Capnography for the nonintubated patient in the emergency setting.

BACKGROUND: Multiple studies illustrate the benefits of waveform capnography in the nonintubated patient. This type of monitoring is routinely used by anesthesia providers to recognize ventilation issues. Its role in the administration of deep sedation is well defined. Prehospital providers embrace the ease and benefit of monitoring capnography. Currently, few community-based emergency physicians utilize capnography with the nonintubated patient. OBJECTIVE: This article will identify clinical areas where monitoring end-tidal carbon dioxide is beneficial to the emergency provider and patient. …

Determination of a sedative protocol for use in California sea lions (Zalophus californianus) with neurologic abnormalities undergoing electroencephalographic examination.

Sedation in sea lions exhibiting abnormal neurologic signs may require modification of established sedatior protocols because of the likely interaction between effects of the sedative and physiologic changes in diseased animals The effects of two sedative combinations, 0.07 mg/kg medetomidine and 0.07 mg/kg medetomidine plus 0.2 mg/kg butorphanol, were compared between California sea lions (Zalophus californianus) with signs of neurologic dysfunctior (n=33) and without neurologic signs (n=8). Sedation depth was scored on a scale of 0 (no effect) to 4 (profound sedation) assessed by response to

The role of dexmedetomidine in neurosurgery.

Dexmedetomidine can be used for sedation and analgesia and has been approved for this use by the European Medicines Agency since 2017. It causes an arousable state of sedation, which is beneficial during neurosurgical procedures that require the patient to cooperate with neurological tests (i.e. tumor surgery or implantation of deep brain stimulators). During procedures where monitoring of somatosensory evoked potentials and/or motor evoked potentials is required, dexmedetomidine can be used as an adjunct to general anesthesia with GABAergic drugs to decrease the dose of the latter when these

Applicability, Validity, and Reliability of the Japanese Version of the Behavioral Pain Scale for Critically Ill Mechanically Ventilated Pediatric Patients: A Prospective Cross-Sectional Observational Study.

Background: Pain assessment in critically ill, mechanically ventilated pediatric patients is highly complex owing to communication barriers … moderate interrater reliability (ICC = 0.66, 95% CI = 0.55-0.76; weighted κ = 0.63-0.71). However, deep sedation (defined as a median RASS score ≤ -4 across observers), present in 68.6% of the observations, caused a pronounced floor effect that suppressed behavioral responses, even during painful procedures. Consequently, the reliability of fine motor subscale …

Inhaled Anesthetics: Beyond the Operating Room.

The development of inhaled anesthetics (IAs) has a rich history dating back many centuries. In modern times they have played a pivotal role in anesthesia and critical care by allowing deep sedation during periods of critical illness and surgery. In addition to their sedating effects, they have many systemic effects allowing for therapy beyond surgical anesthesia. In this narrative review we chronicle the evolution of IAs, from early volatile agents such as ether to the contemporary use of halogenated hydrocarbons. This is followed by a discussion of the mechanisms of action of these agents whi…

Nociception level index variations in ICU: curarized vs non-curarized patients - a pilot study.

PURPOSE: Pain is a major physiological stressor that can worsen critical medical conditions in many ways. Currently, there is no reliable monitoring tool which is available for pain monitoring in the deeply sedated ± curarized critically ill patients. This study aims to assess the effectiveness of the multiparameter nociception index (NOL®) in the critical care setting. We compared NOL with traditionally used neurovegetative signs and examined its correlation with sedation depth measured by bispectral index (BIS®) electroencephalographic (EEG) monitoring. METHODS: This retrospective monocentr

Relationship of tonsil size on an airway blockage maneuver in children during sedation.

A previous report suggested that airway compromise without self-correction may occur in pediatric dental patients sedated with chloral hydrate (CH) and nitrous oxide (N2O) and may be interpreted as "deep" sedation. The purpose of this institutionally approved study was to determine 1) the association between the size of the tonsils and 2) the degree of expired carbon dioxide (CO2) and oxygen saturation (SaO2) changes to simulated airway obstruction using the Moore head-tilt maneuver for 30 sec or less. Thirty healthy children (ASA I), aged 22-40 months, were evaluated for tonsil size and sedat

Altered lower brainstem neurophysiological response is associated with mortality in deeply sedated critically ill patients.

BACKGROUND AND OBJECTIVES: Absent cough reflex is associated with mortality in intensive care unit (ICU) patients requiring deep sedation, suggesting that lower brainstem dysfunction contributes to adverse outcomes. We conducted a multicenter observational cohort study to confirm this hypothesis by assessing the peak latency (PL) of the lower brainstem-generated P14 evoked potential (EP), which is slightly increased by sedatives. We aimed to demonstrate that a P14-PL > 16 ms is independently associated with day-28 mortality. PATIENTS AND METHODS: Mechanically ventilated adult patients, comato…

Intravenous methohexital for brief sedation of pediatric oncology outpatients: physiologic and behavioral responses.

OBJECTIVE: In this successor to a preliminary retrospective study, we sought to confirm the apparent safety and efficacy of intravenous methohexital (MHX) for brief, unconscious sedation of pediatric hematology/oncology outpatients undergoing painful, invasive procedures. METHODS: This prospective study was conducted in a children's hospital-based hematology/oncology clinic. Following published monitoring guidelines for deep pediatric sedation, MHX (1.0 mg/kg) was administered immediately before each procedure, 1% xylocaine was given locally, and additional MHX was titrated to maintain minima

Successful repair of mesenteric ischemia in acute type A aortic dissection.

A 64-year-old man with acute type A aortic dissection had superior mesenteric artery occlusion and marked metabolic acidosis. By an emergency laparotomy, bypass grafting from the left external iliac artery to the superior mesenteric artery was performed with great saphenous vein. After deep sedation and antihypertensive management in the intensive care unit, the acidosis resolved, and central repair was carried out. At 10 months postoperatively, his course has been uneventful without mesenteric complications.

Attaining Low Tidal Volume Ventilation During Patient Triggered Ventilation in Sedated Subjects.

BACKGROUND: Low tidal volume (VT) ventilation has become the preferred approach in patients in the ICU. Sedation reduces VT by attenuating respiratory drive. Even in deep sedation, some patients exhibit high VT. We aimed to determine factors associated with low VT ventilation in deeply sedated subjects who exhibited an inspiratory effort by examination of the acid/base balance using the Stewart model. METHODS: The medical records of 630 consecutive subjects admitted to the ICU over 1 y were reviewed retrospectively, and daily data sets of patients with a persistent inspiratory effort, PaO2 /F…
