Thirty-one women with the recurrent urethral syndrome (abacterial cystitis) had extensive microbiological, cytological and histological investigations. Emphysematous cystitis is a rare, severe infection of the urinary bladder caused by gas-forming organisms like Escherichia coli and Klebsiella pneumoniae (K. pneumoniae). Common risk factors are female sex, chronic urinary infection, immunosuppression, diabetes mellitus, and neurogenic bladder. We report a case of K. pneumoniae emphysematous cystitis in a diabetic male patient being treated with empagliflozin. With this case report, we aim to highlight the potential role of an SGLT-2 inhibitor such as empagliflozin in developing severe urinary infections such as emphysematous cystitis.

Emphysematous cystitis is a rare disease characterized by the presence of gas in the bladder wall and/or cavity. It predominates in diabetic women and its clinical symptoms are varied. We report a case of emphysematous cystitis in an unbalanced diabetic patient, discovered at the stage of septic shock and multivisceral failure following a cardiac decompensation with a rapidly unfavorable evolution. We suggest a probable role of low cardiac output in the genesis of this pathology in some patients.

Although feline idiopathic cystitis (FIC) distresses of many cats, its pathogenesis is unknown and the diagnosis is challenging.
