Dühring on dermatitis herpetiformis.

BACKGROUND: In the late 1800s there was much confusion over the nomenclature and classification of persistent, very itchy vesiculo-bullous eruptions. OBJECTIVE: To review Dühring's contribution to clarifying this confusion. CONCLUSION: By his detailed, perceptive, clinical, and histologic reports over many years, Dühring contributed greatly to organizing our clinical concepts of bullous disorders.

Perioral dermatitis in children.

Perioral dermatitis is a unique skin disorder of childhood. Its exact origin is unknown; it is probably an idiosyncratic response to exogenous factors such as the use of a topical fluorinated corticosteroid or other substances on the face. It is uncommon but not rare. The age of affected children has ranged from 7 months to 13 years, with the median being in the prepubertal period. Boys and girls, blacks and whites are equally affected. Clinical features include the following: (1) absence of systemic symptoms; (2) periorificial distribution (perioral, perinasal, periorbital); (3) skin lesions …

Occupational dermatitis in shoemakers.

In an epidemiological study of occupational dermatitis in 5 different shoe factories, 246 workers were interviewed, examined and patch tested using standard and occupational patch test series. The prevalence of occupational contact dermatitis was 14.6% (36/246): 8.1% (20/246) irritant contact dermatitis (OICD) and 6.5% (16/246) allergic contact dermatitis (OACD). Among the latter, the most common occupational allergens were p-tert-butylphenol-formaldehyde resin and mercaptobenzothiazole. 6% (15/246) presented with hyperkeratosis of the fingertips, while 3.2% (8/246) reported pruritus sine mate…

Immunogenetics of dermatitis herpetiformis.

Dermatitis herpetiformis (DH) is a pruritic, papulovesicular skin disease characterized in part by the presence of granular deposits of IgA at the dermal-epidermal junction, an associated gluten sensitive enteropathy (GSE), and a strong association with specific human histocompatibility leukocyte antigens (HLA). Initial investigations revealed that 60% to 70% of patients with DH expressed the HLA antigen B8 (normal subjects = 21%). Further investigation of the HLA associations seen in patients with DH has revealed an even higher frequency of the HLA class II antigens HLA-DR3 (DH = 95%; normal …

Cosmetics and contact dermatitis.

A complete work-up of most patients with cosmetic allergies can be performed satisfactorily in a dermatologist's office without expensive equipment. After taking a complete history and performing a thorough examination, a biopsy is seldom required. Examine all the patient's cosmetics, eliminate the strong irritants, and proceed with product patch testing. Patch testing with the AAD allergen test tray at the same time is frequently helpful. When results are doubtful or do not fit with the information obtained from the history and examination, supplemental provocative use tests often clarify wha

The prognosis of contact dermatitis.

This article reviews the prognosis of contact dermatitis, particularly of occupational contact dermatitis. Most studies document a poor prognosis for occupational and nonoccupational contact dermatitis. The prognoses of occupational and nonoccupational contact dermatitis, irritant contact dermatitis, and allergic contact dermatitis are similar. Only a minority of studies on the prognosis of occupational contact dermatitis have found that a job change by the affected worker leads to clearing of the dermatitis. Dermatologic and nondermatologic factors associated with a poor prognosis are discuss…

Nickel dermatitis. An update.

Several reports indicate that the prevalence of nickel sensitivity among females in the general population is around 10 per cent and probabl … , although some persons may be unusually sensitive. In dealing with the clinical problems of nickel dermatitis, we must not overlook prophylaxis. It seems important to introduce safe methods of ear piercing (a primary source of nickel hypersensitivity) and to spread information about the health hazard potential of the seemingly harmless sensitivity to nickel …
