Endovascular treatment for posterior inferior cerebellar artery aneurysm with vertebral artery stenosis: The critical role of diagnosing osteophyte compression.

BACKGROUND: Vertebral artery (VA) stenosis can be caused by several factors, including arteriosclerosis, arterial dissection, and mechanical compression. Symptomatic vertebrobasilar insufficiency caused by VA stenosis due to mechanical compression associated with head rotation is well-known as Bow Hunter's syndrome. However, an accurate diagnosis of asymptomatic osteophyte compression-induced nonrotational VA stenosis is difficult. We report a case of left posterior inferior cerebellar artery aneurysm with severe left VA stenosis, treated with stent-assisted coil embolization following appropr

Practical Recommendations for Anticoagulation in Patients With Atrial Fibrillation.

BACKGROUND: The lifetime risk of stroke among non-anticoagulated patients with atrial fibrillation (AF) approximates 1 in 3. Oral anticoagulation (OAC) reduces stroke and systemic embolism by at least two thirds and mortality by one fourth. METHODS: The present narrative review summarizes current evidence and practical recommendations on OAC in AF and highlights recent advances and remaining gaps in knowledge. RESULTS: The threshold for net clinical benefit of OAC is met when stroke risk exceeds 1%-2% per year, typically corresponding to ≥ 1 traditional stroke risk factor (CHA2DS2-VA ≥ 1), w

Differences in prostate size between patients from University and Veterans Affairs Medical Center populations.

BACKGROUND: Numerous studies on prostatic disease have been performed at Veterans Affairs (VA) Medical Centers. Recent investigations evaluating early detection of prostate cancer provide insight that the average prostate volume may be different between patients with similar clinical findings who are from different hospital settings. The objective of this study was to compare prostate size between men from University and VA Medical Centers. METHODS: Patients were enrolled retrospectively from 1989-1996 from the Urology Clinics at a University and a VA Medical Center. All men underwent transre

Associations of wall shear stress and nitric oxide distribution with vertebral artery tortuosity: A morphological and hemodynamic study.

The vertebral artery (VA) exhibits substantial morphological variability and is prone to tortuosity and atherosclerosis at the V1 segment . This study aimed to systematically analyze sex-, laterality-, and age-related differences in the geometry and hemodynamics of the VA V1 segment, and to explore the potential role of hemodynamic factors in VA tortuosity and remodeling. Computed tomography angiography data from 53 subjects were used to reconstruct 3D models of the VA V1 segment. Geometric parameters of the subclavian artery and VA were quantified across sex, laterality, and age groups. Compu

Living alone and outpatient care use by older veterans.

In nonveteran older adults, living alone influences outpatient care use, but its importance in the veteran population has not been well studied. The aims of this study are to describe the use of outpatient care by older veterans who live alone versus those who live with others and determine whether living alone influences outpatient use by older veterans. The data come from the 2001 Veteran Identity Program Survey designed to measure Department of Veterans Affairs (VA) and non-VA outpatient care use. Univariate and bivariate analyses were conducted to examine distributional properties, associa

Elevated B-Type Natriuretic Peptide Level as a Residual Risk Factor for Ventricular Arrhythmias Among Patients Undergoing Cardiac Resynchronization Therapy With Improved Left Ventricular Ejection Fraction.

BACKGROUND: Patients who achieve improved left ventricular ejection fraction (LVEF >35%) with cardiac resynchronization therapy (CRT) are at a lower risk of ventricular arrhythmia (VA). Little is known about the significance of the B-type natriuretic peptide (BNP) level for the risk of VA. This study investigated the risk factors for VA in CRT and the risk stratification of VA with BNP in CRT with improved LVEF. METHODS AND RESULTS: This study evaluated 352 CRT patients from 2012 to 2020. Patients were categorized into 2 groups: improved LVEF (impEF; LVEF >35%), and low LVEF (lowEF; LVEF ≤35%
