Direct oral anticoagulants such as apixaban or dabigatran have revolutionized anticoagulant treatment. Apixaban is a factor Xa inhibitor which is a non-vitamin K dependent oral anticoagulant known tocause the lowest rate of intracranial bleeding among the same kind of inibitors. In this paper, we report a rare case in a 60-year-old man with a history of hypertension and oligodendroglioma on apixaban for deep venous thrombosis who presented to our hospital with decreased level of consciousness and slurred speech with rapid deterioration. We highlight the risk of subdural bleeding requiring immediate neurosurgical intervention due to apixaban, with literature review.

A 57-year-old man with antithrombin III deficiency with deep vein thrombosis and pulmonary thromboembolism after surgery for colon cancer was managed with warfarin and inferior vena cava filtration. A 72-year-old man with non-valvular atrial fibrillation and metastatic liver and lung cancer after surgery for colon cancer developed thrombosis in the right atrium one month after decreasing the dose of warfarin due to the introduction of double anti-platelet therapy for coronary stent implantation. Restoring the warfarin dose with ordinary control for two months did not result in any changes in the size of the thrombus; however, the subsequent substitution of rivaroxaban (oral treatment with a direct Factor Xa inhibitor) for warfarin ultimately resolved the thrombosis.

Bleeding is the main complication of oral anticoagulants, anti-vitamin K or new drugs such as anti-factor Xa or anti-thrombin agents. Prevention and treatment of the venous thromboembolic phenomena should be modified in a near future. The arrival of two new classes of anticoagulants: antifactor Xa and anti-factor IIa, opens a new perspective in an area in which low molecular weight heparins and oral anticoagulants have the exclusivity. Fondaparinux has the approval for its introduction into the market as a maximum representative of this group and has begun to be used. Among the direct antithrombotics we find melagatran and its oral form, ximelagatran, that opts to be the substitute of oral anticoagulants.

In trauma, even therapeutic apixaban levels can significantly exacerbate bleeding. In cases of overdose, the risk of prolonged, life-threatening hemorrhage increases substantially, necessitating targeted interventions, such as pharmacological reversal agents. This prospective observational study reports on 126 women from 25 UK centres with image-proven antenatal venous thromboembolism (VTE), 62% deep vein thrombosis and 38% pulmonary embolism.
