Combination chemotherapy of metastatic breast carcinoma with cyclophosphamide, adriamycin, and peptichemio.

Thirty patients with metastatic carcinoma of the breast were treated with a combination of cyclophosphamide, Adriamycin (doxorubicin), and peptichemio (CAP) as an induction regimen, and maintenance regimen consisting of thiotepa, methotrexate, and 5-fluorouracil (TMF). Twenty-four patients were evaluable. Thirteen patients achieved an objective response for a response rate of 54.0% (complete remission plus partial remission). Median duration of response was 9.5 months (0-32+). The CAP regimen had severe myelotoxicity that led to dose reductions in 67% of patients. Furthermore, 50% of the patie…

[Combination chemotherapy of Hodgkin's disease with vincristine, cyclophosphamide, procarbazine, and prednisolone].

Nineteen patients with stages II B-IV of Hodgkin's disease were treated with a combination of vincristine, cyclophosphamide, procarbazine, and prednisolone (CVPP regimen), given in a 3 week intervals for 3 months. Eighteen patients were available for responses. A complete remission (CR) was obtained in 9 (50%) out of 18 patients. The median duration of CR and relapse-free survival were 7 months and 26+ months, respectively. Hematologic toxicity was mild, although occasionally cumulative for aged patients and for previously treated patients. Other toxicities such as alopecia and neurotoxicity w…

Ftorafur, adriamycin, cyclophosphamide and BCG in the treatment of metastatic breast cancer.

Ftorafur is a 5-fluorouracil analogue which is slowly metabolized to 5-FU, resulting in prolonged therapeutic levels of this latter drug. Ninety-one evaluable patients with metastatic breast cancer were treated with Ftorafur, Adriamycin, cyclophosphamide, and BCG (ACFTOR-BCG), in an attempt to increase the effectiveness of the program or decrease its myelosuppressive toxicity. The results of this trial were compared to those previously reported with the combination of 5-FU, Adriamycin, cyclophosphamide, and BCG (FAC-BCG). Overall objective response rates were 65% and 76% for ACFTOR-BCG and FAC…

Cyclophosphamide alone in the treatment of adult patients with minimal change glomerulonephritis.

Eight adult patients with the nephrotic syndrome due to "minimal change" glomerulonephritis have been treated with cyclophosphamide alone. The effects of therapy have been compared with a similar group of patients treated with conventional diuretic therapy. Complete remission was achieved in seven of the cyclophosphamide treated patients as compared to only two spontaneous remissions in the untreated group. After a mean follow up period of 6 years none of these 9 patients relapsed. The eighth patient treated with cyclophosphamide remitted spontaneously 21 months after stopping cyclophosphamide…

Successful use of cyclophosphamide and prednisone for initial treatment of pemphigus vulgaris.

Previous reports showed that cyclophosphamide used late in the treatment of patients with pemphigus vulgaris reduced the complications due to steroids. In this study, cyclophosphamide was used in the initial treatment of five patients with pemphigus vulgaris in combination with prednisone to avoid side-effects of prednisone. Two patients had no prior treatment with steroids for pemphigus. Results indicate that initial combined program may be a highly effective treatment method for pemphigus vulgaris. Followup from one to four years after treatment shows all five patients in remission; four req…

Effect of cyclophosphamide on the growth of Rickettsia sennetsu in experimentally infected mice.

The growth rate of Rickettsia sennetsu, the etiological agent of sennetsu rickettsiosis, a special type of infectious mononucleosis found in western Japan, is very low when the ordinary experimental methods used for other members of the rickettsiae are employed. Attempts have been made to increase its growth the rickettsiae are employed. Attempts have been made to increase its growth rate in cells and animals. By treating mice with cyclophosphamide, considerable enhancement of growth of the rickettsia was observed. In the drug-treated mice, infective titers expressed as mean lethal dose in the…

The induction of graft versus host disease in mice treated with cyclophosphamide.

In these studies adult mice treated with cyclophosphamide and foreign immunologically competent cells developed a graft versus host disease which outwardly resembled that encountered in other experimental systems. Progressively larger doses of cyclophosphamide produced an increasingly severe disease whereas comparable doses of mechlorethamine were ineffective. Increasingly larger cell inocula from parental, allogeneic, and xenogeneic donors resulted in a correspondingly more severe disease. Nucleated cells obtained from the peripheral blood were found to be the most potent inducers of this syn…

Impact of Donor Age in Haploidentical-Post-Transplantation Cyclophosphamide versus Matched Unrelated Donor Post-Transplantation Cyclophosphamide Hematopoietic Stem Cell Transplantation in Patients with Acute Myeloid Leukemia.

Haploidentical hematopoietic cell transplantation (HCT) with post-transplantation cyclophosphamide (PTCy) graft-versus-host-disease (GVHD) prophylaxis is associated with inferior overall survival (OS) compared to HLA-matched unrelated donor (MUD) HCT with PTCy prophylaxis in patients receiving reduced-intensity conditioning (RIC). Given prognostic implications of donor age, we investigated the differences in outcomes of patients with acute myeloid leukemia (AML; n = 775) undergoing RIC-HCT with a younger MUD (age <35 years; n = 84) versus a younger haploidentical donor (age <35 years; n = 302)…

Treatment of systemic vasculitis with cyclophosphamide and steroids: daily oral low-dose cyclophosphamide administration after failure of a pulse intravenous high-dose regimen in four patients.

The efficacy of cyclophosphamide (CY) in the treatment of severe systemic vasculitis has been demonstrated, but its optimal administration regimen remains to be defined. The four patients described participated in prospective therapeutic trials of polyarteritis nodosa (PAN) without hepatitis B infection markers, Churg-Strauss syndrome (CSS) and Wegener's granulomatosis (WG) and received daily oral low-dose CY after failure of a monthly pulse regimen. These patients were included in this study because of failure to induce clinical remission (two patients) and relapse during the course of treatm…

Matched unrelated vs haploidentical donor hematopoietic cell transplantation using posttransplant cyclophosphamide.

Posttransplant cyclophosphamide (PTCy)-based graft-versus-host disease (GVHD) prophylaxis is now standard for matched unrelated donor (MUD) hematopoietic cell transplantation (HCT). Previous studies comparing MUD and haploidentical donor HCT using PTCy were limited in size and follow-up. We therefore performed a registry-based analysis examining the impact of donor type on HCT with PTCy. Adult patients (n = 5873) receiving MUD (n = 1973) or haploidentical (n = 3900) HCT with PTCy for acute leukemia (74.2%) or myelodysplastic syndrome (MDS; 25.8%) reported to the Center for International Blood …

Risk of Posttransplant Cyclophosphamide-related Cardiotoxicity in Allogeneic Stem Cell Transplantation.

BACKGROUND: Although posttransplant cyclophosphamide (PT-Cy) is currently widely used to prevent graft-vs-host disease after allogeneic hematopoietic stem cell transplantation (alloHSCT), concerns remain regarding its cardiotoxicity. The aim in this study was to assess the association between early cardiotoxicity occurring within the first 100 days posttransplant and PT-Cy. METHODS: We conducted a monocentric retrospective observational study including all consecutive patients who underwent alloHSCT at Saint Louis University Hospital between July 2011 and July 2023. The primary endpoint was a…

From Maternal Exposure to F1 Development: Unveiling Cyclophosphamide-Induced Reproductive Toxicity.

Background: Various controversial conclusions exist regarding the reproductive toxicity of cyclophosphamide, creating uncertainties about the recovery timeline of maternal reproductive capacity and offspring health. Methods: Using a mouse model with a clinically relevant cyclophosphamide dosing regimen, we examined the recovery of female reproductive function after exposure and the long-term survival and development of their offspring. Results: Our findings revealed that cyclophosphamide exposure shortened the maternal reproductive lifespan, characterized by early fertility impairment at one w…

Protective role of lycopene against cyclophosphamide-induced endocrine dysfunction in rats.

OBJECTIVE: Aim: Infertility is the inability to conceive after one year of unprotected intercourse, influenced by various physiological and environmental factors, including chemical exposure. Cyclophosphamide, used for treating tumors and autoimmune diseases, is linked to reproductive toxicity and endocrine disruption via oxidative stress and cell damage in reproductive tissues. This study assesses the protective effect of lycopene, an antioxidant found in red fruits and vegetables, against cyclophosphamide-induced hormonal changes in rats. PATIENTS AND METHODS: Materials and methods: Materia…

Outcomes of Tocilizumab for Cytokine Release Syndrome after Post-Transplant Cyclophosphamide.

Cytokine release syndrome (CRS) is common after allogeneic hematopoietic cell transplantation (alloHCT) using post-transplant cyclophosphamide (PTCy)-based graft-versus-host disease (GVHD) prophylaxis. Although tocilizumab effectively manages CRS, its impact on transplant outcomes in the PTCy setting remains unclear. We performed a single-center retrospective study of adult patients undergoing PTCy-based alloHCT from 2014 to 2024. Patients who received tocilizumab for CRS were matched 1:1 with patients with CRS who did not receive tocilizumab using propensity score matching. Of 346 patients, 1…

Immunomodulatory Activity of Pecan-Derived Peptides: Targeting Cyclophosphamide-Mediated Immune Dysregulation.

Pecan (Carya illinoensis) contains bioactive peptides with immunomodulatory potential. This study identified three pecan peptides─RWEF, LWPL, and FLLR─and evaluated their effects in cyclophosphamide-induced immunosuppressed mice. Peptide treatment significantly restored immune organ indices and biomarkers. Specifically, RWEF increased spleen acid phosphatase (ACP) and lactate dehydrogenase (LDH) by 31.2 and 40.0%, respectively; FLLR elevated ACP by 39.3% and LDH by 17.6%; and LWPL increased by 29.3 and 27.8% (all p < 0.01 compared to the model). The peptides also normalized blood cell counts a…

Intermediate-dose posttransplantation cyclophosphamide for myeloablative HLA-haploidentical bone marrow transplantation.

High-dose posttransplantation cyclophosphamide (HD-PTCy), given at 50 mg/kg/day on days +3/+4, is a standard-of-care graft-versus-host disease (GVHD) prophylaxis for allogeneic hematopoietic cell transplantation (HCT). Our murine MHC-haploidentical HCT studies suggested intermediate-dose PTCy produces superior GVHD control compared with HD-PTCy and PTCy is maximally effective on day +4. We conducted a single-institutional prospective phase 1/2 trial to reduce PTCy dosing to 25 mg/kg/day on days +3/+4 or on day +4 only for myeloablative HLA-haploidentical bone marrow HCT using PTCy, sirolimus, …
