Meckel's diverticula are the result of incomplete degeneration of the vitelline duct. It is generally believed that less than 5% of them bec … evices, with their ease of use and low complication rate, make it reasonable to remove any Meckel's diverticulum that easily fits in the device. If a diverticulum found incidentally is so broad-based or short that stapling cannot be done without difficulty, it is unlikely to become symptomatic and should be left undisturbed.

Three cases, two women and one man, aged 27, 47 and 79 years with perforated peptic ulcer of Meckel's diverticulum are reported. All patients had diffuse peritonitis. One-hundred-and-fifty patients with Zenker's diverticulum were reviewed; twelve patients received no treatment; 26 patients were treated endoscopically by dilatation, five of these had a diverticulectomy performed at a later stage. The author analysed the results of operations on 19 patients with emergency diseases of the abdominal organs caused by pathological processes in Meckel's diverticulum. The inflammatory process in the diverticulum led to acute obstruction of the small intestine in 12, acute diverticulitis in 5, and peptic ulcer complicated by perforation of the intestinal wall in 2 patients. Approximately 4% of patients with Meckel's diverticulum will experience complication, the most common of which are intestinal obstruction, gastrointestinal bleeding, acute inflammation, and perforation. Ectopic gastric mucosa can be found in 40-80% of Meckel's diverticula; parietal cells producing acid are not inhibited by regulatory factors, giving rise in some cases to ulcers, which may perforate. A case report of a patient with a perforated ulcer in a Meckel's diverticulum is given. Meckel's diverticula should be removed surgically when they are diagnosed or when they are found by chance during surgery because of the risk of complications.

Two middle-aged patients are reported who came to laparotomy with a presumptive diagnosis of small bowel tumour. There was no overt evidence of small bowel obstruction in either case. They were found at laparotomy to have intussusception of an invaginated Meckel's diverticulum. This condition should be considered in the differential diagnosis of ileal tumours.
