Eight patients were seen with idiopathic chondrolysis in nine hips, with from one to eight years' follow-up. The adolescent patients showed characteristic diagnostic patterns--patients in the hip and a limp. The disease occurs most frequently in black girls and in them it tends to be more severe. Threre is progressive loss of articular cartilage, frequently progressing to flexion-adduction contracture and fibrous ankylosis. The pathological process in our patients mainly consisted of chronic synovitis and loss of articular cartilate in the weight-bearing surface of the femoral head. The patien

Intertrochanteric osteotomy gives compensatory correction for the severely slipped upper femoral epiphysis without endangering its blood supply. The results of thirty-five such osteotomies carried out over an eighteen-year period are reviewed. The indication for operation was a chronic slip of a third or more of the growth plate in the lateral radiograph. The mean age at operation was fourteen years and the mean follow-up period seven and a half years. The results showed that even a moderate correction of deformity as shown by the radiograph could produce a hip with a functionally satisfactory

BACKGROUND: Treatment of stable slipped capital femoral epiphysis (SCFE) most commonly involves in situ fixation with a standard cannulated screw, leading to physeal arrest. Recently, Pega Medical (Laval, Canada) introduced the free-gliding (FG) SCFE screw, which employs a growth-friendly, telescopic design. This study examines femoral neck growth and remodeling over the first 2 postoperative years in stable SCFE treated with FG versus standard screws.
METHODS: We retrospectively reviewed 32 hips (19 SCFE, 13 prophylactic) in 16 patients treated with FG screws for stable SCFE. We also reviewe

We report the case of a two and a half-year-old girl who presented with injury to the right thigh along with a discharging sinus in the midthigh region following a nonorthopaedic surgical procedure. Radiographs of the thigh indicated osteomyelitis of the femoral diaphysis with pathological fracture. During follow-up examination, shortening was observed in the affected limb, along with complete separation of the femoral epiphysis with partial resorption of the femoral head. A few cases of physeal separation of the femoral head following osteomyelitis of the femoral diaphysis have been reported

Knowledge of the physiology of the epiphyseal cartilage, respectively epiphyseal plate, is essential for an understanding of defective growth and abnormal modeling of the long bones. The epiphyseal cartilage develops from the embryonal, cartilaginous long bone structure. The histology of the epiphyseal cartilage is characterised by definable zones representing the individual differentiation steps from the reformation of cartilage to chondrolysis. Modeling of the ends of the long bones is also influenced by a transversal and longitudinal direction of growth in the epiphyseal cartilage. The inte

BACKGROUND: There is growing evidence that symptomatic femoroacetabular impingement (FAI) can develop after severe slipped capital femoral epiphysis (SCFE) fixed in situ. Realignment procedures have therefore gained popularity, but complication rates remain controversial. Among them, the subcapital shortening osteotomy without hip dislocation has been progressively adopted in France, but results have never been assessed to date.
METHODS: All cases performed in 23 French university hospitals between January 2010 and March 2014 were reviewed to (1) describe the surgical procedure, (2) assess th

We propose here to focus on preoperative assessment for intertrochanteric femoral osteotomies in severe slipped capital femoral epiphysis (SCFE) using computed tomography. This intertrochanteric osteotomy was preoperatively customized for each chronic SCFE patient treated, and has been performed on 22 patients with an average posterior slip angle of 55 degrees. The osteotomy is planned with images from computed tomography as follows. The angle between a provisional axis on one image and the axis of the lateral aspect of the femur on the other image is defined as alpha. When the alpha angle var

CASE: A 16-year-old adolescent boy with autism and vitamin D deficiency sustained a seizure and had bilateral femoral neck fractures and slipped capital femoral epiphysis (SCFEs). He underwent closed reduction and screw stabilization of femoral neck fractures with incorporation of in situ screw fixation of SCFEs. Intraosseous epiphyseal perfusion monitoring was used to confirm the perfusion of the femoral head. Two years postoperatively, he had healed fractures and no evidence for avascular necrosis.
CONCLUSION: A femoral neck fracture in the setting of a SCFE can be treated with closed reduc

BACKGROUND: Previous data have shown that an equal number of threads on each side of the physis maximizes stability for slipped capital femoral epiphysis (SCFE) fixation. The purpose of the current study was to determine if a fully threaded cancellous screw provides greater stability compared with a partially threaded screw in a porcine model.
METHODS: Twenty skeletally immature porcine femurs were sectioned, and a 30-degree angular wedge was resected from the femoral neck to simulate SCFE. Femora were randomly assigned to partially threaded (16 mm) or fully threaded screw groups (n = 10/grou

From 1978 through 1988, 27 severe slipped epiphyses (Grade III) in 23 adolescent patients were treated with a cuneiform osteotomy of the proximal neck of the femur. Average time of followup was 8 years 5 months. The results were graded by the criteria of Southwick, with emphasis on pain, function, range of motion, and radiographic appearance. Arbitrarily, no hips were rated excellent, because the authors thought that this rating should be reserved for normal hips that have not had any disease process nor surgical procedure. There were 19 good, 4 fair, and 4 poor results. The 4 poor results wer

BACKGROUND: The creation of accurate markers for skeletal maturity has been of significant interest to orthopaedic surgeons. They guide the management of diverse disorders such as adolescent idiopathic scoliosis, leg length discrepancy, cruciate ligament injuries, and slipped capital femoral epiphysis. Multiple systems have been described to predict growth using radiographic skeletal markers; however, no such system has yet been developed for the proximal tibia. The purpose of this study was to establish quantitative radiographic parameters within the proximal tibia that can be used to assess

A rare case of metaphyseal cupping of the distal femur resulting from infantile scury in a child is reported. The radiological and surgical aspects of this case are of particular interest. MRI demonstrated central epiphysiodesis of the right knee, surgery achieved rupture of the epiphyseal plate and resulted in femoral lengthening. The child's limp disappeared as a result.

STUDY DESIGN: Radiographic and histologic evaluation of a rat model of lumbar spine slippage.
OBJECTIVES: To clarify the pathomechanism of slippage in the immature spine.
SUMMARY OF BACKGROUND DATA: There are controversial hypotheses regarding the pathogenesis of slippage of the pediatric spine with pars defects. Some studies supported that disc degeneration was its cause, while others indicated the growth plate injury was the cause.
METHODS: An immature lumbar spine slippage model in 4-week-old rats was used. Following posterior destabilizing surgery, the lumbar spine was radiographically

BACKGROUND: Modified Dunn osteotomy has gained popularity over the past decade in the treatment of moderate to severe adolescent slipped capital femoral epiphysis. The purpose of this study was to retrospectively evaluate a consecutive series of adolescent slipped capital femoral epiphysis patients treated with the modified Dunn procedure at a single institution. We analyze the indications for the procedure as well as the complications after surgical treatment.
METHODS: Forty-three adolescent patients (18 boys and 25 girls) were treated with the modified Dunn procedure at our institution betw

BACKGROUND: Since its initial description, the modified Dunn procedure has emerged as a valuable surgical option for treating slipped capital femoral epiphysis (SCFE), particularly in cases of severe deformity. Previous literature has focused on complications and short-term results following this procedure, but little is known about the morphologic development of the upper femoral epiphysis following successful treatment with the modified Dunn technique, uncomplicated by osteonecrosis.
METHODS: A retrospective study was conducted, including all modified Dunn procedures performed by 7 surgeons

UNLABELLED: Atypical slipped capital femoral epiphyseal (SCFE) is associated with endocrine or metabolic disorders and radiation therapy. In this review, we use case examples of hyperparathyroidism, hypothyroidism, and growth hormone deficiency, as well as renal osteodystrophy, radiation-induced, and valgus SCFE to inform the nuances of these unusual cases. From this, we learn that routine laboratory screening of "typical" patients with SCFE is not cost-effective. Patients with atypical SCFE are often short in stature, underweight, and present either older or younger than the typical age range

The aim of this study was to investigate whether the timing of surgery and surgical technique affect the rate of osteonecrosis in unstable slipped capital femoral epiphysis (SCFE). This is a retrospective review of all unstable slips that were treated at our institution over 8.5 years with a minimum follow-up period of 12 months. Patients with stable slips were excluded from this analysis. Demographic data, time to surgery, and surgical technique were analyzed. Twenty-three unstable slips were included for study after excluding 40 stable slips. There were 17 males and six females, with an aver
