BACKGROUND AND OBJECTIVES: Anti-CD38 monoclonal antibodies, including daratumumab and isatuximab, often interfere with pretransfusion testing. Dithiothreitol (DTT) treatment of red blood cells (RBCs) negates this interference. However, the optimum DTT concentration and treatment time have not been well defined. INTRODUCTION: Autoimmune hemolytic anemia (AIHA) is a rare and generally self-limiting disease in children.
MATERIAL AND METHODS: A descriptive cross-sectional study was performed in children under 18 years diagnosed with AIHA from January/1997 to July/2019. Clinical variables were collected and AIHA was classified according to the direct antiglobulin test (DAT) in warm AIHA (IgG+/-C3d) and cold AIHA (C3d). We describe two cases of hemolytic anemia following high-dose intravenous human immunoglobulin administration in young females with idiopathic thrombocytopenic purpura. Inducement of hemolytic anemia by anti-A and anti-B antibodies in these cases necessitates the performance of a minor crossmatching procedure prior to immunoglobulin use to prevent the occurrence of this adverse reaction.

A total of 29 infertile couples (group A) with male antisperm antibodies detected by the mixed antiglobulin reaction (MAR) and partly by flow cytometry (n = 21) were treated using an intracytoplasmic sperm injection (ICSI) technique to assist fertilization. Antibody screening tests were performed in 29 unsensitized pregnant women after antepartum Rh immune prophylaxis, using the indirect Coombs test (ICT) and a more sensitive ID-microtyping-system (IDM). With the ICT, anti-D antibodies were detected in 85% for at least 4 weeks and at most 8 weeks after immunisation. The maximum titer was 1:8. Immune hemolytic anemia due to a drug-adsorption mechanism has been described primarily in patients receiving penicillins and first-generation cephalosporins. We describe a patient who developed anemia while receiving intravenous cefotetan. Cefotetan-dependent antibodies were detected in the patient's serum and in an eluate prepared from his red blood cells. In thirty patients bilateral orchiopexy was performed in puberty. At the operation twenty-five patients underwent bilateral testicular biopsies, and five patients underwent unilateral biopsies only. In adulthood the semen was analysed for antisperm antibodies by the direct mixed antiglobulin reaction (MAR) IgG test. In none of the cases a positive direct MAR IgG test was found.

In an attempt to confirm the immune nature of the thrombocytopenia complicating infectious mononucleosis, the amount of IgG bound to the platelets of 2 affected patients was measured. Using a sensitive quantitative antiglobulin consumption technique, increased amounts of IgG were demonstrated during the thrombocytopenic phase of the illness. This observation represents convincing evidence in favour of platelet destruction by an immune mechanism.
