Predicting postoperative facial nerve paralysis by using intraoperative nerve monitoring during parotid surgery.

OBJECTIVES: To investigate a method for predicting postoperative facial nerve paralysis (POFNP) during parotid surgery using intraoperative nerve monitoring (IONM). METHODS: We assessed prediction for POFNP by using IONM, comparing between stimulation in the facial nerve trunk and each branch by using facial nerve monitoring. The amplitude response ratio (ARR) was calculated for the trunk/periphery. In addition, we then examined the correlation between ARR and time to recovery of paralyzed branches. RESULTS: 372 branches of 93 patients did not develop POFNP and were classified as group A. Am

Residual Epiphora After Successful Periocular Surgery for Facial Paralysis: Pathophysiology and Management.

The prevalence of residual epiphora following successful periocular surgery for facial nerve paralysis can be as high as 30% or more. The pathophysiology of residual epiphora is complex, but identification of the etiology is paramount because the therapeutic approach varies accordingly. Treatments range from medical management of systemic disease to botulinum toxin injections for conditions that arise from aberrant reinnervation to surgical procedures that bypass the lacrimal drainage system completely. We describe a case report and review the pathophysiology and management of residual epiphor

Total facial rehabilitation: the evolving concept of reconstructive surgery.

Total facial rehabilitation requires an understanding of the contributing components of the so-called aging face syndrome. This syndrome may be due to atrophy of the jaws or may result from ablative surgery for cancer. The aging face requires correction of every anatomic component (i.e., facial skeleton, dentoalveolar processes, and soft tissue). The surgical armamentarium includes maxillofacial osteotomies, autogenous bone grafts, biomaterials, internal rigid fixation, and other ancillary procedures. The introduction of osseointegrated implants has allowed the surgeon to use a stable base on

Facial nerve neurorrhaphy due to unexpected facial nerve injury during parotid gland tumor surgery.

BACKGROUND: Unexpected facial nerve damage can occur during parotid gland tumor surgery. We sought to determine the incidence and treatment outcomes of unexpected facial nerve injuries in patients with parotid gland tumor surgery. METHODS: We retrospectively enrolled in this study five patients, who underwent facial nerve neurorrhaphy due to unexpected facial nerve injury during parotid gland tumor surgery January 2012-August 2019. RESULTS: There were five patients (0.008%) with unexpected facial nerve injuries during the parotid gland tumor surgery of 577 patients in our hospital for approx

Morphologic grading and treatment of facial actinic keratosis.

Actinic keratoses (AKs) represent the earliest stage in the development of squamous cell carcinoma (SCC) and represent important biomarkers for individuals at risk for development of invasive SCC. Based on clinical morphology, AK can be subdivided into three different grades, which correspond to specific dermatoscopic, reflectance confocal microscopic, and histopathologic substrates. Given the risk for potential progression toward invasive SCC, AK should be treated at the earliest stage. A wide range of minimal destructive or topical therapies is available for the treatment of AK. The choice o

Intracranial schwannoma as atypical facial pain. Case report.

Facial pain is a common complaint that leads those who have it to seek professional help. Often times, the general dentist is the first clinician that a patient consults because of a presumed odontogenic origin of the pain. Occasionally a small number of these patients will be found to have an intracranial tumor. The case reported here is one such patient who was diagnosed and treated for a seventh nerve schwannoma.

Facial nerve and parotid surgery.

A series of 100 surgical procedures on the parotid gland affected by tumours is reported, with a detailed analysis of facial nerve function after parotid surgery. A low percentage of temporary lesions (28%) affecting only one branch of the facial nerve, all of which regressed in a short time, and only one case of a permanent lesion affecting the whole nerve, anatomically intact, however, was observed. The authors confirm the safety of parotidectomy, if correctly performed.

Facial nerve neurinoma: two cases located in the horizontal portion.

Two small facial neurinomas located in the horizontal portion were presented to demonstrate the clinical features, diagnostic evaluation, and surgical treatment. These two cases show the early state of facial neurinoma and suggest the mechanism of facial paralysis by facial neurinoma. Facial nerve decompression is recommended for Bell's palsy unresponsive to the conservative therapy and recurrent facial palsy.

Clinical experience with the diagnosis and treatment of middle ear adenomatous neuroendocrine tumors.

Middle ear adenoma, also known as middle ear adenomatous neuroendocrine tumors (MEANTs), is a rare neoplasm, accounting for < 2% of all middle ear tumors. It is characterized by slow growth, non-invasive behavior, and nonspecific symptoms, which often complicate preoperative diagnosis. This review summarizes our clinical experience in the diagnosis and treatment of 11 cases of MEANT. In this cohort, four cases were preoperatively diagnosed as cholesterol granuloma, five as cholesteatoma, and two as middle ear tumors. The median age at diagnosis was 36.7 years, with all patients (100%) experien

Treatment and Diagnostic Challenges in a Patient with Atypical SARS-CoV-2-Associated Encephalitis Mimicking a Neoplasm: A Case Report.

Background and Clinical Significance: Encephalitis is a rare neurological complication associated with Severe Acute Respiratory Syndrome Coronavirus 2 (SARS-CoV-2) infection. In rare cases, focal neuroinflammation can manifest as a mass-like parenchymal lesion, creating profound diagnostic and treatment dilemmas by mimicking primary central nervous system neoplasms. Case Presentation: A 34-year-old female presented with cephalalgia, nausea, confusion, facial palsy, and a new onset of focal impaired awareness seizures (FIAS). Brain magnetic resonance imaging (MRI) revealed a prominent hyperinte

Odontogenic carcinosarcoma of the mandible: A case report.

KEY CLINICAL MESSAGE: Odontogenic carcinosarcoma, a rare and challenging diagnosis, was identified in a 60-year-old male through histopathology, revealing a biphasic neoplasm with malignant epithelial and mesenchymal components. Surgical resection is crucial for management, highlighting the importance of vigilant postoperative follow-up to ensure early detection of any recurrence. ABSTRACT: One rare mixed malignant odontogenic tumor is odontogenic carcinosarcoma, which comprises malignant epithelial and mesenchymal components. Diagnosing odontogenic carcinosarcoma is challenging due to its ra

Nasopharyngeal carcinoma (NPC) in the ophthalmic practice: A serious neoplasm presenting initially to ophthalmologists.

PURPOSE: Nasopharyngeal carcinoma (NPC) is an aggressive malignant tumor that arises from the nasopharyngeal epithelial lining. Most patients with NPC present with a neck mass or cranial nerve palsy. It is infrequent for patients with NPC to present to an ophthalmologist initially with ophthalmic complaints and absence of prior diagnosis of NPC. We are reporting a series of six NPC cases that presented solely with ophthalmic complaints, to attract the attention of ophthalmologist to such a serious neoplasm. METHODS: A retrospective observational study of all consecutive patients diagnosed wit

Epithelial-myoepithelial carcinoma of the parotid gland: Case report.

INTRODUCTION AND IMPORTANCE: Malignant salivary gland neoplasms present a significant diagnostic challenge, requiring the integration of clinical, radiological, and pathological assessments. Epithelial-myoepithelial carcinoma (EMC) is a rare malignant neoplasm that exhibits a biphasic histopathological pattern. Although the major salivary glands are most commonly affected, EMC can also involve minor salivary glands and, in rare instances, the seromucous glands of the upper aerodigestive tract. PRESENTATION OF CASE: A 47-year-old male presented with a right-sided parotid mass. On physical exam
