Usefulness of a cognitive behavioural and rehabilitative approach to enhance long lasting benefit after lumbar spinal stenosis and degenerative spondylolisthesis surgery. A case report.

Most spine-surgically treated patients are considered eligible for rehabilitative treatment. Unfortunately, an evidence-based consensus on the best postsurgical conservative treatment is not mentioned. In absence of a shared pathway, physical therapies, exercises, back schools, massages and other manual therapies are sometimes recommended by surgeons and clinicians. Like common low back pain, spinal problems in the postsurgical phase should be reconsidered through a bio-psycho-social model, advocating comprehensive evaluations and broader answers to patient-perceived pain, disability, and qual

ZJP-2, a novel wild jujube polysaccharide, enhances longevity, cognitive function, and mitigates oxidative stress via mitochondrial regulation in Caenorhabditis elegans.

Oxidative stress, caused by an imbalance between reactive oxygen species (ROS) production and antioxidant defenses, drives aging and age related functional decline. Natural polysaccharides from medicinal plants are promising interventions against oxidative stress related disorders. This study examined the protective effects of a novel polysaccharide (ZJP-2) isolated from wild jujube (Ziziphus jujuba Mill. var. spinosa) against stress induced physiological deterioration in Caenorhabditis elegans. Using lipopolysaccharide exposure, heat shock, and oxidative stress models, we assessed ZJP-2 effec

Enhanced brain delivery of oleanolic acid by GSH-PEGylated liposomes and its amelioration of scopolamine-induced cognitive dysfunction via multi-target regulation.

Oleanolic acid (OA) exhibits pronounced neuroprotective effects in the central nervous system; however, its clinical translation is limited by poor solubility and low bioavailability. To overcome these limitations, we developed glutathione-PEGylated OA liposomes (GSH-PEG-OA-LIP) via a post-insertion method, achieving an load efficiency of 94.2%. In vivo imaging demonstrated that GSH-PEG modification significantly prolonged blood circulation and enhanced brain accumulation compared to unmodified liposomes. In a mouse model of scopolamine (SCOP)-induced cognitive deficit, GSH-PEG-OA-LIP treatmen

P1642-1, a novel pancreatic polypeptide analogue, ameliorates cognitive impairment in 5 ×FAD mice and is associated with enhanced PINK1/Parkin-related mitophagy.

Currently, few pharmacological treatments are available for Alzheimer's disease (AD). However, gut-brain peptides, especially pancreatic polypeptide (PP) analogues, have shown promise. PP analogues have been reported to cross the blood-brain barrier and activate neuropeptide Y4 receptor (NPY4R) in the brain, thereby ameliorating AD-related cognitive deficits. P1642-1 is a novel PP analogue, but its role and mechanism in AD remain unexplored. This study utilized 5 ×FAD mice as a model to assess the effects of P1642-1 on cognitive dysfunction and its underlying mechanisms, while an Aβ25-35-induc

Intragastric Balloon Treatment Enhances Weight Maintenance Adjunct to Low-Energy Diet and Group-Based Cognitive Behavioural Therapy: A Randomized Controlled Trial.

AIMS: Low-energy diet (LED) plus cognitive-behavioural therapy (CBT) produces substantial short-term weight loss in obesity, but long-term maintenance remains difficult. We evaluated whether adding intragastric balloon (IGB) treatment to group-based CBT improves weight maintenance after initial weight loss using LED. MATERIALS AND METHODS: In this open-label randomized controlled trial, adults aged 30-65 years with BMI 32.5-45 kg/m2 underwent a 6-month run-in period combining LED and group-based CBT and were subsequently randomized to CBT and 1-year IGB or CBT alone. The primary endpoint was

Longitudinal evidence of technology-enhanced, individualized neuromotor rehabilitation on autonomy, cognition, quality of life, and psychological well-being: Pilot multi-sample study.

BACKGROUND: Over the last decades, neuromotor rehabilitation programs have integrated multidisciplinary approaches with the implementation of emerging technology (e.g., robotics and virtual reality - VR), to effectively target recovery complexity. While this strategy supported patient's physical improvement, little evidence has been reported regarding the widespread effects on non-motor rehabilitation outcomes. METHODS: A prospective, two-arm, non-randomized study design was adopted to provide pilot feasibility evidence on the multi-domain impact of personalized technology-enhanced neuromotor

The novel sphingolipids cP1P and P1P attenuate neuroinflammation and enhance S1PR1/Akt/mTOR signaling to mitigate cognitive decline in an Alzheimer's disease mouse model.

BACKGROUND: Alzheimer's disease (AD) is the leading cause of dementia, characterized by the accumulation of amyloid-βeta (Aβ) peptides and hyperphosphorylated tau protein. Altered sphingosine 1-phosphate (S1P) metabolism is associated with abnormal Aβ peptide accumulation in the brain. S1P receptors are increasingly being targeted for modulating the neuroinflammatory process in AD. METHODS: Wild-type male C57BL/6J mice were administered Aβ to induce the pathological state. The study included four experimental groups: (1) Control group (saline-treated), (2) Aβ group (Aβ + saline-treated), (3)

Enhancing cognitive abilities through transcutaneous auricular vagus nerve stimulation: Findings from prefrontal functional connectivity analysis and virtual brain simulation.

Recent studies have indicated the potential of transcutaneous auricular vagus nerve stimulation (taVNS) as an intervention for cognitive decline. In this study, we systematically analyzed the effects of taVNS on cognitive enhancement from the perspective of brain networks, by combining functional near-infrared spectroscopy (fNIRS) signal analysis with virtual brain simulations. Behavioral experiments with older adults demonstrated that participants with low baseline performance experienced significant improvements in working memory performance following taVNS, while those with high baseline pe…

Nitric Oxide in Parkinson's Disease: The Potential Role of Dietary Nitrate in Enhancing Cognitive and Motor Health via the Nitrate-Nitrite-Nitric Oxide Pathway.

BACKGROUND/OBJECTIVES: Parkinson's disease (PD) is a progressive neurodegenerative disorder characterized by the loss of dopaminergic neurons in the substantia nigra pars compacta, leading to motor symptoms such as tremor, rigidity, and bradykinesia. The pathological hallmarks of PD include Lewy bodies and mechanisms like oxidative/nitrosative stress, chronic inflammation, and mitochondrial dysfunction. Nitric oxide (NO), produced by nitric oxide synthase (NOS) isoforms, plays a dual role in neuroprotection and neurodegeneration. Excessive NO production exacerbates neuroinflammation and oxidat

Psilocybin restrains activity-based anorexia in female rats by enhancing cognitive flexibility: contributions from 5-HT1A and 5-HT2A receptor mechanisms.

Psilocybin has shown promise for alleviating symptoms of depression and is currently in clinical trials for the treatment of anorexia nervosa (AN), a condition that is characterised by persistent cognitive inflexibility. Considering that enhanced cognitive flexibility after psilocybin treatment is reported to occur in individuals with depression, it is plausible that psilocybin could improve symptoms of AN by breaking down cognitive inflexibility. A mechanistic understanding of the actions of psilocybin is required to tailor the clinical application of psilocybin to individuals most likely to

Epigallocatechin-3-gallate Enhances Cognitive and Memory Performance and Protects Against Brain Injury in Methionine-induced Hyperhomocysteinemia Through Interdependent Molecular Pathways.

Brain injury and cognitive impairment are major health issues associated with neurodegenerative diseases in young and aged persons worldwide. Epigallocatechin-3-gallate (EGCG) was studied for its ability to protect against methionine (Met)-induced brain damage and cognitive dysfunction. Male mice were given Met-supplemented in drinking water to produce hyperhomocysteinemia (HHcy)-induced animals. EGCG was administered daily concurrently with Met by gavage. EGCG attenuated the rise in homocysteine levels in the plasma and the formation of amyloid-β and tau protein in the brain. Cognitive and me

Does the addition of cognitive therapy to exposure and response prevention for obsessive compulsive disorder enhance clinical efficacy? A randomized controlled trial in a community setting.

OBJECTIVES: Exposure and response prevention (ERP) remains the most empirically supported psychological treatment for obsessive compulsive disorder (OCD). Clinical guidelines recommend the addition of cognitive approaches to ERP although the presumed additive benefits have not been directly tested. The aim of this was to compare a treatment that integrated cognitive therapy with ERP (ERP + CT) to traditional, manualized ERP to test the additive benefits. DESIGN: A longitudinal, randomized control trial design was used. METHODS: Participants (N = 127) with OCD were randomly assigned to receiv

Comparing cognitive behavioural therapy for eating disorders integrated with behavioural weight loss therapy to cognitive behavioural therapy-enhanced alone in overweight or obese people with bulimia nervosa or binge eating disorder: study protocol for a randomised controlled trial.

BACKGROUND: Around 40 % of individuals with eating disorders of recurrent binge eating, namely bulimia nervosa and binge eating disorder, are obese. In contrast to binge eating disorder, currently there is no evidence base for weight management or weight loss psychological therapies in the treatment of bulimia nervosa despite their efficacy in binge eating disorder. Thus, a manualised therapy called HAPIFED (Healthy APproach to weIght management and Food in Eating Disorders) has been developed. HAPIFED integrates the leading evidence-based psychological therapies, cognitive behavioural therapy

Metacontrol-related aperiodic and periodic neural activity in cognitive aging: enhancing the neural signal-to-noise ratio through anodal transcranial direct current stimulation.

Metacontrol, the ability to adapt cognitive control to task demands, declines with age and is thought to be reflected in aperiodic and periodic neural dynamics. Given that anodal transcranial direct current stimulation (atDCS) can modulate cortical excitability via membrane potential shifts, we tested whether atDCS alters the neurophysiological signatures of metacontrol in younger and older adults. In a mixed design, younger and older participants performed a Go/Nogo task under both atDCS and sham stimulation conditions; resting-state EEG data were also acquired. Aperiodic activity was analyze
