Preeclampsia (PE), a hypertensive disorder of pregnancy, is one of the leading causes of maternal and fetal mortality worldwide. Beyond the immediate pregnancy impact, it is also associated with increased long-term risks in both the mother and the offspring. Currently, no causal therapy exists. Consequently, the complex and heterogeneous nature of PE necessitates the exploration of novel therapeutic targets to improve maternal and fetal outcomes. This review summarizes models of pathogenesis in PE, particularly the two-stage model of the development of PE, where the placenta is suggested to be

The ongoing review conducted by the International Commission on Radiological Protection (ICRP) includes an examination of the ethical foundations of the System of Radiological Protection. ICRP (2018Ann. ICRP47) identified Prudence as one of the four ethical values underpinning the work of the ICRP. In most recent publications, the term regularly appears as a synonym or in conjunction with the Precautionary Principle. However, despite their frequent pairing, the two concepts have distinct meanings and unique histories in ICRP publications. Given the importance of the Precautionary Principle out

This narrative review evaluates innovations in assisted reproductive technology (ART) for female infertility, focusing on efficacy, safety, and long-term outcomes. The key objectives include synthesizing advancements such as artificial intelligence (AI)-driven embryo selection, preimplantation genetic testing (PGT), vitrification, and time-lapse imaging, along with their clinical applications. Evidence indicates that PGT can improve implantation and live birth rates per transfer in selected patient groups, that is, those with recurrent pregnancy loss or advanced maternal age. Evidence of AI-as

Organ bioprinting is a rapidly evolving field designed to address the persistent shortage of donor organs by engineering patient-specific tissues that replicate the function and structure of natural organs. Despite significant technological advancements, bioprinting still faces major obstacles, including tissue rejection, inadequate vascularization, limited physiological functionality, and various ethical and translational challenges. In this review, we assess current bioprinting modalities, particularly extrusion-based printing, inkjet printing, laser-assisted bioprinting (LAB), and stereolit

PURPOSE OF REVIEW: Heart failure is a final common pathway for many patients with adult congenital heart disease (ACHD). As such, ACHD heart transplantation is increasingly being considered and performed. Ethical considerations exist at each step in the transplantation process.
RECENT FINDINGS: ACHD patients face many challenges to successful heart transplantation. A dearth of ACHD cardiologists and many barriers to transplant consideration highlight inequities related to recognition of end-stage disease and timely referral for advanced therapies. Certain vulnerable ACHD sub-groups may more e

Heart transplantation (HTx) is a life-saving procedure for patients with end-stage heart failure and has undergone remarkable advancements since the first successful transplant in 1967. The introduction of cyclosporine in the 1970s significantly improved patient outcomes, leading to a global increase in transplants, including in India, where the practice has grown despite initial challenges. This review provides an extensive overview of HTx, focusing on current practices, technological advancements, and the ongoing challenges the field faces today. It explores the evolution of surgical techniq

INTRODUCTION: Systemic sclerosis (SSc) is a progressive autoimmune rheumatic disease with high morbidity and mortality and few effective treatment options. Increased biopharmaceutical investment in therapeutic development for rare diseases has created new opportunities for drug discovery in SSc. However, despite the increased pipeline activity in SSc, success rates remain dismally low.
AREAS COVERED: This review describes the current state of therapeutic development in SSc with an in-depth coverage of cohort enrichment strategies, as well as a discussion of relevant ethical and feasibility co

BACKGROUND: Whereas legislation for body mass index (BMI) surveillance and screening programs has passed in 25 states, the programs are often subject to ethical debates about confidentiality and privacy, school-to-parent communication, and safety and self-esteem issues for students. Despite this debate, no comprehensive analysis has been completed that compares and contrasts how these issues differentially affect schools, parents, and students.
METHODS: A keyword search from electronic databases and a review of state legislation related to BMI surveillance and screening were used to identify

BACKGROUND: In paediatric palliative care (PPC) research, meaningful engagement of children and young people (CYP) remains underdeveloped. Despite widespread endorsement of patient and public involvement (PPI) for its ability to enhance research relevance, ethics, and overall impact, current studies provide limited insight into how CYP are involved - specifically, the types, levels (e.g., consultation, collaboration, child-led), and stages (from priority setting to dissemination). This scoping review aims to systematically map existing literature on the involvement of CYP (aged 4-24 years) as

Innovative research in childhood rheumatic diseases mandates international collaborations. However, researchers struggle with significant regulatory heterogeneity; an enabling European Union (EU)-wide framework is missing. The aims of the study were to systematically review the evidence for best practice and to establish recommendations for collaborative research. The Paediatric Rheumatology European Single Hub and Access point for paediatric Rheumatology in Europe (SHARE) project enabled a scoping review and expert discussion, which then informed the systematic literature review. TOPIC IMPORTANCE: Extracorporeal membrane oxygenation (ECMO) is a relatively novel, intensive medical intervention. It is known to raise important yet unsettled ethical concerns.
OVERVIEW: We established the ECMO Ethics Workgroup in 2021 to improve our understanding of these ethical issues and their possible resolutions. This paper explains the methods used to produce our insights and recommendations, which additional papers from the project elaborate on in detail. Initially, over 6 months, the 37 group members described the full range of ethical issues in ECMO care they were familiar with fr

Anaesthesia waste gases (AWGs) represent a significant yet often overlooked contributor to healthcare's environmental footprint, with implications extending beyond ecological concerns to encompass occupational hazards and complex ethical considerations. This article examines the intersection of AWGs and bioethics through the framework of environmental stewardship, occupational health principles, patient-centered care, and distributive justice. Contemporary volatile anaesthetics possess global warming potentials thousands of times greater than carbon dioxide, while implicated occupational expos

Recent research aimed at the discovery, integration, and communication of health outcome measures (or "biomarkers") in Alzheimer's disease has raised challenging questions related to whether, how and when results from these investigations should be disclosed to research participants. Reflecting the apparent heterogeneity of many neurodegenerative diseases, biomarker or other risk factor results are often probabilistic, interactive, multi-modal, and selective. Such characteristics make it very complex to summarize and communicate to clinicians, researchers, and research participants. Whereas th

Recently, a technique of assisted reproduction was developed to allow lesbian women to share biological motherhood of their offspring - the ROPA method (in Spanish - Recepción de Ovocitos de Pareja; in English - Reception of Partner's Oocytes), also known as lesbian shared in vitro fertilization. One mother provides the oocytes (genetic mother) and the other receives the embryo and gets pregnant (gestational mother). As for most issues related to medically assisted reproduction, this technique raises a lot of ethical questions in respect to patients, future offspring, gametes, and embryos. Fur

The history of human subjects research and the abuses that led to the creation of the United States' (US) regulatory schema for research, which includes institutional review boards (IRBs), is long and complex. However, when one understands the history, one can see direct links between those abuses and the regulations, as well as the ethical principles, contained in the Belmont Report. To that end, we provide a brief analysis of the history of human subjects research and its abuses, including a lesser known case (the atrocities committed by Nazi scientists and physicians during World War II) an

Patients with both chronic pain and substance use disorders are increasingly encountered in a variety of treatment settings. The treatment of these patients raises a number of ethical and patient care issues. Consultation-liaison psychiatrists possess the knowledge and skills to constructively address these issues. This chapter provides clinicians with a review of clinical and ethical dilemmas related to opioid treatment of chronic pain in patients with substance use disorders. The core conflict of beneficence and nonmaleficence will be explored in relation to the concepts of autonomy, justice
