Subperiosteal hemangioendothelioma of the femur.

Primary neoplastic vascular lesions of bone are rare, and include haemangiomas, haemangioendothelioma, epithelioid haemangioendothelioma and angiosarcoma. These lesions may be multicentric, and when they involve bone are typically intraosseous and lytic. Radiological findings are not always specific. We report a case of haemangioendothelioma localised to the subperiosteum of the femur, a site not previously described for this lesion. The nomenclature for vascular neoplasms in bone is discussed.

Fluorosis-induced hyperparathyroidism mimicking a giant-cell tumour of the femur.

We report the case of a young woman who, over a period of five years was diagnosed and treated for a giant-cell tumour of bone, osteomalacia and fluorosis. A review of the literature revealed a correlation between these three diagnoses, the primary pathology being fluorosis and the remaining symptoms being secondary manifestations. It is important to be aware of this association, especially in regions with endemic skeletal fluorosis.

Management of gynecologic oncology patients with a preoperative deep vein thrombosis.

Our experience with gynecologic oncology patients presenting preoperatively with a deep vein femoral thrombosis is reported. Over a 3-year period data were collected on all patients at the University of South Florida (USF) requiring surgery for a known or suspected gynecologic cancer and having a concomitant active femoral venous thrombosis. Twelve such patients were managed. Management was divided among three options: heparinization, preoperative inferior vena cava (IVC) filter, and intraoperative IVC ligation. For two patients a filter could not be placed preoperatively due to tumor compress

Multiple metaphyseal involvement of a thymic lymphoma associated with hypercalcemia in a puppy.

A six-month-old, female German shepherd dog was presented because of depression, anorexia, vomiting, polyuria, and polydipsia of approximately 10 days' duration. The puppy was depressed, and pain could be elicited on palpation of both shoulders and hips. The most significant results of serum chemistries and hematology were hypercalcemia; increased blood urea nitrogen, creatinine, and alkaline phosphatase; and leukocytosis with neutrophilia. Thoracic radiographs revealed a large thymic mass, diagnosed on histological examination as a thymic lymphoma. Radiographs of the shoulders revealed destru

Unifocal Langerhans' cell histiocytosis (eosinophilic granuloma) resembling Ewing's sarcoma.

In a 9 year old boy, a destructive lesion in the diaphysis of the right femur was wrongly diagnosed as a Ewing's sarcoma on the basis of the radiologic findings and fine needle aspiration cytology report. The clinical and radiologic picture was suggestive of Ewing's sarcoma, but an open biopsy of the lesion revealed a histopathological pathological picture of eosinophilic granuloma. A brief review of the literature is given, together with discussion on the differential diagnosis of a mid shaft femoral lesion in young patients.

Mesenchymal chondrosarcoma arising in fibrous dysplasia of the femur.

The occurrence of mesenchymal chondrosarcoma in an area of fibrous dysplasia in the upper femur of a 28 year old man is reported. It is believed that this is the first documented example of such an association. A further unusual feature is the presence of benign giant cells in the mesenchymal chondrosarcomatous component.

[Osteoid osteoma of the femoral neck in children: diagnostic problems].

The authors report a case of an osteoid osteoma of the femoral neck in a child, aged 3 1/2. This is a benign bone tumor, frequently painful with variable relief obtained with salicylates. Clinical latency is not exceptional in children, owing to the delayed diagnosis. When located in the femoral neck it is usually discovered after a delay, owing to the misleading clinical signs and the frequently normal standard x rays. The only treatment for cure is the excision of the nidus, which is usually difficult because of the anatomical location and the perioperative adjustment.

[Predominantly crural hemiparesis and ipsilateral ataxia caused by meningioma of the falx cerebri].

In a 61 year-old woman with a right crural hemiparesis, and a history of progressive development of ipsilateral ataxia, CT scan showed a meningioma of falx cerebri confirmed by neurosurgery. Neurologic signs regressed within weeks. Compression of corticopontine and corticospinal tracts at their origin could account for the clinical disorders.

[Technique, complications, possible misinterpretations and indications of retrograde renal phlebography (author's transl)].

A chronological survey is given of the gradual development of retrograde renal phlebography whereby the different techniques are contraste with each other in detail. Today's method of choice is the retrograde selective visualization of renal veins using the Seldinger procedure via the femoral vein, with or without pharmacological influence and applying larger doses of contrast medium (up to 30 ml) and higher injection pressures (up to 6 kg/cm2). Indications for simple and for pharmacophlebography are specified and exemplified by means of a scheme. Possible complications and misinterpretations

Skeletal lymphoma.

The records of forty-two patients with lymphoma of bone [.11 with Hodgkin disease (HD), 31 with non-Hodgkin lymphoma (NHL)] were retrospectively reviewed to correlate histologic type with sites of osseous involvement, radiographic patterns of bone destruction, and prognosis. The vertebral column was the most frequent site of axial involvement and the femur was the commonest site overall. In HD, the most frequent radiographic pattern was vertebral sclerosis, while periosteal reaction was noted in two focal femoral lesions and hypertrophic pulmonary osteoarthropathy in one patient. In NHL, perme

Reconstruction with Custom Unicondylar Hemiarthroplasty following Tumor Resection: A Case Series and Review of the Literature.

For patients with tumors of the distal femur, options for limb salvage include tumor resection followed by reconstruction. While reconstruction commonly involves a distal femoral replacement, careful selection of patients with tumor involvement limited to a single condyle may be candidates for reconstruction with distal femur hemiarthroplasty. In these procedures, resection spares considerably more native anatomy. Three consecutive patients who underwent resection and reconstruction at the distal femur with custom unicondylar hemiarthroplasty are presented in this case series at a mean follow-

Fatal soft-tissue and bony complications following radiotherapy for HIV-negative Kaposi's sarcoma: a rare case of post-radiation necrotizing fasciitis and osteomyelitis in a resource-limited setting.

BACKGROUND: Kaposi's sarcoma (KS) is a vascular neoplasm strongly associated with HIV infection; however, HIV-negative KS is exceptionally rare, particularly in sub-Saharan Africa. Radiotherapy (RT) remains a standard local treatment for KS, providing excellent symptom relief and local control. Despite its efficacy, severe complications such as necrotizing fasciitis (NF) and osteomyelitis are exceedingly uncommon and scarcely reported in HIV-negative patients. CASE PRESENTATION: We report a 47-year-old HIV-negative man with biopsy-confirmed KS of the right thigh, initially treated with chemot

Limb-Sparing Surgery and Stifle Arthrodesis Using Patient-Specific 3D-Printed Guides and Endoprosthesis for Distal Femoral Chondrosarcoma in a Dog: A Case Report.

Limb-sparing techniques for appendicular primary bone tumors are still associated with a high rate of complications. Three-dimensional (3D)-printed patient-specific instruments could reduce these complications. The aim of this study is to describe a limb-sparing surgery using 3D-printed patient-specific guides (PSGs) and an endoprosthesis (PSE) to treat femoral chondrosarcoma in a dog. An eight-year-old female Golden Retriever presented with persistent lameness of the right hind limb, reluctance to move and difficulty in maintaining a standing position. Palpation of the right femur revealed an

Extensive ischemic ulcers due to limb occlusion after endovascular aneurysm repair: a case report.

STUDY DESIGN: Resident's case problem. BACKGROUND: Kaposi's sarcoma (KS) is the most common form of cancer in patients with human immunodeficiency virus (HIV) infection. Although KS is often initially asymptomatic, this neoplasm may progress to affect multiple organ systems, including structures of the musculoskeletal system, which can produce symptoms similar to those associated with common orthopaedic conditions. This resident's case problem describes the evaluation and differential diagnosis of a 45-year-old male with HIV and KS, referred to physical therapy with an initial diagnosis of ra

Pediatric osteosarcoma. Successful retreatment of relapsed primary tumor and soft tissue recurrence with intraarterial cis-diamminedichloroplatin-II.

Six patients with extremity osteosarcoma initially underwent successful treatment of the primary tumor with four to seven courses of intraarterial cis-diamminedichloroplatin-II (IA-CDP; 100 to 150 mg/m2/course). The primary tumors in two patients were then extirpated by limb salvage. However, a local soft tissue recurrence occurred in both patients several months later. The other four patients refused surgical intervention and later also developed local bony recurrences. Reinstatement of three to five courses of IA-CDP in all six patients induced a complete response in five. The local recurren

Current orthopaedic management of bony metastases in the proximal third of the femur.

Patients presenting with skeletal metastatic lesions requiring surgery are increasing, this is due to an ageing population and improved oncological survival following advances in medical management. Furthermore, patients are living longer with skeletal metastatic disease. The majority of bone metastases occur in the proximal femur. Robust diagnosis (Biopsy) and treatment is required to ensure that patients can mobilise fully weight bearing immediately post-surgery and that the construct should outlive the patients expected survival. We review the changing management of metastatic disease in th

Oncologic reconstruction of the proximal femur in children younger than 9 years using a proximal humerus allograft and a hip prosthesis: Report of two cases and description of the surgical technique.

IntroductionProximal femoral tumor resection poses a major challenge in orthopaedic oncology, particularly in very young pediatric patients (<9 years), where the need to preserve function and limb length collides with the limited availability of reconstructive options (3D-printed implants, endoprostheses, biological constructs such as vascularized fibular grafts, and allograft-prosthetic composites) or leads to amputation.ObjectiveTo describe in detail the surgical technique for proximal femoral tumor resection and reconstruction using an allograft-prosthetic composite (APC) with a proximal hu

A bone metastasis of non-small cell lung carcinoma with prominent clear cell features.

We report the case of a 78-year-old man with multiple bone tumours and three round, smooth nodules in the right lung. He was referred to our hospital because of the left femoral neck pathological fracture. The histological characteristics of the femoral tumour corresponded to clear cell carcinoma, and bone and lung lesions were regarded as metastatic from an unknown primary site, since there was no clinical evidence of renal carcinoma. His general conditions gradually declined, and he died 2 months after the operation. At autopsy, no neoplasia lesion was found in the kidneys. Although clear ce
