Cochlear implantation has become a standard therapy for children with bilateral profound hearing loss, resulting in substantial and sustainable benefits for the development of expressive and receptive and expressive language skills and cognition. During the last few years, audiologic and otologic criteria for cochlear implantation have been expanded. Recently, patients with profound single-sided deafness with or without tinnitus have received cochlear implants despite normal to near-normal hearing on the contralateral side. This indication, however, has thus far been restricted to adult patien…

OBJECTIVES/HYPOTHESIS: Congenital cytomegalovirus (cCMV) infection is a common cause of sensorineural hearing loss (SNHL). The incidence of SNHL is higher in symptomatic cCMV infants and is usually identified early. By contrast, the incidence of SNHL is lower in children with asymptomatic cCMV, and the hearing loss can be delayed in onset and progressive. The objective was to compare the outcome of cochlear implantation in children deafened by cCMV with a control group of children with implants who do not have the condition.
STUDY DESIGN: Retrospective review of case notes and data base.

Vertigo and dizziness are the most common complications of cochlear implantation. Data of a multicentric study about clinical aspects and cost utility were collected for vertigo and dizziness: incidence, clinical and treatment were analyzed among 469 adults and children. Results demonstrated that 16% of adults and 3% of children experienced dizziness postoperatively. In a few cases a specific mechanism as perilymphatic fistula was identified. In other cases the mechanism underlying delayed vertigo remains speculative and endolymphatic hydrops was suggested.

Partial cochlear obstruction is a relatively common finding in candidates for cochlear implants and frequently involves the inferior segment of the scala tympani in the basal turn of the cochlea. In such patients, the scala vestibuli is often patent and offers an alternative site for implantation. The current report describes two patients with such partial obstruction of the inferior segment of the basal cochlear turn, caused in one case by systemic vasculitis (Takayasu's disease) and in the other by obliterative otosclerosis. A scala vestibuli implantation allowed for complete insertion of th

A 4-year-old girl with congenital profound deafness underwent cochlear implant surgery. Preoperative CT and MRI revealed that her inner ears had common-cavity or aplasia-type malformation. The bilateral internal auditory meatus were markedly narrowed. Audiometric examination demonstrated that only slight residual hearing remained in the low-frequency range and that a hearing aid would be of no benefit. Cochlear implantation was performed in her left ear. Because of the abnormal position of the facial nerve, the routine facial recess approach could not be performed. A canal-wall-down mastoidect…

Radiotherapy of the head and neck can be associated with conductive and/or sensori-neural hearing loss. We report the case of a 67-year-old man who developed complete bilateral deafness caused by labyrinthitis and radiation-induced neuritis of the acoustic nerve after postoperative radiotherapy of a nasopharyngeal carcinoma. Two years postoperatively extensive clinical workup including computed tomography and magnetic resonance imaging showed no recurrence or secondary brain tumors. To facilitate sound perception a Combi 40 cochlear implant was implanted. Because of fibrosis the insertion dept

OBJECTIVE: Commercial cochlear-implant (CI) telehealth is manufacturer-specific and generally does not replicate standard sound-field tests. This study developed and tested a manufacturer-independent CI telehealth system designed to replace a standard clinic visit.
DESIGN: The system comprised a laptop, a sound card, headphones (placed over the CI sound processor), manufacturer's programming software and cables, and custom software for aided speech-recognition and warble-tone detection-threshold assessment. Phase 1 compared telehealth-system aided assessment to standard sound-field testing. I

PURPOSE OF REVIEW: Frequency-to-place mismatch is the discrepancy between the frequency information delivered by a cochlear implant electrode and the natural cochlear place frequency location that is stimulated. Frequency-to-place mismatch has been shown to negatively influence cochlear implant outcomes. Anatomy-based fitting (ABF) aligns filter frequencies with cochlear frequency place to minimize or eliminate frequency-to-place mismatches. This article reviews the current evidence on variability in electrode array placement, influence of frequency-to-place mismatches on cochlear implant reci

IMPORTANCE: Cochlear implants enable improvements in speech perception, but music perception outcomes remain variable. Image-guided cochlear implant programming has emerged as a potential programming strategy for increasing the quality of spectral information delivered through the cochlear implant to improve outcomes.
OBJECTIVES: To perform 2 experiments, the first of which modeled the variance in music perception scores as a function of electrode positioning factors, and the second of which evaluated image-guided cochlear implant programming as a strategy to improve music perception with a c

Sensory prostheses replace dysfunctional sensory organs with electrical stimulation but currently fail to restore normal perception. Outcomes may be limited by stimulation strategies, neural degeneration, or suboptimal decoding by the brain. We propose a deep learning framework to evaluate these issues by estimating best-case outcomes with task-optimized decoders operating on simulated prosthetic input. We applied the framework to cochlear implants - the standard treatment for deafness - by training artificial neural networks to recognize and localize sounds using simulated auditory nerve inpu

Cochlear implantation is an established surgical intervention for patients with severe to profound sensorineural hearing loss. Although technological advancements have improved surgical outcomes, complications can still arise, affecting both short- and long-term postoperative results. Identifying and managing these complications is very important for optimizing patient outcomes. This review examined the key complications associated with cochlear implantation, discussing their mechanisms, clinical implications, and management strategies based on current literature. A comprehensive literature re…
