Nona-D-arginine amide for prophylaxis and treatment of experimental Pseudomonas aeruginosa keratitis.

PURPOSE: Nona-D-arginine amide (D9R) affected a cure of Pseudomonas aeruginosa corneal disease when combined with ciprofloxacin. In this study, we show that D9R alone and prophylactic treatment with D9R significantly reduced corneal pathology and bacterial burden associated with P. aeruginosa infection. MATERIALS AND METHODS: Right eyes of Swiss Black mice received a 5 microl drop of either phosphate buffered saline (PBS, pH 7.4) or100 microM D9R every half-hour or hourly for 5 hr (total of 6-12 drops/eye) immediately after corneal wounding and subsequent infection with 1 x 10(6) colony formi

Culture-negative Capnocytophaga canimorsus meningitis diagnosed by 16s ribosomal RNA polymerase chain reaction in an immunocompetent veterinarian and a review of the literature.

Capnocytophaga canimorsus is a fastidious, capnophilic and facultative anaerobic Gram-negative rod found commonly in the oral flora of dogs that may cause zoonotic infections such as cellulitis and eye infections. In immunocompromised patients, it may cause fulminant sepsis. Meningitis due to C. canimorsus is, however, a rare manifestation. This is the first reported case of C. canimorsus meningitis in Australia in an immunocompetent veterinarian diagnosed by 16s ribosomal RNA polymerase chain reaction.

Opportunistic ocular infections in the setting of HIV.

PURPOSE OF REVIEW: The aim of this review is to highlight recent changes in opportunistic ocular infections (OOIs) in the era of modern combination antiretroviral therapy (cART), in the setting of HIV-infected patients. RECENT FINDINGS: Improvements in modern cART has led to a progressive decline in the incidence of OOIs and mortality among patients with AIDS. Not only has there been a decreasing incidence of cytomegalovirus (CMV) retinitis, but there also has been a decline in progression of such retinitis when it does occur in AIDS patients, since the introduction of cART. Nevertheless, CMV

Endophthalmitis caused by Hathewaya histolytica: A diagnostic dilemma.

A 76-year-old male presented with pain, redness, and decreased vision in the left eye for 5 days. The patient had undergone combined cataract extraction and vitrectomy for a macular hole 2 weeks back. The vision had reduced to the perception of light in the left eye. Clinical examination revealed lid edema, conjunctival congestion, hazy cornea, and exudative membrane in the pupillary axis with no view of the fundus. Ultrasound examination of the left eye showed plenty of moderate reflective dot echoes. The patient was diagnosed with postoperative endophthalmitis and underwent vitrectomy with i

Diabetic keratopathy.

Diabetic keratopathy is a significant problem that is commonly overlooked and underdiagnosed by many healthcare providers. More than half of the diabetic population can have tear film changes, corneal epithelial abnormalities, neurotrophic keratopathy, and increased postoperative surgical complications. Clinical manifestations can be quite variable and thus diabetic patients with ocular complaints should be referred to an eye care professional. Treatment of diabetic keratopathy can be managed with a stepwise approach that includes increasing corneal surface lubrication, preventing infections o

[Sympathetic ophthalmia versus Harada disease].

An algerian woman presents with a panuveitis of the right eye. Main features of this uveitis are exudative retinal detachments. That happened several months after a corneal ulcer of unknown aetiology on the left eye. Immunologic, neurologic and infections workup associated with previous ocular injury points to the diagnosis of sympathetic ophthalmia. However, the diagnosis of Harada disease can also be considered in this case because of similarities regarding the clinical and the histologic aspects which are exposed. If corticosteroids are the drug of choice, enucleation is still indicated for

[Action of gentamycin against a background of magnetotherapy of the anterior chamber in a traumatic infected erosion of the cornea].

Fifteen patients with surface infections of the cornea due to a foreign body in one eye were treated with magnetophoresis and gentamicin before and after removal of the foreign bodies. The lower eyelid was turned back and a cotton tampon impregnated with 1 ml of a 0.3 per cent aqueous solution of gentamicin was applied. After that the tube of the alternating magneto was placed before the eyelids. The magnetic field (50 oersted, 50 Hz) was applied for 9 minutes. Such a treatment promoted suppression of the eye inflammatory reaction and accelerated corneal epithelization.

Antibiotic prophylaxis in penetrating injuries of the eye.

In a specially developed experimental model of an infected penetrating eye injury the importance of time and mode of antibiotic administration in the prophylaxis of post-traumatic eye infection was evaluated. It was concluded that the early administration of an antibiotic to which infecting organisms are sensitive can prevent the development of an inflammatory reaction in the majority of injured eyes. The late administration of the same antibiotic (6 h after injury) has no effect at all. Subconjunctival injection was found to be superior to intramuscular if the antibiotic was given at a simila

Infectious uveitis: Epidemiology, etiology, diagnostic test performance and treatment.

Infectious uveitis is an inflammation of the uveal tract, corresponding to the middle layer of the eye, triggered by a response against an infectious agent. This condition is classified into anterior, intermediate, posterior and panuveitis. The most frequent infectious agents in anterior uveitis are viruses, mainly Herpes simplex virus, although it can also be caused by other pathogens such as Bartonella spp., Mycobacterium tuberculosis and Fusarium spp. In intermediate uveitis, syphilis plays a predominant role, while in posterior uveitis various microorganisms are involved, among them Toxopl

Emerging antimicrobial resistance and need for antimicrobial stewardship for ocular infections in India: A narrative review.

Patients with ocular infections are at increased risk of vision impairment and may require immediate medical care to preserve their vision. Management of ocular bacterial infections has evolved in recent years and includes a pragmatic selection of broad-spectrum antibiotics based on the causative bacteria. Nevertheless, the treatment of bacterial ocular infections is increasingly becoming a challenge, as the causative bacterium acquires resistance to antibiotics through intrinsic and acquired methods. From an Indian perspective, along with the challenges of antibiotic resistance, there are oth

[Emergencies in human immunodeficiency virus infection].

Emergencies during HIV infection consist mainly in opportunistic infections. They are varied and the physician should keep an "internist's eye" on the condition; they also require emergency complementary examinations for confirmation of diagnosis. If such diagnostic tests cannot be rapidly made, treatment should be started on the basis of presumption and frequency. In France, the most frequently observed opportunistic infections are: pneumocystosis, cerebral toxoplasmosis and candidiasis. Tuberculosis is a major concern of those responsible for public health care. Surgical complications follow

Chlamydia trachomatis conjunctivitis in a male teenager: a case report.

An 18 year old man was seen at a Sexually Transmitted Infections (STIs) clinic for counselling and treatment of Chlamydia trachomatis genital infection which had been diagnosed during a screening survey of high school students. For two months he had reported conjunctival hyperaemia, increased tearing, itching, and mucopurulent secretions, predominantly on the left eye. His ophthalmologist had made a diagnosis of follicular conjunctivitis and lower superficial punctate keratitis (left eye more than right eye), irresponsive to topical treatment. Chlamydial conjunctivitis was suspected and confir
