External fixation with Wagner's apparatus can, because of its mechanical properties, be employed for temporary immobilization of the knee joint. This method is especially suitable for patients for whom immobilization in plaster is problematic or disadvantageous. It can be applied to the following categories of patients: Patients with injuries to the bones or ligaments of the knee which are associated with wounds or injuries of the blood vessels or nerves; Patients with multiple injuries of the leg (e.g., fractures of the tibia and femur) and concomitant intra-articular knee injuries; Patients

The mechanical principle of function of the Harrington instruments or of the spinal stabilization by means of plates does not meet the anatomical and mechanical conditions in instable fractures of the thoracic and lumbar spine sufficiently. The requested qualities in regard to fixation of the material, shortness of fusion distance, possibility of reduction and variability according to individual situations can be met only by the fixateur externe. Magerl was able to verify the correctness of this thesis by means of the fixateur externe constructed by himself. It was logic for us to practice the

Fractures of the femoral shaft are often treated by an established method of internal fixation, such as a medullary nail or a plate and screws, to obviate the many disadvantages of traction and prolonged rest in bed. Fractures of the femoral shaft which are severely comminuted and open are usually unsuitable for internal fixation. Between July 1981 and March 1983 we treated seven patients with severely comminuted fractures of the femoral shaft (of which three were open), using the ASIF tubular external fixator system or the Wagner apparatus. The technique of application of the external fixator

In the treatment of multi-fragment fractures of the base of the radius plate osteosynthesis using the small-fragment instrument set is preferred to conservative treatment due to the superior results achieved. However, one still cannot dispense with an additional immobilization of the fracture in a plaster cast. The extension of intra-articular comminuted fractures of the base of the radius is a technically relatively simple procedure and does not require a supporting plaster cast. In this paper we report the results of the treatment of twenty-seven wrist-fracture patients who were treated by e

The scientific background for limb replantation began 75 yr ago with the work of Hopfner and of Carrel and Guthrie. Thirty-five years ago, Hall indicated that the techniques were available, but it was not until 17 yr ago that Malt and McKhann performed the first successful replantation of a limb in man. Since then, the techniques have become more sophisticated and guidelines have been formulated. Many successful replantations have been reported. We present two children with partial amputation of a lower limb. Both had severe soft tissue and bone damage sufficient to contraindicate replantation

Background: Neglected dorsal subluxation of the proximal interphalangeal joint (PIPJ) results in significant compromise of hand function. Operations like volar plate arthroplasty, hemihamate arthroplasty and joint fusions have been described to treat them. However, all are open procedures and technically demanding. Dynamic external fixation has been used as a treatment for acute injuries. There is a paucity of literature on the use of static external fixation in the treatment of neglected PIPJ fracture subluxations. We describe the outcomes of the use of static external fixation followed by sp

INTRODUCTION: Tibial bone transport using external fixation is widely employed for the reconstruction of large and complex tibial bone defects; however, indications and surgical strategies remain highly heterogeneous. Tibial bone transport using external fixation is a demanding but reliable reconstructive technique, characterized by low failure rates and generally favorable outcomes despite frequent complications. Adequate soft-tissue management, selective docking-site grafting, and early functional rehabilitation were commonly reported. Further prospective studies are needed to standardize tr

Arthrofibrosis (AF) is a common pathological condition characterized by joint dysfunction. However, traditional non-invasive external fixation methods are difficult to maintain on the small limbs of rats. This study aimed to develop a novel, non-invasive, and stable rat model of knee arthrofibrosis using a thermoplastic polymer resin. Sixty male Sprague-Dawley rats were randomly assigned into a Sham group and immobilization groups (1, 2, 4, and 6 weeks). Utilizing the material's property of being malleable at high temperatures and rigid at room temperature, a custom-fitted "thigh-crus-trunk" e

BACKGROUND: Webplasty can be conducted for complex syndactyly caused by Apert syndrome (also referred to as Apert hand) by extending the soft tissue in the lateral direction using an external fixator. This study aimed to verify the usefulness of webplasty without skin grafting.
METHODS: Webplasty with lateral extension was conducted at a single institution from 2015 to 2023. The patients were four children with Apert hand aged 1-3 years. A custom-made small external fixator was used for all of the soft tissue extension.
RESULTS: Webplasty without skin grafting was completed by the time all f

Pin tract infections are virtually universal complications with the use of external fixation. While most are superficial and respond to oral antibiotics and local care, septic loosening may occur at the bone-pin interface, which may lead to instability of the fixator, catastrophic failure, fracture, and long-term osteomyelitis. Classification systems and prevention protocols have been developed to address this ubiquitous complication. Treatment of severe pin tract infections often requires debridement, parenteral antibiotics, and removal of the offending pin or the entire external fixator. In

BACKGROUND: Symptomatic patients with severe brachymetatarsia are commonly treated with callus distraction using external mini-fixator (EF) or internal device (ID). This study points out advantages and limitations of both methods comparing clinical and radiographical parameters.
METHODS: Retrospective analysis of 21 metatarsal bones in twelve patients. Twelve metatarsals were treated with ID (Genos Mini), nine with EF (MiniRail).
RESULTS: Mean lengthening distance was 17.3 mm using EF and 11.7 mm using ID (p = 0.016). Adverse results were observed in 89% of metatarsals treated with EF and in

INTRODUCTION: Among all the phalanges, most fractured is the proximal phalanx. Frequently encountered complications are malunion, stiffness, and soft-tissue injury which invariably increase the disability. The aim of fracture reduction, therefore, comprises acceptable alignment and the gliding of the flexor and extensor tendons are maintained. Factors affecting management are fracture location, type of fracture, soft-tissue injury, and fracture stability.

BACKGROUND: Unstable pelvic fractures in children are serious and complex injuries, and the optimal method to manage these injuries is controversial.
HYPOTHESIS: We hypothesized that an external fixator would be a satisfactory method of treating unstable pelvic fractures in children.
PATIENTS AND METHODS: We retrospectively reviewed 40 pediatric patients with unstable pelvic fractures who were treated with an external fixator in Beijing Children's Hospital from June 2006 to June 2016. Postoperative follow-up was 12 to 108 months, with an average of 26 months. One year after the operation, fr

INTRODUCTION: Traumatic intercalary defects of the tibia may be effectively managed with the free fibula flap. However, any alteration of limb alignment with residual bony angular deformity of the tibia must be also addressed. We describe the use of the free fibula flap in conjunction with external fixation to allow residual deformity correction and patient mobilization ambulation during healing of the free flap.
METHODS: Retrospective medical record review was conducted of patients with segmental tibial defects greater than 7 cm who underwent reconstruction with fibula free flap and simple p
