Compartment Syndrome in Children: Diagnosis and Management.

Compartment syndrome (CS) can present differently in children than in adults. Increased need for analgesics is the first sign of evolving CS in children. Children with supracondylar humeral fractures, floating elbow injuries, operatively treated forearm fractures, and tibial fractures are at high risk for CS. Elbow flexion beyond 90° in supracondylar humeral fractures and closed treatment of forearm fractures in floating elbow injuries are associated with increased risk for CS. Prompt diagnosis and treatment with fasciotomy in children result in excellent long-term outcomes.

Elbow arthroscopy: treatment of the thrower's elbow.

The athlete's elbow has been described as one of the last frontiers in orthopaedic sports medicine. It has been considered separately from other athletic injuries because of the unique constellation of pathology that results from repetitive overhead throwing. Tremendous gains in understanding the complex interplay between the dynamic and static stabilizers of the athlete's elbow have occurred over the past decade. The desire to treat these injuries in a minimally invasive manner has driven the development of techniques and instrumentation for elbow arthroscopy, a successful and essential techn

Elbow Fracture-Dislocations: Determining Treatment Strategies.

Elbow dislocations represent common injuries. A quarter of these injuries involve at least 1 fracture. The sequel of elbow fracture-dislocations can be fraught with complications, including recurrent instability, posttraumatic arthritis, elbow contracture, and poor functional results. The 3 main patterns of injury are valgus posterolateral rotatory instability, varus posteromedial rotatory instability, and transolecranon fracture-dislocation. This article discusses each pattern individually, including the anatomy, the typical injury pattern, and treatment strategies. It also discusses common c

Epihyseal Separation of Lower end Humerus in A Neonate-Diagnostic and Management Difficulty.

INTRODUCTION: Birth injuries are common phenomenon. But commonly encountered birth injuries are clavicle fracture, humerus fractures and uncommonly femur fracture. Incidence of Distal humerus Epiphyseal injuries as birth injury are rare. As the first secondary centre around elbow-capitellum appears at 3-9 months, its radilological assessment of alignment with elbow or the radius shaft is not possible in neonates. It is difficult to differentiate elbow dislocation from epiphyseal injury in neonates. CASE REPORT: Here we are reporting a case of lower end humerus epiphyseal injury, as birth inju

Upper extremity injuries in golf.

Golf is an asymmetric sport with unique patterns of injury depending upon the skill level. Higher handicap players typically experience injuries that result from swing mechanics, whereas lower handicap and professional players have overuse as the major cause of their injuries. The majority of shoulder injuries affecting golfers occur in the nondominant shoulder. Common shoulder injuries include subacromial impingement, rotator cuff pathology, glenohumeral instability, and arthritis involving the acromioclavicular and/or glenohumeral joints. Lead arm elbow pain resulting from lateral epicondylo

Ukrainian Orthopedic Trauma Experience in Managing Wartime Injuries of the Upper Extremity.

Warfare injuries of the upper extremity constitute approximately 45% of all injuries to the extremities. We review a warfare treatment algorithm for injuries to the shoulder, elbow, soft tissue, and peripheral nerves of the upper extremity. A very challenging problem is reconstruction of the elbow joint after gunshot injuries, with defects of distal humerus and proximal forearm. Warfare injuries of the peripheral nerves are part of a polystructural injury, with bone fractures and soft tissue defects. They are combined nerve injury patterns with the best results when treatment occurs between 3

Surgical reconstructions for adult brachial plexus injuries. Part I: Treatments for combined C5 and C6 injuries, with or without C7 injuries.

Brachial plexus injuries will cause a significantly decreased quality of life. Patients with upper arm type brachial plexus injuries, which means C5 and C6 roots injury, will lose their shoulder elevation/abduction/external rotation, and elbow flexion function. Additional elbow, wrist, and hand extension function deficit will occur in patients with C7 root injury. With the advances of reconstructive procedures, the upper arm brachial plexus injuries can be successfully restored through nerve repair, nerve grafting, nerve transfer, muscle / tendon transfer and free functioning muscle transfer.

Extensive soft tissue lesions in redislocated after simple elbow dislocations.

BACKGROUND: The majority of simple elbow dislocations (no associated fractures) can be treated nonoperatively with a short period of immobilization followed by guided aftercare. This case series describes the soft tissue injuries in a rare subset of patients in whom the elbow redislocated despite adequate immobilization. METHODS: During a 6-year period, 8 patients were identified. They were all treated with reduction and casting in 90° of flexion or more. At 1 week of follow-up, redislocation had occurred in all patients and open soft tissue repair was performed. The injuries were documented

Surgical management of unstable elbow dislocation without intra-articular fracture.

The purpose of this study was to describe soft tissue injury patterns and report the clinical results of primary ligament repair with use of protected early mobilisation in unstable elbow dislocations with pure capsulo-ligamentous injuries. Twenty-four patients who presented with traumatic unstable elbow dislocation without associated intra-articular fracture were reviewed. Anatomical repair was performed using metal anchor screws and the bone tunnel method. Ligament avulsion was noted in 55% for the medial collateral ligament, 80% for the lateral collateral ligament, 60% for the flexor tendon

Coronoid fractures : how do we best classify them and what is the 'true terrible triad'?

The 'terrible triad' injury of the elbow is defined as a dislocation of the elbow with concomitant fractures of the radial head and coronoid process. The term was based on reports of poor outcomes following this pattern of injury. However, the 'terrible triad' concept has meant that some very complex elbow fracture-dislocations are considered alongside more benign injuries, leading to problems with our understanding and interpretation of the literature. This annotation explores how our knowledge of elbow stability has evolved, and how the importance of the coronoid to this is becoming increasi

[Diagnosis of upper arm, elbow, and proximal lower arm fractures].

The complex anatomy of the elbow joint enables a wide range of movement and complex functions in everyday and professional life. Typical injuries of this joint include a variety of different pathologies. Due to overlaying structures in plain radiographs, diagnosis of injuries to the elbow joint places high demands on the treating physicians and often needs further diagnostic imaging. The following article will give an overview of the most common of these injuries and their diagnosis.

Coronoid fractures.

Coronoid fractures are rarely isolated injuries; they are most commonly encountered in association with other elbow injuries as part of 3 major instability patterns. Type 1 fractures are usually associated with terrible triad injuries. Type 2 fractures are associated with varus posteromedial rotatory instability. Type III fractures are associated with transolecranon fracture-dislocations. Current recommendations are to repair all coronoid fractures associated with elbow instability, regardless of fragment size. Surgical techniques are described in detail.

Elbow collateral ligament repairs with suture anchors after acute complex elbow dislocations have favorable outcomes; clinical results at a mean follow-up of eight years, a stress radiography-based study.

BACKGROUND: This study aimed to compare the medium- to long-term results of elbow collateral ligament repairs performed with suture anchors. METHODS: Patients undergoing surgery for elbow collateral ligament repairs between 2011 and 2023 were retrospectively analyzed. We included patients who had undergone surgery for complex elbow dislocations. Patients were excluded from the study if they had a previous infection, a fracture, an operation on the same elbow, a stiff elbow, or a follow-up period of less than 1 year. For the functional evaluation, the range of motion (ROM) and the Mayo Elbow P

Intraosseous Plate Reduction and Fixation of Complex Proximal Ulnar Fractures: A Report of 2 Cases.

CASE: We describe 2 cases using a novel technique of intraosseous plating of the coronoid as a reduction tool and fixation construct for complex elbow fracture dislocations. Each patient sustained traumatic, complex injuries of the elbow and were radiographically healed at the final follow-up. CONCLUSION: Elbow fracture dislocations are complex injuries because of the interplay between primary and secondary stabilizers. This technique provides access to fracture fragments with minimal to no soft-tissue disruption and does not obstruct additional reduction or fixation of the injury.

Re: Zyluk A. and Szlosser Z. The results of carpal tunnel release for carpal tunnel syndrome diagnosed on clinical grounds, with or without electrophysiological investigations: a randomized study. J Hand Surg Eur. 2013, 38: 44-9.

OBJECTIVES: The aim of this study was to describe, using video analysis, the mechanisms of head injuries and of incidents with a high risk of head injury in elite football. METHODS: Videotapes and injury information were collected prospectively for 313 of the 409 matches played in the Norwegian (2000 season) and Icelandic (1999 and 2000 season) professional leagues. Video recordings of incidents where a player appeared to be hit in the head and the match was consequently interrupted by the referee were analysed and cross referenced with reports of acute time loss injuries from the team medica

Combined Ipsilateral Humeral Shaft and Galeazzi Fractures Creating a Floating Elbow Variant.

"Floating elbow" injuries of the arm traditionally represent a combination of humeral shaft and forearm fractures which require anatomic rig … f motion of the elbow and wrist resulted in an excellent clinical and radiographic result. Floating elbow injuries and their variants should be promptly recognized as early anatomic reduction, and rigid internal fixation can allow for good elbow function with minimization of stiffness.

Interosseus Membrane Reconstruction Following a Forearm Injury: A Case Report.

Background  Essex-Lopresti injuries are complex, frequently missed forearm/elbow injuries consisting of a fracture of the radial head with disruption of both the distal radioulnar joint (DRUJ) and the interosseous membrane (IOM). Ideal management of these injuries remains controversial, especially in relation to the IOM, due in part to underdiagnosis of IOM injury and incomplete understanding of the IOM role. Methods  A 43-year-old man sustained an open radial shaft fracture with associated radial head subluxation, coronoid base fracture, and DRUJ injury with ulnar head subluxation. He was tak…

Medial epicondyle fractures in the pediatric population.

Humeral medial epicondyle fractures in the pediatric population account for up to 20% of elbow fractures, 60% of which are associated with elbow dislocation. Isolated injuries can occur from either direct trauma or avulsion. Medial epicondyle fractures also occur in combination with elbow dislocations. Traditional management by cast immobilization increasingly is being replaced with early fixation and mobilization. Relative indications for surgical fixation include ulnar nerve entrapment, gross elbow instability, and fractures in athletic or other patients who require high-demand upper extremi
