Atomic Force Microscope Characterization of the Bending Stiffness and Surface Topography of Silicon and Polymeric Electrodes.

Implanted electrodes in the brain are increasingly used in research and clinical settings to understand and treat neurological conditions. However, a foreign body response typically occurs after implantation, and glial encapsulation of the device is a commonly observed. Multiple factors affect how gliosis surrounding the implantable electrodes evolves. Characterizing and measuring the surface features and mechanical properties of these devices may allow us to predict where gliosis will occur, and understanding how electrode design features may impact astrogliosis may give researchers a set of

Cochlear implants: Causes, effects and mitigation strategies for the foreign body response and inflammation.

Cochlear implants provide effective auditory rehabilitation for patients with severe to profound sensorineural hearing loss. Recent advances in cochlear implant technology and surgical approaches have enabled a greater number of patients to benefit from this technology, including those with significant residual low frequency acoustic hearing. Nearly all cochleae implanted with a cochlear implant electrode array develop an inflammatory and fibrotic response. This tissue reaction can have deleterious consequences for implant function, residual acoustic hearing, and the development of the next ge

Efficacy and mechanisms of neuromodulation in the treatment of irritable bowel syndrome.

Disorders of gut-brain interaction (DGBI), including irritable bowel syndrome (IBS), have a significant impact on patients, reducing their quality of life and work efficiency. Pharmacological therapy is primarily used as a frontline treatment option for treating IBS. However, owing to the heterogeneous characteristics of IBS and its limited pathophysiological understanding, pharmacological therapy is rather disappointing. Therefore, patients with IBS often use alternative therapies, such as electrical neuromodulation, to treat IBS-related symptoms. Neuromodulation includes invasive and noninva

Individual hemispheric asymmetry in vocal fold control of the squirrel monkey.

In ten squirrel monkeys (Saimiri sciureus), electrodes were implanted into the left and right facial motor cortex at sites producing bilateral vocal fold adduction when electrically stimulated. All animals, in addition, had electrodes in the periaqueductal grey of the midbrain (PAG) at sites producing vocalization when electrically stimulated. In eight out of ten animals, motorcortical stimulation during periaqueductally elicited vocalization caused a change in vocal output. This change, in four cases, was more severe with left-sided ipsilateral motor cortex/PAG stimulation than with right-sid

Speech recognition under conditions of frequency-place compression and expansion.

In normal acoustic hearing the mapping of acoustic frequency information onto the appropriate cochlear place is a natural biological function, but in cochlear implants it is controlled by the speech processor. The cochlear tonotopic range of the implant is determined by the length and insertion depth of the electrode array. Conventional cochlear implant electrode arrays are designed for an insertion of 25 mm inside the round window and the active electrodes occupy 16 mm, which would place the electrodes in a cochlear region corresponding to an acoustic frequency range of 500-6000 Hz. However,

Reducing Behavioral Detection Thresholds per Electrode via Synchronous, Spatially-Dependent Intracortical Microstimulation.

Intracortical microstimulation (ICMS) has shown promise in restoring quality of life to patients suffering from paralysis, specifically when used in the primary somatosensory cortex (S1). However, these benefits can be hampered by long-term degradation of electrode performance due to the brain's foreign body response. Advances in microfabrication techniques have allowed for the development of neuroprostheses with subcellular electrodes, which are characterized by greater versatility and a less detrimental immune response during chronic use. These probes are hypothesized to enable more selectiv

[Incomplete partition type III revisited-long-term results following cochlear implant. German version].

BACKGROUND: Incomplete partition type III (IP III) is defined by a missing lamina cribrosa between the cochlea and the internal auditory canal (IAC). Cochlear implantation (CI) may result in an insertion of the electrode array into the IAC. The aim of this study is to evaluate CI surgery protocols, long-term audiological outcome, mapping and electrophysiological data after CI in IP III patients. MATERIALS AND METHODS: Nine IP III patients were implanted with perimodiolar electrode arrays between 1999 and 2014; eight of them were included in this study. We evaluated mapping data, stapedius ref

A randomized controlled clinical study to quantify the effect of small changes in the design of pacing electrodes on threshold voltages.

A double-blind randomized study of three pacing leads, identical in all ways except for the surface of their electrodes, is presented. The leads were implanted in 30 patients. Ten patients (Group A) received a standard Laserdish lead (dish electrode with laser pores), 10 patients (Group B) received a platinum-iridium coated modified Laserdish electrode, and 10 patients (Group C) received a platinum-iridium coated dish electrode identical to that in Group B except for the absence of laser pores. All leads were implanted by the same operator via the subclavian vein and all patients received an O

Multichannel auditory brainstem implantation: the Australian experience.

The multichannel auditory brainstem implant (ABI) provides the potential for hearing restoration in patients with neurofibromatosis type 2 (NF2). Programmes for auditory brainstem implantation have been established in two Australian centres. Eight patients have been implanted under the protocol of an international multi-centre clinical trial. Three patients had ABI insertion at the time of first side tumour removal, four at second side tumour removal and one after previous bilateral surgery where there was some residual tumour. The translabyrinthine approach was used in all cases. Successful p

Clinical experience with a helical bipolar stimulating lead.

Over 100 patients have been treated for partial epilepsy using a NeuroCybernetic Prosthesis System (NCP). The NCP System is comprised of an implantable pulse generator, an implantable bipolar stimulating lead, and an external communication system. The lead delivers electrical impulses from the NCP Generator to the vagus nerve, and includes a connector end that plugs into the generator, a silicone insulated lead body, and the helical electrode array that attaches to the nerve. The surgical implantation technique has a significant impact on lead reliability and performance. The lead electrode ha

Design and In Vivo Evaluation of an Intraocular Electrode for Ciliary Muscle Biopotential Measurement in a Non-Human Primate Model of Human Accommodation.

The measurement of electrical potentials in the human body is becoming increasingly important in healthcare as a valuable diagnostic parameter. In ophthalmology, while these signals are primarily used to assess retinal function, other applications, such as recording accommodation-related biopotentials from the ciliary muscle, remain poorly understood. Here, we present the development and evaluation of a novel implantable ring electrode for recording biopotentials from the ciliary muscle. Inspired by capsular tension rings, the electrode was fabricated using laser cutting, wiring, and physical

[Transvenous subcutaneous implantation technique of the cardioverter/defibrillator].

For years the high efficacy of implantable cardioverter-defibrillators (ICD) to prevent sudden cardiac death was impaired by the substantial perioperative mortality of the therapy. With the introduction of transvenous-subcutaneous defibrillation leads, thoracotomy could be abandoned and perioperative mortality was reduced to less than 1%. Despite frequent lead complications as dislocations, conductor fractures and isolation failures, long-term efficacy in termination of ventricular tachyarrhythmias remained approximately 98% and prevention of sudden cardiac death was not impaired. All nonthora

A guide to the positioning of brainstem implants using intraoperative electrical auditory brainstem responses.

The number of electrodes that elicit usable auditory sensations with an auditory brainstem implant varies significantly between subjects. For those with only very few, movement of the array by only a few millimetres could make a significant improvement to their outcome, but yet the point at which this is normally discovered is during activation, weeks after the surgery. The number of the electrodes that are able to stimulate the auditory system can be more reliably assured by the use of electrophysiologic guidance in the placement of the implanted electrode array. This chapter describes a proc

[Deep brain stimulation in psychiatric disorders].

Deep brain stimulation is a technique in which electrodes are stereotactically implanted in specific regions of the brain to administer electric pulses. After implantation, the activity of the electrodes can be adjusted. The field of psychiatry shows great interest in deep brain stimulation because there is little risk of complications and because it is reversible and postoperatively adjustable to optimalize the treatment. Deep brain stimulation is being investigated as a potential treatment for obsessive compulsive disorder, Tourette's syndrome, and major depressive disorder. Deep brain stimu

Ultrastructural Analysis of Neuroimplant-Parenchyma Interfaces Uncover Remarkable Neuroregeneration Along-With Barriers That Limit the Implant Electrophysiological Functions.

Despite increasing use of in vivo multielectrode array (MEA) implants for basic research and medical applications, the critical structural interfaces formed between the implants and the brain parenchyma, remain elusive. Prevailing view assumes that formation of multicellular inflammatory encapsulating-scar around the implants [the foreign body response (FBR)] degrades the implant electrophysiological functions. Using gold mushroom shaped microelectrodes (gMμEs) based perforated polyimide MEA platforms (PPMPs) that in contrast to standard probes can be thin sectioned along with the interfacing

Lesion detection in epilepsy surgery: Lessons from a prospective evaluation of a machine learning algorithm.

AIM: To evaluate a lesion detection algorithm designed to detect focal cortical dysplasia (FCD) in children undergoing stereoelectroencephalography (SEEG) as part of their presurgical evaluation for drug-resistant epilepsy. METHOD: This was a prospective, single-arm, interventional study (Idea, Development, Exploration, Assessment, and Long-Term Follow-Up phase 1/2a). After routine SEEG planning, structural magnetic resonance imaging sequences were run through an FCD lesion detection algorithm to identify putative clusters. If the top three clusters were not already sampled, up to three addit

High-density peripheral nerve cuffs restore natural sensation to individuals with lower-limb amputations.

OBJECTIVE: Sensory input in lower-limb amputees is critically important to maintaining balance, preventing falls, negotiating uneven terrain, responding to unexpected perturbations, and developing the confidence required for societal participation and public interactions in unfamiliar environments. Despite noteworthy advances in robotic prostheses for lower-limb amputees, such as microprocessor knees and powered ankles, natural somatosensory feedback from the lost limb has not yet been incorporated in current prosthetic technologies.
