Prevalence and associated factors of serious unintentional physical injury, road traffic injury and near drowning experience among a nationally representative sample of school-aged adolescents in the Philippines in 2019.

BACKGROUND: The aim of this study is to report on the frequency and correlates of serious unintentional physical injury (SPI), road traffic injury, and near drowning experience as well as the mediation of exposure to interpersonal violence in the relationship between substance use and SPI among adolescents in Philippines in 2019. METHOD: The 2019 Philippines Global School-based Student Health Survey (GSHS), a nationally representative survey of teenagers aged 11 to 18 (mean age 13.8 years, Standard Deviation-SD=1.5) that used a multistage sampling technique, provided the study's data. In orde…

Diatoms in forensic analysis: A practical approach in rats.

A diagnosis of drowning is a challenge in legal medicine, as there is generally a lack of pathognomonic findings indicative of drowning. Diatom analysis has been considered very supportive for a diagnosis of drowning, although the test is still controversial for some investigators. We assessed diatoms association with drowning in the peripheral tissues of drowned rats and the effects of the drowning medium on the diatom yield. A modified acid digestion method was optimised for diatom recovery in water and rat tissues. Eighteen adult Wistar rats were employed for the study, subdivided into six …

Accidents and child abuse in bathtub submersions.

Non-accidental bath drowning is an infrequently reported form of child abuse. Details of 44 children who suffered from drowning or near drowning in the bath were analysed from a two year (1988-9) UK study to investigate factors that might point to abuse. Cases of near drowning were notified through the British Paediatric Surveillance Unit inquiry system and drowning cases from the Office of Population Censuses and Surveys, the Scottish Government Record Office, and the Northern Ireland Office. In 28 cases the story was of accidental submersion with a baby of modal age 9 months being left unsup…

2024 American Heart Association and American Academy of Pediatrics Focused Update on Special Circumstances: Resuscitation Following Drowning: An Update to the American Heart Association Guidelines for Cardiopulmonary Resuscitation and Emergency Cardiovascular Care.

Drowning is the third leading cause of death from unintentional injury worldwide, accounting for 7% of all injury-related deaths. The World Health Organization estimates that there are ≈236 000 deaths due to drowning worldwide each year. Significant efforts have focused on creating systems to prevent drowning, but an average of 4000 fatal and 8000 nonfatal drownings still occur annually in the United States-likely an underestimate. Drowning generally progresses from initial respiratory arrest due to submersion-related hypoxia to cardiac arrest; thus, it can be challenging to distinguish respir…

Drowning is the third leading cause of death from unintentional injury worldwide, accounting for 7% of all injury-related deaths. In the United States, drowning is the leading cause of death in children 1 to 4 years of age and second leading cause of death due to unintentional injury in those aged 5 to 14 years. Drowning generally progresses from initial respiratory arrest due to submersion-related hypoxia to cardiac arrest; thus, it can be challenging to distinguish respiratory arrest from cardiac arrest because pulses are difficult to accurately palpate within the recommended 10-second windo…

Metagenomic profiling reveals lung multi-kingdom microbes as forensic markers for aquatic corpses investigation.

The forensic investigation of corpses recovered from aquatic environments presents a major practical challenge. Recent studies have demonstrated that the bacterial community in the lung serves as a valuable indicator for diagnosing drowning, determining the drowning medium and estimating postmortem submersion interval (PMSI). However, the application and significance of lung multi-kingdom microbiome (archaea, eukaryota, and viruses) remains inadequately characterized. Meanwhile, the insufficient sequencing depth of commonly employed techniques, such as amplicon sequencing, restricts our unders…

Submersion injuries in children and adults.

Drowning and near drowning remain a common cause of childhood death and disability. Toddlers aged one through four drown in private swimming pools. Submersions greater than 10 minutes and lack of CPR at the scene or the need for greater than 20 minutes of resuscitation portends a poor prognosis. Management of respiratory failure without neurologic impairment has the most successful outcome. Prevention of drowning morbidity is dependent on constant parental supervision, and immediate and expert CPR.

Can pulmonary foam arise after postmortem submersion in water? An animal experimental pilot study.

It is difficult to differentiate drowning from postmortem submersion. Pulmonary foam can be found in bodies retrieved from water. It is unknown whether foam is a result of drowning or if it also forms after postmortem submersion. We divided deceased piglets into three groups: postmortem saltwater submersion (N = 20), postmortem freshwater submersion (N = 20) and dry-land controls (N = 20). All carcasses underwent endoscopic examination within 24 h of death and the presence of external and internal pulmonary foam was scored. No external foam was detected in the postmortem freshwater or the post…

Detection of marine and freshwater bacterioplankton in immersed victims: Post-mortem bacterial invasion does not readily occur.

We previously applied our method of detecting marine or freshwater bacterioplankton (bacteria) in the blood of immersed victims as a marker of drowning. However, we did not confirm the absence of post-mortem bacterial invasion during immersion. Here we examined the nature of bacterioplankton in blood samples from 21 immersed and 4 non-immersed cadavers. We found only freshwater bacterioplankton in the blood of two victims that were retrieved from the sea or an estuary inhabited by marine bacterioplankton even though one victim was highly putrefied. The results of diatom testing suggested that …

Diatom testing for closed organs should be carefully performed and interpreted: the effect of post-mortem change and resuscitation.

INTRODUCTION: Diatoms inhaled with a drowning medium are thought to reach closed organs, including the kidney and liver, through circulation; however, this is uncertain. Thus, to assess this doubt, we evaluated the relationship between resuscitation or decomposition and diatom concentration in closed organs (kidney and liver) (Cc). MATERIALS AND METHODS: We retrospectively reviewed our diatom test results in 62 natural water- and five bathtub-water drowning consecutive forensic autopsies. Tukey's honestly significant difference (HSD) test and multiple regression analysis were performed in nat…

Postmortem intravascular bubbling: a decompression artifact?

The relationships between drowning occurring in divers using compressed air, with subsequent recovery of the body to the surface, and the finding of intravascular gas bubbles at autopsy were studied. Guinea pigs were exposed to compressed air in a hyperbaric chamber at various depths [98, 294, and 392 kPa (1,3, and 4 atm)] for various intervals and then drowned at depth. Various decompression schedules were used in returning the drowned animals to the surface. Autopsy examination indicated that large and small intravascular gas bubbles may be found, in the absence of traumatic air embolism or …

Potential of photoautotrophic microbial organisms in deciphering forensic issues.

In forensic pathology, solving the crime mystery of death due to drowning still remains a challenging issue. The amalgamation of autopsy findings and comparative study of diatoms recovered from the victim's body organs and suspected drowning site help to decipher the cause of death due to drowning or post-mortem immersion. Since the correct interpretation of the cause of death is an important criterion to provide justice to the victim, therefore, the main objective of our study is to throw light on the application of photoautotrophic micro-algal organisms, known as Diatoms, in solving seven ca…

Cost-benefit analysis of universal pool fencing for Texas households with young children.

BACKGROUND: Texas bears a disproportionate burden of pediatric drowning in the United States. Functional pool fencing prevents pediatric drownings, but its economic impact on drowning-related costs remains unclear. We estimated the potential cost savings of implementing a universal pool fencing program for households with young children. METHODS: We conducted a cost-benefit analysis of universal pool fencing for Texas households with one or more children ages 1-9 years. Drowning burden was estimated from Texas data with a case-fatality rate of 1:6. Direct medical and lifetime indirect costs p…

Evaluation of postmortem calcium and magnesium levels in the pericardial fluid with regard to the cause of death in medicolegal autopsy.

Previous studies have suggested the usefulness of postmortem serum calcium (Ca) and magnesium (Mg) for investigating cause of death. The present study investigated their levels in the pericardial fluid of serial autopsy cases of adults within 48 h postmortem (n=385), including fatalities from blunt injury (n=57), sharp instrument injury (n=9), mechanical asphyxiation (n=28), salt- and freshwater drowning (n=14 and n=61, respectively), fire fatality (n=35), intoxication (n=23), hypothermia (cold exposure, n=12), hyperthermia (heat stroke, n=7), acute cardiac death (ACD, n=86), pneumonia (n=9) a…

Anoxia Effects in Asphyxiated Green Sea Turtles (Chelonia mydas) Caught in an Artisanal Fishing Net on the Coast of Brazil.

Among studies of drowning in green sea turtles (Chelonia mydas), none have associated drowning with injuries of organs other than the lung. We describe the gross and microscopical findings in 23 green sea turtles found dead in a fishing net. Deprived of air, these animals experienced hypoxia and anoxia before dying, which caused congestion, hydropic degeneration and necrosis in several organs. There was no evidence of an alternative cause of death. These findings demonstrate a pattern characteristic of death by asphyxia caused by drowning.

Evaluation of postmortem serum calcium and magnesium levels in relation to the causes of death in forensic autopsy.

There appears to be very poor investigation of postmortem serum calcium (Ca) and magnesium (Mg) for diagnostic evidence to determine the cause of death. The aim of the present study was a comprehensive analysis of the serum levels in relation to the causes of death in routine casework. Autopsy cases (total, n=360; 5-48 h postmortem), including blunt injury (n=76), sharp injury (n=29), asphyxiation (n=42), drownings (n=28: freshwater, n=11; saltwater, n=17), fire fatalities (n=79), methamphetamine (MA) poisoning (n=8), delayed death from traumas (n=37), and acute myocardial infarction/ischemia …

Detection of diatoms in a case of mud aspiration at a coastal area.

The source of diatoms detected in non-drowned bodies have been attributed to postmortem invasion, contamination during autopsy or diatom tes … e may have entered the sea. Autopsy revealed intratracheal mud, with no obvious findings indicating drowning, suggesting that he died from mud aspiration probably due to hypothermia or non-lethal ethanol intoxication. In the diatom test, 10-100 diatoms/g were detected in bilateral lung samples, which were similar to those found in the intraoral and intratr …

Difference in molecular pathology of natriuretic peptides in the myocardium between acute asphyxial and cardiac deaths.

In investigating death due to mechanical asphyxiation and drowning, a cardiac attack is important for discriminating between possible causes of death and as a contributory factor in death processes; however, general pathologies involving visceral congestion are often similar. The present study compared terminal cardiac dysfunction in these fatalities using the molecular pathology of atrial and brain natriuretic peptides (ANP and BNP) in the myocardium as markers of cardiac strain. Both mechanical asphyxiation (n=27) and drowning (n=23) showed significantly lower ANP and BNP mRNA expressions in…

Morphometric investigation of death by asphyxia.

The aim of this study was to investigate the possibility of distinguishing deaths by asphyxia from those due to natural causes by comparing morphometric measurements in lungs. The study population comprised 27 subjects: 14 cases of death by asphyxia (hanging or drowning) and 13 cases of sudden natural death. Eighty parenchyma sections were used for each subject. Data were collected by computerized image analysis. Measurements aimed at quantifying, as percentages, pulmonary parenchyma (mean values of positive-fraction areas [PFA] and their standard deviations). Subjects who died of natural caus…
