolling the next cohort, an assumption that may no

Platform Assessment of Concentration-QTc Relationship Across GalNAc-siRNA Molecules.

BACKGROUND: GalNAc-conjugated short interfering RNAs (GalNAc-siRNAs) lack a biologically plausible mechanism for QT/QTc prolongation based on their unique physicochemical and absorption, distribution, metabolism, and excretion (ADME) properties. Nevertheless, regulatory agencies require characterization of QTc effects via either a thorough QT (TQT) study or a concentration-QTc (C-QTc) analysis using early-phase data. This manuscript summarizes platform-level experience across eight GalNAc-siRNAs to assess QTc prolongation risk via C-QTc analysis. METHODS: Time-matched electrocardiogram (ECG)

Depletion of interfering IgG and IgM is critical to determine the role of IgE in pegvaliase-associated hypersensitivity.

Pegvaliase is an enzyme substitution therapy developed to lower blood phenylalanine (Phe) in adults with phenylketonuria (PKU). In phase 3 clinical studies, pegvaliase substantially reduced mean blood Phe in adult subjects with PKU. The most common type of adverse event observed in the pegvaliase clinical program was hypersensitivity adverse events (HAEs), which included occurrences of arthralgia, rash, and pruritis. The most clinically relevant HAEs were acute systemic hypersensitivity reactions consistent with anaphylaxis observed in 4.6% of phase 3 patients. HAEs were more commonly observed

Anti-IL-6 treatment for inflammatory bowel diseases: next cytokine, next target.

Inflammatory bowel diseases (IBD) are chronic, relapsing, and destructive inflammatory disorders of the gastrointestinal tract. Both Crohn's disease (CD) and ulcerative colitis (UC) seem to arise from an impaired dialog between the environment and gut microbiota in genetically susceptible hosts, leading to an inappropriate immune activation and resulting in the over-production of pro-inflammatory cytokines. IL-6 is a key modulator of inflammatory response. It is a phylogenetically important cytokine with relevance in IBD, as well as in other chronic inflammatory diseases and cancer. Influencin

[Treatment of B-cell lymphoma with a chimeric anti-CD20 monoclonal antibody(rituximab)].

CD20 antigen is expressed on nearly all human B-cells and B-lymphoma cells. Rituximab is a chimeric anti-CD20 monoclonal antibody with mouse variable and human constant regions. The toxicities of rituximab are mainly infusion-related, non-hematological grade 1 or 2 episodes. Of the 11 eligible patients enrolled in the phase I study in Japan, 2 showed CR and 5 showed PR. 90 relapsed pts were enrolled in the subsequent phase II study and treated with rituximab at 375 mg/m2 x 4 weekly infusions. The overall response rates in relapsed indolent B-cell lymphoma and mantle cell lymphoma were 61% (37/

Tomoxetine (Eli Lilly & Co).

Eli Lilly is developing tomoxetine, a norepinephrine reuptake inhibitor, for the potential treatment of attention deficit hyperactivity disorder (ADHD) and depression. As of May 2000, tomoxetine was undergoing phase III trials in the US [368128]. An NDA was filed with the FDA in October 2001, with a launch expected in the second half of 2002 [426786]. Tomoxetine was first investigated by Lilly in the 1980s as a potential treatment for depressive illness. The compound was selected from a series of potent inhibitors of norepinephrine reuptake, and reached large-scale phase II clinical trials for

Innovative approach for the clinical development of a Chlamydia trachomatis vaccine through a human challenge model in women.

Chlamydia trachomatis remains the most common sexually transmitted bacterial infection worldwide. Recent phase I trials of the C. trachomatis vaccine candidate CTH522 in women and men have shown that the adjuvanted and non-adjuvanted formulations are safe and elicit systemic (immunoglobulin G) and mucosal (immunoglobulin A) antibodies, as well as vaccine-specific cell-mediated immunity. In advance of a standard phase II randomized controlled trial to determine the vaccine's efficacy, this perspective advocates for a carefully and ethically designed human challenge model as an innovative approa
