Trans-canal tragal cartilage myringoplasty; a comparative analysis of endoscopic and microscopic approaches.

BACKGROUND: The advent of Endoscopic Ear Surgery (EES) has allowed otologists an improved view of the surgical field compared with conventional Microscopic Ear Surgery (MES). EES presents different challenges for surgeons and a learning curve is necessary. AIMS: The purpose of this study was to compare the efficacy of EES and MES for trans-canal tragal cartilage myringoplasty, an entry level EES. METHODS: We retrospectively analysed patients who underwent push through trans-canal tragal cartilage myringoplasty in our institution over 5 years (2016-2020). Exclusion criteria were: patients wit

Cartilage tympanoplasty methods: proposal of a classification.

UNLABELLED: The 23 known cartilage tympanoplasty methods to reconstruct the eardrum are classified in six main groups. Each method is briefly defined, described, and illustrated: GROUP A: Cartilage tympanoplasty with palisades, stripes, and slices. The eardrum is reconstructed by several, various, full-thickness pieces of cartilage with attached perichondrium on the ear canal side. In this group six different methods are described. GROUP B: Cartilage tympanoplasty with foils, thin plates, and thick plates, not covered with the perichondrium. In this group four methods are included. GROUP C:

Follow-up study of reconstructed auricle in microtia: comparison of the results of the rib cartilage framework and the silicone rubber framework methods.

Plastic surgery for microtia had achieved fairly consistent good results since TANZER (Plast. Reconstr. Surg. 23: 1-15, 1959) and recently the modified methods have become popular. The authors did plastic surgery of auricles with the rib cartilage framework method from 1968 until 1986, and with the silicone rubber framework method from 1975 until 1986, and observed each postoperative course for 2 to 10 years. The operative results and the merits and the demerits of each method are reported here. The subjects were 49 ears for the rib cartilage framework method (17 of the former period, 1968-197

Evaluation of Techniques and Outcomes in Pediatric Partial Ear Reconstruction: A 15-Year Experience.

BACKGROUND: Partial ear reconstruction of large defects challenges surgeons to blend native and reconstructive auricular structural and soft tissue segments while preserving natural ear contours, accommodating variable etiologies and anatomical involvement. For pediatric patients, consideration of skin coloration, scar location and visibility, readiness for surgery, and compliance with postsurgical care contribute heavily to what is considered a successful outcome. This study reviews our institutional experience with techniques to manage large partial auricular defects in pediatric patients wh

[Relapsing polychondritis. A case report].

The authors describe a case of "relapsing polychondritis" showing that the characteristic manifestation of the disease on nasal and ear cartilage took place at a later stage than in the eye vessels, the inner ear and the trunk and valves of the aorta. The authors therefore consider the term "relapsing polychondritis" to be somewhat restrictive and claim that owing to the systemic diffusion of the disease in all vessels, R.P. should be included among systemic vasculitis diseases.

Biomechanical evaluation of surgical correction of prominent ear.

BACKGROUND: Mustardé sutures and conchal setback are widely used for surgical correction of prominent ear, and numerous cartilage-manipulation techniques accompanying these two methods are also available. However, it is unknown how each technique works biomechanically. The effects of otoplasty were evaluated by finite element analysis. METHODS: Data of eight prominent ears were obtained with a noncontact three-dimensional digitizer, and three-dimensional auricular cartilage models were produced. These models were modified to cartilage-manipulation models: scaphal-incision, abrasion, conchal-i

Auricular Reconstructive Surgery Improvement to the Firmin Technique for Placing an Earring.

Microtia has an incidence of 1 in 7000 to 8000 births. Ear reconstruction has 2 main aims: reconstructive and aesthetic, and a considerable number of patients ask for an earring at the end of their treatment. Herein, we explain our team's modification to the Firmin technique, perforating the lowest part of the autologous cartilage framework (Parri's modification). The orifice is cartilaginous and the skin covering both sides is easily perforable without contacting the rest of the framework. In conclusion, our modification for placing an earring is simple; it does not increase the surgical time

Simultaneous Vibrant Soundbridge Implantation and 2nd Stage Auricular Reconstruction for Microtia with Aural Atresia.

Aural atresia and severe microtia are associated malformations that result in problems with hearing and cosmesis, associated speech and language difficulties and diminished self-esteem. In cases where middle ear ossiculoplasty and aural atresia canalplasty are expected to give poor hearing outcomes that would eventually require the use of hearing aids, bone anchored hearing aids or active middle ear implants may be better options. This case report describes a simultaneous Vibrant Soundbridge implantation and 2(nd) stage auricular reconstruction with rib graft cartilage for an 11-year-old boy w

Biomechanical modeling and design optimization of cartilage myringoplasty using finite element analysis.

The purpose of this study was to determine the acoustic transfer characteristics of cartilage for optimal cartilage myringoplasty. In order to do so, we developed a cartilage plate/tympanic membrane-coupled model using finite element analysis. Cartilage specimens of the tragus were obtained from fresh human cadavers, and the parameters of the tragus were determined by curve fitting and cross-calibration. A cartilage plate was used to repair an eardrum perforation, and the new coupled tympanic membrane-cartilage complex was loaded into our 3-dimensional biomechanical model of the middle ear for

Evaluation and processing of physical properties of anti-calcified glutaraldehyde-treated three-dimensional cultured cartilage tissues.

Reconstruction using cartilage tissue is necessary to address deformities of the nose, ears, and maxillofacial region in several cases. However, autologous cartilage tissue transplantation is limited in the amount that can be harvested owing to invasiveness to the human body. Moreover, artificial materials such as implants cannot be used in many situations, given their potential to induce reactions to foreign bodies. Therefore, there is a growing demand for biomaterials that are less likely to cause foreign body reactions. Given that a tissue with a functionally superior three-dimensional (3D)

[Reconstruction of the tympanic membrane applying cartilage: indications, techniques and results].

Autogenous cartilage-perichondrium-composite-grafts have become essential and well established transplant materials in reconstructive middle ear surgery. They have proven to be particularly appropriate for surgical situations which benefit from the bradytrophy and stiffness of the cartilage (adhesive otitis, retraction pockets, subtotal tympanic membrane defects, revision tympanoplasty). Of the numerous techniques available, the perichondrium-island technique and the palisade-cartilage technique are described in detail in this review, also regarding modifications and comments in the literature
