Early reoccurrence of traumatic posterior atlantoaxial dislocation without fracture: A case report.

BACKGROUND: In general, atlantoaxial dislocation is rare due to the stability of the C1-C2 complex. Traumatic atlantoaxial dislocations are usually anterior and accompanied by odontoid fractures. Posterior atlantoaxial dislocations are rare, and complete posterior dislocation without associated fracture is even more rare. A case of early recurrence of posterior atlantoaxial dislocation without fracture being in therapy of first closed reduction and then open reduction has not been previously reported. CASE SUMMARY: A 45-year-old female presented with traumatic posterior atlantoaxial dislocati

Posterior Fibular Entrapment in a Bosworth Fracture-Dislocation: A Case Report and Review of Diagnostic and Operative Considerations.

INTRODUCTION: Bosworth fracture-dislocations are rare ankle injuries characterized by posterior entrapment of the fibula behind the tibia, making closed reduction inherently difficult. Their subtle radiographic appearance often delays diagnosis. This case highlights the diagnostic challenges and operative considerations associated with this uncommon and frequently irreducible injury pattern. CASE REPORT: A 34-year-old man presented after a fall with a posterior malleolus fracture, distal fibular fracture, and tibiotalar dislocation. Closed reduction improved alignment, but persistent mortise

Intramedullary nailing for paediatric diaphyseal forearm bone fracture.

BACKGROUND: Though most of the pediatric diaphyseal forearm bone fracture can be treated with closed reduction and cast application, indications for operative intervention in pediatric both-bone forearm fractures include open fractures, irreducible fractures, and unstable fractures. Controversy exists as to what amount of angulation, displacement, and rotation constitutes an acceptable reduction. OBJECTIVE: To review union time and functional outcome of pediatric diaphyseal forearm bone fracture managed with intramedullary rush pin by closed or open reduction. METHODS: Fifty patients with bo

Surgical indications for distal tibial epiphyseal fractures in children.

The goal of this study was to investigate the treatment methods and surgical indications of distal tibial epiphyseal fractures in children. Two hundred eighty-six children with distal tibial epiphyseal fractures were included in the study. Among these patients, 202 were male and 84 were female. Mean age was 11.7 years. A retrospective study on the postoperative long-term complications and related risk factors was performed. Treatment methods were determined according to the distance of fracture displacement. A long-leg cast was applied after closed reduction for patients with primary fracture

Treatment of long bone fractures in South American camelids: 5 cases.

An adult llama and four alpacas were referred with long bone fractures. The llama presented with a closed, comminuted fracture of the right metatarsal bones. Two of the alpacas presented with comminuted fractures of the proximal radius. One fracture was closed and one was open. One alpaca had a closed, comminuted fracture of the distal radius, and the final alpaca had a closed, oblique fracture of the metatarsus. A diagnosis was made in each animal on the basis of clinical examination and radiographs. All fractures were managed by open reduction and internal fixation using selective placement

Distal tibial triplane fractures: long-term follow-up.

The cases of 35 patients in whom a triplane fracture had been treated at the Ramón y Cajal Hospital were reviewed. The diagnosis of biplane and triplane fracture can now be made by standard radiograph. However, plain radiographs alone did not accurately demonstrate the configuration of the fracture, and computed tomography must be performed. Closed reduction should be attempted first. Failure to obtain or maintain (or both) an adequate closed reduction is an indication for operative treatment. Prognosis is surprisingly good. Only when adequate reduction (< 2 mm displacement) has not been achie

Interposition of Tibialis Posterior and Flexor Digitorum Longus Tendons Resulted in Irreducible Ankle Fracture-Dislocation.

Irreducible ankle fracture-dislocations are very rare entities. The present case report demonstrates an unusual finding of tibialis posterior and flexor digitorum longus tendons interposed in the tibiofibular joint impairing successful closed reduction of ankle fracture-dislocation. A 45-year-old patient presented with a bimalleolar pronation-external rotation ankle fracture-dislocation after a motorcycle accident. Attempts to perform closed reduction before surgery were unsuccessful. Subsequent urgent open reduction and internal fixation surgical management revealed interposition of the tibia

Iatrogenic displacement of fracture-dislocations of the shoulder. A report of seven cases.

We report seven cases in which open or closed reduction of a shoulder dislocation associated with a fracture of the humeral neck led to displacement of the neck fracture. Avascular necrosis of the humeral head developed in all six patients with anatomical neck fractures. All five anterior dislocations also had a fracture of the greater tuberosity and both posterior dislocations had a fracture of the lesser tuberosity. The neck fracture had not initially been recognised in three of the seven cases. In five cases attempted shoulder reduction led to complete displacement of the head segment, whic

Intramedullary fracture reduction of long bone under visual control: experimental results of an endoscopic technique.

In controlling the closed reduction of a shaft fracture of long bone during an operation, intraoperative radiography is essential. The amount of imaging needed depends on the patient, the fracture, and the surgeon. This article documents our first experimental results of closed fracture reduction performed under direct visual control with the endoscopic technique of intramedullary bone endoscopy (IBE), which eliminates the need for fluoroscopy. On 3 human tibial cadaveric bones, an artificial shaft fracture was set. The lower leg was fixed on a brace, and the endoscope was inserted at the stan…

Treatment goals for distal radius fractures in 2018: recommendations and practical advice.

The goals of distal radius fracture treatment in patients above 65 years of age would not change over time if the fracture were the only factor to consider. However, people change, and fixation methods also change. Since this fracture heals in nearly every case and volar plates have eliminated the worry of malunion, we are left with two main goals. In active patients with weakened bones, the aim is to help them regain their quality of life as quickly as possible while avoiding iatrogenic conditions. This compromise is possible because of new tools-but at what price?

Ultrasound-guided techniques for managing length- unstable femoral shaft fractures in children.

OBJECTIVE: The management of length-unstable femoral shaft fractures(LUFSFs) in pediatric patients is still controversial. This study aims to explore the clinical efficacy of ultrasound-guided closed reduction combined with external fixation for treating LUFSFs in children. METHODS: We conducted a retrospective analysis of clinical data from 19 pediatric patients with LUFSFs who underwent ultrasound-guided closed reduction and external fixation between January 2018 and January 2023. Ultrasound was employed not only to facilitate closed reduction of the fracture but also to guide real-time ins

A Simple and Minimal Invasive Method in Reduction of Depressed Nasal Bone Fracture by Using a Foley Catheter Ballooning Technique.

The nasal bone fracture is the most common type of facial bone fracture. Closed reduction with metal reduction instrument is commonly conducted for the treatment of a type II nasal bone fracture. The authors defined a new catheter dilation technique and used it in patients with type II depressed nasal bone fractures. Preoperative and postoperative nasal appearance and radiologic examination of the patients were compared. There was a statistically significant improvement in the nasal appearance of all patients. No recurrence or dorsal irregularity has been observed. This new, easily applicable

Treatment of supracondylar fractures following prosthetic arthroplasty of the knee.

Fifteen supracondylar fractures of the femur in patients with ipsilateral total knee arthroplasties were treated between 1975 and 1982. Three groups were identified for analyzation of treatment and end result. The average follow-up period after fracture was 18 months, with a range of ten to 48 months. Group I had four patients treated by closed reduction, cast immobilization, and early weight-bearing. At follow-up evaluation, three had a decrease in knee rating score, and one required a corrective osteotomy. Group II had eight patients treated by traction followed by cast or cast-brace immobil

Closed vs open nailing for displaced middle third fracture of clavicle. Does it matter?

BACKGROUND: Intramedullary nailing for fractures of middle third clavicle has become increasingly popular. Though open nailing has been widely described, closed nailing finds less mention. OBJECTIVES: In this study we compared closed nailing with open nailing in fractures of middle third of clavicle to assess if the former holds any advantage. MATERIAL AND METHODS: 34 patients with closed nailing were compared to 31 patients with open nailing in terms of operative time, length of incision, pain, time to union and functional outcome. RESULTS: Only operative time and length of incision were s

Cementless acetabular reconstruction after acetabular fracture.

BACKGROUND: Total hip arthroplasty in patients with posttraumatic arthritis has produced results inferior to those in patients with nontraumatic arthritis. The use of cementless acetabular reconstruction, however, has not been extensively studied in this clinical context. Our purpose was to compare the intermediate-term results of total hip arthroplasty with a cementless acetabular component in patients with posttraumatic arthritis with those of the same procedure in patients with nontraumatic arthritis. We also compared the results of arthroplasty in patients who had had prior operative treat

Management of Proximal Interphalangeal Joint Fracture Dislocations.

Fracture dislocations of the proximal interphalangeal (PIP) joint of the finger are often caused by axial load applied to a flexed joint. The most common injury pattern is a dorsal fracture dislocation with a volar lip fracture of the middle phalanx. Damage to the soft-tissue stabilizers of the PIP joint contributes to the deformity seen with these fracture patterns. Unfortunately, these injuries are commonly written off and left untreated. A late-presenting PIP joint fracture dislocation has a poor chance of regaining normal range of motion. The provider must be suspicious of these injuries.

Effectiveness of reduction in conservative treatment of boxer fractures : A retrospective study of 115 patients.

PURPOSE: Subcapital fractures of the fifth metacarpal bone are one of the most common fractures of the hand. Surgical and conservative treatment is possible but the respective indications remain controversial. Closed reduction and cast retention are commonly performed for these fractures but their effectiveness in fracture retention is unclear. Therefore, the aim of this study was to evaluate the efficacy of closed reduction with consecutive below-elbow casting and finger splinting. METHODS: Radiographs of 115 patients with a subcapital fracture of the fifth metacarpal bone at an academic lev

Synostosis after fracture of both forearm bones treated by intramedullary nailing.

This study evaluated the risk of radioulnar synostosis after fracture of both forearm bones at the same level. We hypothesized that (i) the incidence of synostosis in both-bone forearm fractures at the same level is low with intramedullary nailing (IMN); (ii) the type of fracture (open/closed) and type of reduction (open/closed) affect time to union. Seventy-eight patients who had been treated with IMN for fracture of both forearm bones and had at least 1 year of follow-up were included in the study retrospectively. All the patients were treated by IMN following closed reduction or open surger

Posterior sternoclavicular joint injuries in skeletally immature patients.

BACKGROUND: Posterior injuries to the sternoclavicular (SC) joint are uncommon. In the skeletally immature (SI) population, these injuries have been described as either dislocations of the SC joint or fractures of the medial clavicular physis. The current literature and standardized test questions state that a posterior SC injury, in a SI patient, is more likely a physeal fracture than a SC joint dislocation. However, this injury characterization is based on case reports or small case series. The purpose of this study is to characterize posterior SC injuries in SI patients in terms of the prev
