Polyps are reported in 3-5% of adults undergoing upper gastrointestinal endoscopy. The most common types in the stomach are fundic gland polyps, followed by hyperplastic polyps and adenomas. Gastric metaplasia, adenomas and inflammatory polyps are the most common polyps in the duodenum. Treatment options and aggressiveness should be balanced by detailed knowledge of the malignant potential of the different polyps as well as the symptoms, the patient's age and comorbidity.

We report a case of intestinal obstruction due to intramural hematoma of the duodenum following therapeutic endoscopy for a bleeding duodenal ulcer in a patient with liver cirrhosis. A 44-year-old man was admitted to our hospital with severe epigastralgia, nausea and tarry stool. Two years previously he had undergone endoscopic sclerotherapy for esophageal varices caused by alcoholic liver cirrhosis. We present a case of an anomalous pancreaticobiliary junction (a long common channel) that was clearly demonstrated by extraductal ultrasonography with a transduodenoscopic miniprobe placed in the duodenal lumen. The present case suggests a potential indication for the ultrasound miniprobe, in extraductal ultrasonography, in the pancreatobiliary region. In this method, the position of a miniprobe can readily be adjusted under endoscopic guidance, unlike ordinary endoscopic ultrasonography.

To study prognostic value of chemiluminescent (CL) test of duodenal ulcer treatment efficacy with microwave resonance therapy (MWRT), 39 duodenal ulcer patients were divided into two groups differing only by the size of ulcer defect. A 9-year-old boy with Henoch-Schönlein purpura had a duodenal ulcer. Endoscopic injection with pure ethanol was performed on a pulsating visible vessel in the third part of the duodenum, resulting in complete hemostasis. A bleeding ulcer, although rare, may be a serious gastrointestinal complication of Henoch-Schönlein purpura and may require aggressive intervention.

Thirty-four consecutive patients presenting to the Endoscopy clinic of the Ahmadu Bello University Teaching Hospital, Zaria with symptoms of upper abdominal pain were investigated for chronic duodenitis by endoscopy and histology and for associated diseases. We have reviewed all endoscopies performed in our hospital between 1977 and 1986. During that period, 1337 endoscopies were performed to identify bleeding from peptic ulcers. Excluded were cases in which a predisposing factor was found, such as the use of ulcerogenic drugs. Also excluded were chronic or critically ill patients. The remaining 540 cases were reviewed. During a one year period (1988-1989), 40 consecutive patients were submitted to emergency gastroduodenoscopy because of severe gastroduodenal bleeding. Twenty-four cases of duodenal adenomas which did not originate at the ampulla of Vater are discussed. The radiographic and clinical features of these lesions are presented together with a review of 120 cases previously reported in the literature. The role of barium studies and endoscopy in the evaluation of these adenomas is emphasized. A diameter of more than 5 cm represents a criterion favoring malignant degeneration of such non-Vaterian adenomas.
