Epidemic typhus is caused by a small strictly intracellular virus named Rickettsia prowazekii, a member of the Rickettsial family. It is transmitted to man by the body louse, Pediculus humanus. Although now rare in Western countries, exanthematic typhus remains common in the Southern hemisphere due to poverty, inadequate clothing hygiene, and poor socioeconomic conditions. In Africa, outbreaks have historically occurred in Burundi, Rwanda, southwest Ouganda, and Ethiopia. The largest outbreak of epidemic typhus since the World War II was reported in Burundi where ongoing civil war since Octobe

In March 1999 an epidemic of Legionella pneumonia developed in the province of Noord Holland, the Netherlands: by the end of April 233 patients had become ill after a visit to a flower show, in 106 of whom the diagnosis of Legionella pneumonia was established, while in 48 the disease was probable and in 4 it was possible; 23 patients died, in 16 of whom the diagnosis of legionnaires' disease was established. In 37 patients Legionella pneumophila was isolated. There was a significant association with stands on the show that displayed anticondense fluids to clean glasses, steam ironing machines

Kaposi's sarcoma (KS) is the most common malignancy associated with human immunodeficiency virus-1 (HIV-1) infection and can result in significant morbidity. The clinical course of KS is quite variable, although for the majority of patients, KS is ultimately a progressive disease requiring systemic therapy. For early indolent KS, local therapies may be appropriate and may provide significant palliation. For patients with more advanced or rapidly progressive disease, systemic therapy is the treatment of choice. Interferon-alfa (Intron A, Roferon-A), with or without antiretroviral agents, is par

In parts of sub-Saharan Africa, nearly 25 percent of the population is HIV-positive as a result of heterosexual transmission of the virus. Could lack of circumcision make men in this region particularly susceptible?

BACKGROUND: Sporotrichosis is a subcutaneous fungal infection. Lymphocutaneous and fixed sporotrichosis are the most common forms; cases of disseminated sporotrichosis are rare. There have been isolated reports and some epidemic familial outbreaks of the infection.
METHODS: We studied four members of two families who contracted sporotrichosis after sleeping in an old and rust-stained camping tent.
RESULTS: All cases presented with polymorphic lesions, three of them with multiple sites of inoculation. The camping tent was shown to be the source of infection.
CONCLUSIONS: We report an epidemi

An outbreak of typhoid fever was observed in a group of students and accompanying teachers during a journey to Haiti in summer 1991. All (6) infected persons were symptomatic and rapidly cured by antibiotics. The Salmonella typhi strains isolated from blood cultures were all identical by ribotyping. All the participants had a medical checkup and stool cultures were obtained; asymptomatic carriers could be excluded. Secondarily infected persons were not observed. The infectious source has not been identified, but various hypotheses are considered. Oral vaccination does not appear to modify the

Epidemic nephropathy, a form of hemorrhagic fever with renal syndrome, caused by the Puumala serotype of hantaviruses and occurring endemically in northern Scandinavia, was studied in 13 children. The clinical symptoms and signs were somewhat different from those reported in adults; none of our patients had hemorrhagic manifestations despite low thrombocyte counts. The most common presenting symptoms were fever, abdominal pain, and renal tenderness with oliguria followed by polyuria. The predominant laboratory findings were proteinuria and/or hematuria and elevated serum creatinine levels. Thr
