Imaging options for defining anomalous origin of the coronary artery from the pulmonary artery.

Anomalous origin of the coronary artery from the pulmonary artery is a rare type of congenital heart disease (CHD). According to the establi … ary artery die within the first year of life. The treatment of the disease is to restore the double coronary circulation as soon as possible by surgery after early detection by diagnostic imaging. Presently, medical imaging is the most commonly used examination method for a diagnosis of the disease. The comprehensive application of various imaging modalities is the ba …

Vasospastic Angina and its Relationship with the Coronary Microcirculation.

Vasospastic angina is an important cause of chest pain due to coronary artery vasospasm that is related to poor quality of life and can lead to myocardial infarction, arrhythmias and death. Since its first description as "Prinzmetal or variant angina" which was believed to be a focal spam that occurred in non-obstructed epicardial coronary arteries, physician and researchers were gradually confronted with the clinical reality and came to the conclusion that the coronary artery vasospasm was considerably more polymorphic than initially described. Although mechanism leading to vasospastic angina

[Congenital intercoronary arterial anastomosis. Apropos of a case and review of the literature].

The authors report a case of coronary arterial anastomosis between the right coronary and left circumflex arteries in the absence of other coronary pathology. This is an exceptionally rare congenital malformation which is situated either in the atrioventricular groove or at the apex in the interventricular groove. It constitutes a diagnostic pitfall because, in contrast to a collateral circulation, it is not an indirect sign of occlusive coronary artery.

The normal and abnormal anatomy of the coronary arteries.

Beneficial therapeutic options are increasingly available for coronary arterial disease. To take full advantage of these options, those performing the interventions require a thorough knowledge of the normal arrangement, and likely anatomic variations, of the coronary arterial system. A comprehensive appreciation of the architecture of the coronary arterial system, therefore, is crucial to optimal cardiac care. Simple attention to potential variations in the origin and course of the major coronary arteries can greatly enhance clinical outcomes. The objective of this review, therefore, is to de

Reversible myocardial perfusion defects in patients not suffering from obstructive epicardial coronary artery disease as assessed by coronary angiography.

In approximately 10-30% of patients presenting with angina complaints, normal or non-obstructive coronary arteries are found on angiography. … angiographically undetectable plaques are associated with an abnormal vasodilation capacity of the coronary circulation and may results in reversible perfusion defects on myocardial perfusion imaging (MPI). Organic non-atherosclerotic causes of epicardial coronary artery disease such as anomalous coronary arteries that course between the aorta and pulmonary artery, m …

Flow in coronary artery bypass grafts to totally and partially occluded left anterior descending coronary arteries.

Flow was determined by electromagnetic flowmeter in vein bypass grafts in 20 patients with a totally occluded left anterior descending (LAD) coronary artery and on 61 patients with a partially occluded LAD. The median flow in LAD grafts was 14.5 ml/min with total LAD occlusion, and 40 ml/min with partial LAD occlusion (p less than 0.001). In cases of total LAD occlusion, the presence of mild or moderate anteroseptal wall dysfunction was associated with more satisfactory flow than was the case with severe anteroseptal wall dysfunction (p less than 0.02). Flows over 25 ml/min were found only whe

Transatrial selective coronary arteriography in pulmonary atresia with intact ventricular septum.

We describe a technique for transvenous, transatrial selective coronary arteriography, which may prove useful in the diagnostic evaluation of the neonate with pulmonary atresia with intact ventricular septum when right ventriculography and antegrade aortography inadequately define the coronary arterial circulation.

Presence of coronary collaterals is associated with a decreased incidence of cognitive decline after coronary artery bypass surgery.

OBJECTIVE: Coronary artery bypass grafting (CABG) is associated with significant cerebral morbidity, usually manifested as cognitive decline or stroke. The underlying mechanism leading to cognitive decline is still unclear. Presence of coronary collateral arteries, which may reflect an overall better cardiovascular condition, recently appeared to relate to a better cardiac outcome after CABG. In this study, we investigated the hypothesis that presence of coronary collaterals is associated with less cognitive decline after coronary artery bypass grafting. METHODS: Data from 281 patients underg

Anomalous origin of left main coronary artery: the value of myocardial scintigraphic and spiral computed tomography scans.

Anomalous origin of the left main coronary artery (LMCA) from the right sinus of the Valsalva or the proximal right coronary artery (RCA) is one of the most clinically important anomalies of coronary circulation. We report the case of a patient with chest pain and abnormal thallium myocardial perfusion scan in whom the anomaly was first detected on invasive coronary arteriography. The exact anatomic course of anomalous LMCA was confirmed using contrast enhanced computed tomography.

[Unstable angina due to communication between the coronary artery and the right pulmonary artery in patient with Takayasu's arteritis].

A sixty three year old woman suffering from Takayasu's arteritis with involvement of the aortic arch, subclavia arteries and occlusion of the right pulmonary artery, developed progressive precordial pain. Exercise electrocardiogram disclosed ST segment depression. Coronary arteriograms demonstrated no coronary stenosis, but collateral circulation arising from circumflex coronary artery to right pulmonary artery. The collateral blood flow was considered to be of great importance, and causing the coronary steal syndrome, leading to angina pectoris.

Anomalous origin of the left coronary artery from the pulmonary artery in an adult: tubular reconstruction of the left main coronary artery under coronary perfusion.

A 38-year-old female with anomalous origin of the left coronary artery (LCA) from pulmonary artery was surgically corrected by tubular reconstruction of the left main coronary artery (LMCA) using the pulmonary artery wall, and this repair was performed under beating heart. Thus, the pulmonary artery was divided above the orifice level and just above the pulmonary valve, and the commissure between nonfacing and left side sinuses was dissected away from the pulmonary artery wall to obtain lateral flaps. The pulmonary artery defect was reconstructed with a roll using an autologous pericardial pat

[Coronary angiography in patients with U wave inversion during coronary artery spasm].

During ergonovine-induced vasospastic angina, U wave inversion without significant ST segment deviation on the precordial electrocardiograms was documented in four patients. Coronary angiography revealed incomplete spastic obstruction of the left anterior descending artery without delayed filling and runoff in three patients. In the remaining patient, the proximal left anterior descending artery was totally occluded and there were well-developed collaterals from the non-spastic artery. Thus, ergonovine-induced U wave inversion was related to the presence of coronary vasospasm, and angiography

Successful primary percutaneous coronary intervention in a patient with acute myocardial infarction and single coronary artery ostium.

One of the single anomalous origins of coronary artery that has rarely been reported is a congenital anomaly of coronary circulation that occurs in the left coronary artery originating from the right coronary sinus of valsalva. We report a 49-year-old male patient with non-ST segment elevated myocardial infarction that was identified to have an anomalous origin of the left coronary artery from the right coronary artery (RCA) with thrombotic total occlusion of RCA by coronary angiography and cardiac computed tomography. The patient underwent successful percutaneous coronary intervention in tota…

Collaterals in STEMI patients: An uncommon but critical lifeline.

Angiographically visible collaterals are seen in a minority of STEMI patients, predominantly to the RCA and in patients with delays to reperfusion. Patients with visible collaterals were less likely to have cardiogenic shock and tended to have a longer survival. Further studies will determine the mechanisms of collateral formation and their protective associations.

Coronary artery collateral flow and its effect on myocardial perfusion in a patient with unilateral pulmonary artery atresia.

Unilateral pulmonary artery atresia is a rare congenital defect. The affected lung in unilateral pulmonary artery atresia usually receives its blood supply from many collateral vessels including the coronary artery. We report a case of an elderly woman with suspected anginal symptoms who had a coronary artery fistula functioning as a collateral vessel due to unilateral pulmonary artery atresia.
