The semantics of "Doctor": A case of confused identity between nurses and physicians?

The growing number of Doctor of Nursing Practice graduates providing direct clinical care has reignited public and professional debate about the use of the title "Doctor." Although based on academic achievement, the title can unintentionally create confusion among patients and team members regarding provider roles and responsibilities. This commentary reexamines this issue in the context of doctorally prepared health care professionals who, through their education, training, and licensure, provide direct patient care. Recognizing that it truly "takes a village" to deliver the highest quality h…

Somatostatin analogues for ADPKD: ending the confusing story of mGFR and eGFR.

This commentary discusses the Lanreotide In Polycystic kidney disease Study (LIPS) that is published in this issue of Kidney International. It represents the last randomized evaluation of somatostatin analogues in patients with autosomal dominant polycystic kidney disease. Lanreotide did not demonstrate a beneficial effect on the primary outcome, measured glomerular filtration rate decline, but did suggest an effect on creatinine-estimated glomerular filtration rate slope. The discrepancy between both outcomes highlights important challenges in the interpretation of kidney function end points

Splenectomy training in 2026: A modest proposal for the thoroughly confused surgeon.

Since the first laparoscopic splenectomy in 1991, splenic surgery has evolved steadily toward minimally invasive techniques, which now represent the gold standard for most indications. In children, the principal indications remain hemolytic disorders, mainly sickle cell disease and hereditary spherocytosis, for which the optimal timing of surgery and the choice between partial and total splenectomy continue to be debated. Two laparoscopic approach, anterior and posterior, are described, each dictating a specific patient position, dissection sequence, and trocar placement. Teaching the procedur

Reconceptualizing Health Literacy to End Confusions: An Umbrella Review Research.

BACKGROUND AND AIMS: The evolving definitions and interpretations of health literacy have created confusion, potentially hindering the development of effective interventions and policies. This umbrella review aimed to comprehensively evaluate systematic reviews (SRs) that provided definitions and conceptualizations of health literacy, with the goal of categorizing and clarifying the available information and eliminating inconsistencies. METHODS: This study followed the Joanna Briggs Institute (JBI) methodology for umbrella reviews. In July 2023, nine databases and two organizational websites …

Pigmented Birthmarks and Spinal Neurofibromas in KRAS Mosaicism-Not to Be Confused With NF1.

We report a child presenting with pigmentary skin lesions and spinal neurofibromas who was diagnosed molecularly with KRAS mosaicism. We review the previous literature of two cases of congenital skin lesions and neurofibromas and spinal nerve root hypertrophy caused by KRAS variants and highlight this presentation as an important differential diagnosis for neurofibromatosis.

Cannabis and Polypharmacy Leading to Hospitalization for Acute Confusion and Inability to Ambulate.

Inability to ambulate is a common issue for which an Emergency Department (ED) may request inpatient admission to the hospital. Frequent contributing factors include advanced age, spinal stenosis, osteoarthritis, deconditioning, and polypharmacy. We present an interesting case of a patient admitted due to inability to ambulate who had a significant acute decline in function coincident with the introduction of gabapentin in the setting of heavy tetrahydrocannabinol (THC) use.

Serotonin Syndrome in Parkinson's Disease: Don't Get It Confused by a Tremor.

Serotonin syndrome (SS) is a potentially life-threatening condition caused by excessive serotonergic activity, often due to drug interactions. It classically manifests with autonomic and neuromuscular hyperactivity and by mental status changes that might include restlessness, delirium, and agitation. We present a case of a 76-year-old patient with Parkinson's disease with SS triggered by interaction between rasagiline and buspirone. He presented with altered mental status, hyperreflexia, and muscle rigidity. Due to the severity of symptoms, he was early admitted to the ICU, and his clinical co

Rare huge bilateral adrenal myelolipoma confused with retroperitoneal liposarcoma.

INTRODUCTION: Adrenal myelolipomas are uncommon benign adrenal tumors, which mostly occur unilaterally. We describe a rare case of giant bilateral adrenal myelolipoma mistaken for retroperitoneal liposarcoma. CASE PRESENTATION: A 49-year-old man developed fever, left flank pain, and a large mass in his left abdomen. Plain computed tomography showed a 23-cm mass with fat-containing components in the retroperitoneal cavity, suggestive of retroperitoneal liposarcoma. A similar 9-cm mass was observed in the right adrenal gland, suggesting adrenal metastasis. With a clinical diagnosis of retroperi

Pellagra as a differential diagnosis in the confused patient on the acute medical unit.

A man in his 80s was admitted via the acute medical take after presenting with increased confusion and features of alcohol withdrawal. He had a several-month history of a worsening pruritic rash surrounding his neck, arms and legs in addition to new, profuse diarrhoea. In view of the background of known chronic alcoholism and the coexisting symptoms of rash, confusion and diarrhoea, pellagra was diagnosed via a multidisciplinary approach. Oral nicotinamide supplementation was commenced and his symptoms responded rapidly. The bias and challenge of reaching a unified diagnosis in the context of …

Congenital syphilis, the great mimicker, confused with non-accidental trauma.

Congenital syphilis (CS) remains a major health problem worldwide. Children born with CS can present with variable symptoms that can mimic other disease processes. This report describes the case of a 4-month-old infant who presented with the left wrist swelling and distal radial fracture, causing concern for non-accidental trauma (NAT). An osseous survey showed additional findings and led to the correct diagnosis. Although radiographic features of NAT may overlap with CS, certain features such as bilaterally symmetrical skeletal involvement and metaphyseal destruction, as seen in our case, can

Neural correlates of confusability in recognition of morphologically complex Korean words.

When people confuse and reject a non-word that is created by switching two adjacent letters from an actual word, is called the transposition confusability effect (TCE). The TCE is known to occur at the very early stages of visual word recognition with such unit exchange as letters or syllables, but little is known about the brain mechanisms of TCE. In this study, we examined the neural correlates of TCE and the effect of a morpheme boundary placement on TCE. We manipulated the placement of a morpheme boundary by exchanging places of two syllables embedded in Korean morphologically complex word

Confusion, dissociation and bizarre behaviour as the onset of an early Susac syndrome.

A 47-year-old woman presented an episode of confusion and disorientation. According to remarkable psychiatric records, she had been treated for major depression and obsessive-compulsive disorder; however, no other relevant background was known. After preliminary examinations, blood analysis and neurological tests were unspecific and inconclusive. Therefore, the case was treated as a possible psychiatric episode related to her previous psychiatric disorders. However, due to the atypical presentation of the case, a cerebral MRI was performed, which demonstrated multiple central lesions of the co…

Sándor Ferenczi's multiple confusions of tongues and their influence on psychoanalytical thinking.

Using a poststructuralist model, this article explores the lecture given by Ferenczi and published under the title "Confusion of Tongues between Adults and the Child-(The Language of Tenderness and Passion)." By initially focusing on the closed structure of the text, the author identifies two types of confusion of tongues that are closely interlinked: the confusion between adults and the child, and that between the analyst and the analysand. By then placing the manuscript within the corpus of Ferenczi, he connects it to the latter's multilingualism and pleads in favour of autobiographical dete…

Isolated rupture of the accessory soleus tendon: an original and confusing picture.

The accessory soleus muscle is an uncommon congenital anatomical variant with a prevalence ranging from 0.7 to 5.5%. Although intermittent and exertional symptoms caused by this supernumerary muscle have been well documented, acute injuries have not. We present a case of an isolated rupture of the accessory soleus tendon with myotendinous retraction, mimicking clinically a "tennis leg." A 29-year-old woman sustained a hyperdorsal flexion injury of the right ankle with a severe and sudden pain in the middle part of the calf. Radiographs were normal and the diagnosis of "tennis leg" was clinical

Review of the Nomenclature of the Retaining Ligaments of the Cheek: Frequently Confused Terminology.

Since the time of its inception within facial anatomy, wide variability in the terminology as well as the location and extent of retaining ligaments has resulted in confusion over nomenclature. Confusion over nomenclature also arises with regard to the subcutaneous ligamentous attachments, and in the anatomic location and extent described, particularly for zygomatic and masseteric ligaments. Certain historical terms-McGregor's patch, the platysma auricular ligament, parotid cutaneous ligament, platysma auricular fascia, temporoparotid fasica (Lore's fascia), anterior platysma-cutaneous ligamen…
