BACKGROUND: Continuous glucose monitoring (CGM) shows in more detail the glycaemic pattern of diabetic subjects and provides several new parameters ("glucometrics") to assess patients' glycaemia and consensually guide treatment. A better control of glucose levels might result in improvement of clinical outcome and reduce disease complications. This study aimed to gather an expert consensus on the clinical and prognostic use of CGM in diabetic patients at high cardiovascular risk or with heart disease.
METHODS: A list of 22 statements concerning type of patients who can benefit from CGM, progn…

BACKGROUND Insulinoma presenting only with postprandial hypoglycemia is difficult to diagnose. Repeated episodes of hypoglycemia can lead to "hypoglycemia unawareness", which can be even more dangerous and requires early detection and treatment. CASE REPORT We report the case of a 35-year-old man with an insulinoma presenting as postprandial hypoglycemia who was treated with diazoxide and monitored using a factory-calibrated continuous glucose monitoring (CGM) system until surgery. When the patient initially presented with hypoglycemia, relative hyperinsulinemia was present. There were no obvi…

Parallel glucose measurements in blood and other different tissues give us knowledge about dynamics of glycemia changes, which depend on vascularization, distribution space and local utilization by tissues. Such information is important for the understanding of glucose homeostasis and regulation. The aim of our study was to determine the time-lag between blood, brain, and adipose tissue during rapid glucose changes in a male hHTG rat (n=15). The CGMS sensor Guardian RT (Minimed/Medtronic, USA) was inserted into the brain and into the abdominal subcutaneous tissue. Fixed insulin and variable ra

Continuous glucose monitoring (CGM) is now central to diabetes management, yet variation in how respective medical products are evaluated limits meaningful comparison between CGM systems. Three barriers currently constrain reliable interpretation of glucose-derived measures. The first is limited transparency: in several regulatory settings, particularly those using Conformité Européenne marking, clinical-study reports, reference-method information and analytical documentation required for market authorisation are not publicly accessible. The second barrier is heterogeneity in study procedures.…

This review examined study characteristics and common practices in continuous glucose monitoring (CGM)-based exercise studies. Following PRISMA guidelines, a systematic search in the PubMed database was conducted. Data were extracted from 93 publications. A minority of studies (12.9%) focused on CGM system validation. Acute exercise studies were more common (87.1%) than chronic exercise studies (12.9%). Randomized crossover designs predominated (71.0%). Participant populations varied and included 46.2% non-diabetic individuals (7.5% athletes), 36.6% individuals with type 1 diabetes mellitus, a…

Cutaneous infections with Mycobacterium abscessus are increasingly recognized following disruption of the skin barrier. We report a 72-year-old man with an indurated nodule at a continuous glucose monitor (CGM) insertion site that persisted despite multiple courses of antibiotics. Histopathology demonstrated suppurative granulomatous inflammation, and tissue culture confirmed M. abscessus. This case identifies continuous glucose monitor use as a novel and likely underrecognized source of primary inoculation and underscores the importance of early biopsy with histopathologic, microbiologic, and

Our study aims to understand the barriers and facilitators surrounding continuous glucose monitors (CGMs) in adolescents experiencing diabetes distress from type 1 diabetes through a biopsychosocial lens. We qualitatively analyzed interviews of 21 adolescents and coded their emotional experiences. Findings show that biologically, adolescents noted improved mood with healthier glucose ranges and future health prospects. Psychologically, adolescents preferred a sense of control over when to use, and take a break from, their CGM. Socially, they described mixed feelings surrounding how CGM use imp

BACKGROUND: Older adults with diabetes in the hospital are generally managed similarly to younger adults, however, it is unknown if the degree of frailty can affect glucose control among hospitalized patients.
METHODS: We examined glycemic parameters derived from continuous glucose monitoring (CGM) in older adults with type 2 diabetes and frailty who were hospitalized in non-acute settings. Data was pooled from 3 prospective studies using CGM including 97 patients wearing Libre CGM sensors and 166 patients wearing Dexcom G6 CGM. Glycemic parameters (time in range (TIR) 70-180; time below rang…

Continuous glucose monitoring (CGM) is indicated in poorly controlled insulin-treated patients with type 2 diabetes (T2D) to improve glycemic control and reduce the risk of hypoglycemia, but the benefits of CGM for lower risk patients have not been well studied. Among 17,422 insulin-treated patients with T2D with hemoglobin A1c (HbA1c) <8% and no recent severe hypoglycemia (based on emergency room visits or hospitalizations), CGM initiation occurred in 149 patients (17,273 noninitiators served as reference). Changes in HbA1c and severe hypoglycemia rates for the 12 months before and after CGM …

INTRODUCTION: Perioperative glycaemic control is associated with reduced postoperative complications. However, regulatory restrictions on point-of-care device operation and operative positioning constraints, reducing accessibility for dermal puncture, may hinder intraoperative monitoring. Although continuous glucose monitors (CGM) support real-time glucose monitoring, intraoperative patient positioning may increase compression low susceptibility, and electrosurgery interference may affect sensor-receiver connectivity. Alternative CGM sensor placement may improve intraoperative sensor-receiver

BACKGROUND: Glycated hemoglobin (HbA1c) is a convenient tool to evaluate glycemic status but its ability to detect individuals at risk for type 2 diabetes is limited.
OBJECTIVE: Exploiting the glycemic variability captured in continuous glucose monitoring (CGM), we used a well-characterized Asian cohort study from Singapore to assess whether utilizing CGM features in a machine learning model can improve the detection of prediabetes as compared to using HbA1c alone.
METHODS: In this study, 406 nondiabetic Asian participants underwent an oral glucose tolerance test and had their fasting and 2-…
