Electroconvulsive therapy as a treatment modality in adolescent psychiatric patients.

OBJECTIVE: Due to the relatively rare clinical application of electroconvulsive therapy (ECT) in adolescent psychiatric patients and its underrepresentation in the literature, the aim of this study was to explore indications for ECT in adolescents by systematically describing the clinical characteristics of patients treated at the Child and Adolescent Psychiatry Department, Aarhus University Hospital. METHODS: Data were extracted from the Business intelligence (BI) portal, comprising comprehensive electronic medical records. The study included 25 patients under the age of 21, who received ECT…

Prevention of Recurrent Cases of Emergence Delirium in Electroconvulsive Therapy.

Electroconvulsive therapy (ECT) is an effective treatment for severe and treatment-resistant depression but may be complicated by postictal/emergence delirium. We report the case of a 56-year-old woman with psychotic, treatment-resistant depression who developed severe emergence delirium after each of her first four ECT sessions performed under thiopental anesthesia, with the need for repeated administration of benzodiazepines and propofol to terminate symptoms. Concurrently, her lithium medication dose was gradually reduced. After switching the anesthetic agent from thiopental to propofol at …

[Call for the standardized application of the electroconvulsive therapy sessions in depression].

Electroconvulsive therapy (ECT) is an effective treatment for depression, but it is associated with severe side effects such as memory impairment. The number of ECT sessions is a key determinant of clinical efficacy and adverse effects, such as memory impairment. The frequency of ECT is the main factor affecting clinical efficacy and side effects, yet its clinical application has long relied on an empirical range of 6-12 sessions, with significant controversy and practice heterogeneity, leading to insufficient or excessive treatment. This article systematically analyzes the current status of EC…

Use of electroconvulsive therapy in Nordic and western European countries.

BACKGROUND: Electroconvulsive therapy (ECT) is a safe and effective medical treatment, yet provision varies internationally. We provide an up-to-date review of ECT practice in Nordic and western European countries. METHODS: European experts contributed data from peer-reviewed literature, governmental publications, unpublished administrative datasets, and hitherto unpublished surveys. The primary outcome was the number of individuals treated with ECT in each country per year. Primary analysis compared the annual treated person rate per million population in the most recent year since 2015 for …

Glia as a New Target for Therapeutic Actions of Electroconvulsive Therapy.

Although electroconvulsive therapy (ECT) has immediate and profound effects on severe psychiatric disorders compared to pharmacotherapy, the mechanisms underlying its therapeutic effects remain elusive. Increasing evidence indicates that glial activation is a common pathogenetic factor in both major depression and schizophrenia, raising the question of whether ECT can inhibit glial activation. This article summarizes the findings from both clinical and experimental studies addressing this key question. Based on the findings, it is proposed that the suppression of glial activation associated wi…

The effects of electroconvulsive therapy on cognition: an exploratory retrospective study.

OBJECTIVES: Electroconvulsive therapy (ECT) is one of the most effective treatments for depression, but worries about cognitive side effects remain. This retrospective study evaluated cognitive outcomes and the antidepressant efficacy of ECT in a real-life sample of patients with treatment-resistant uni- or bipolar depression. METHODS: We included 90 depressed inpatients aged 49 ± 13.8 (SD) years who underwent 10 ± 2.1 (SD) unilateral or bitemporal ECT treatments and completed an extensive pre- and post-treatment psychological test battery. The Hamilton Depression Rating Scale (HAMD) and the …

Electroconvulsive therapy and occupational functioning in individuals with major depression.

PURPOSE: Electroconvulsive therapy (ECT) is a well-established treatment for severe depressive episodes, but its effect on subsequent occupational functioning remains unclear. This study examined labour market trajectories among individuals with major depression treated with ECT, compared to similar individuals with depression who did not receive ECT. METHODS: This population-based cohort study included all patients aged 15-55 with a first-time hospital contact for major depression (ICD-10 codes F32 and F33) in Denmark between 2000 and 2022. Patients treated with ECT were identified using the…

Effectiveness of Electroconvulsive Therapy in Postpartum Psychosis: A Systematic Review.

Postpartum psychosis is a severe psychiatric condition that emerges shortly after childbirth. Electroconvulsive therapy provides rapid symptom relief, particularly in severe and treatment-resistant cases. Despite evidence supporting its effectiveness, the use of electroconvulsive therapy in postpartum psychosis remains limited and unstandardized. This systematic review evaluates the clinical effectiveness of electroconvulsive therapy in treating postpartum psychosis. A comprehensive search of PubMed/MEDLINE, Cochrane, SciELO, SCOPUS, and Web of Science identified 255 studies, of which 7 met th…

A case report of ineffective electroconvulsive therapy for chronic pain.

BACKGROUND: Somatic symptom disorder (SSD), which includes chronic pain, is a common mental disorder characterized by significant functional impairment and other psychiatric comorbidities. Electroconvulsive therapy (ECT) has been proposed as a potential treatment for refractory chronic pain. However, evidence supporting its efficacy is limited and/or low quality. We present a case of SSD with chronic pain in which ECT was ineffective. CASE PRESENTATION: The patient was a 63-year-old man with chronic pain in the lower back, buttocks, thighs, and soles of the feet. The duration of his chronic p…

Electroconvulsive therapy in the maintenance phase of psychotic unipolar depression.

OBJECTIVE: To determine whether the rates of readmissions and suicide vary in psychotic unipolar depression based on whether patients receive maintenance electroconvulsive therapy (M-ECT) following the initial series of ECT, and to examine if there is an age-dependent association. METHODS: We used Swedish national registries to identify hospitalized patients with psychotic unipolar depression, treated 2008-2019 who received ECT during their hospital stay. The patients who received subsequent M-ECT within 14 days after discharge were compared with those who did not. The primary composite outco…

What Is Brain Damage and Does Electroconvulsive Therapy Cause It?

Surveys show public misperceptions and confusion about brain damage and electroconvulsive therapy (ECT). Fictional movies have misrepresented ECT to suggest brain damage and to ridicule mental illness and psychiatric patients. "Brain damage" has become a colloquial expression without consistent meaning. In contrast, brain injury is the medical term for destruction of brain cells, such as from kinetic impact (concussion), hypoxia, or infection. Studies of both high-resolution magnetic resonance imaging (MRI) and enzyme assays find that causes of brain injury are accompanied by observable struct…

Investigation of Neurofunctional Changes Over the Course of Electroconvulsive Therapy.

BACKGROUND: Electroconvulsive therapy (ECT) is an effective treatment for patients suffering from depression. Yet the exact neurobiological mechanisms underlying the efficacy of ECT and indicators of who might respond best to it remain to be elucidated. Identifying neural markers that can inform about an individual's response to ECT would enable more optimal treatment strategies and increase clinical efficacy. METHODS: Twenty-one acutely depressed inpatients completed an emotional working memory task during functional magnetic resonance imaging before and after receiving treatment with ECT. N…

Factors associated with early and late response to electroconvulsive therapy.

OBJECTIVE: Electroconvulsive therapy (ECT) is an effective treatment for severe depressive symptoms, yet more research is needed to examine predictors of treatment response, and factors associated with response in patients not initially improving with treatment. This study reports factors associated with time to response (early vs. late) to ECT in a real-world setting. METHODS: This was a retrospective, single-center cohort study of patients endorsing moderate to severe depressive symptoms using the Quick Inventory of Depressive Symptomatology (QIDS; QIDS>10). Response was defined as 50% or g…
