Antenatal monitoring of the high risk fetus.

Fetus may be at high risk of perinatal mortality or morbidity or childhood morbidity either due to congenital malformations, genetically inherited disorders, chromosomal aberrations or due to antenatal and/or intrapartum hypoxia. Environmental hazards of infection, radiation and drug ingestion or maternal medical disorders are associated with some risk to the fetus. So it is important to identify those at risk by obtaining a detailed medical and family history. Maternal age and past obstetric history would also suggest the high risk nature of the pregnancy. A detailed clinical examination of t

Monoamniotic twins in a triplet gestation with favorable outcome.

BACKGROUND: Monoamniotic twins in a triplet gestation is a rare combination. Each condition is separately associated with significant perinatal morbidity and mortality. There are no data about the outcome and management of such gestations. CASE: We report a case of monoamniotic twins in a spontaneous dichorionic triplet gestation with a favorable outcome. Transvaginal ultrasound examination at nine weeks confirmed the diagnosis. Close follow-up was established that included continuous fetal monitoring after 30 weeks. At 35 weeks and after confirming fetal lung maturity, elective cesarean deli…

The natural history of an umbilical cord hemangioma.

Umbilical cord hemangiomas are rare, and the natural history is poorly understood. We present a case where the clinical course was complicated by distal umbilical cord edema, episodes of proximal obstruction of umbilical artery blood flow, transient fetal pleural and pericardial effusions, and position-dependent abnormal fetal heart rate monitoring with periods of sustained fetal tachycardia. Delivery was performed for fetal growth restriction with abnormal fetal surveillance. This case highlights possible mechanisms for fetal decompensation as well as the importance of a multifaceted approach

Fetal diagnosis of KCNQ1-variant long QT syndrome using fetal echocardiography and magnetocardiography.

A pregnant woman with KCNQ1 variant long QT syndrome (LQTS) underwent fetal magnetocardiography (fMCG) after atrioventricular (AV) block was noted during fetal echocardiogram-atypical for LQTS type 1. Concern for fetal LQTS on fMCG prompted monitoring of maternal labs, change of maternal beta blocker therapy, and frequent fetal echocardiograms. Collaboration between obstetricians, neonatologists, and pediatric cardiologists ensured safe delivery. Beta blocker therapy was initiated after birth, and postnatal evaluation confirmed genotype and phenotype positive LQTS in the infant. Our experience

A customized bed based stand alone array of optically pumped magnetometers for fetal magnetocardiography measurements.

Fetal magnetocardiography (fMCG) is a non-invasive technique that measures the magnetic fields associated with fetal heart electrical activity outside of the maternal abdomen. fMCG has high temporal precision for measuring fetal heart rate and its variability which reflects fetal neurodevelopment. Free of cryogenics and low-cost sensors called microfabricated optically pumped magnetometers (OPMs) have emerged as an alternate to cryogenic SQUID (Superconducting Quantum Interference Device) systems to record fMCG. Previous research has demonstrated the ability of the OPMs to measure the fMCG at

Fetal scalp blood sampling during labor: an appraisal of the physiological basis and scientific evidence.

Fetal cardiotocography is characterized by low specificity; therefore, in an attempt to ensure fetal well-being, fetal scalp blood sampling has been recommended by most obstetric societies in the case of a non-reassuring cardiotocography. The scientific agreement on the evidence for using fetal scalp blood sampling to decrease the rate of operative delivery for fetal distress is ambiguous. Based on the same studies, a Cochrane review states that fetal scalp blood sampling increases the rate of instrumental delivery while decreasing neonatal acidosis, whereas the National Institute of Health an

European experience with a novel noninvasive sensor for intra-amniotic or extra-amniotic evaluation of fetal oxygen saturation.

OBJECTIVE: We evaluated the feasibility of a new instrument for continuous fetal pulse oximetry during labor. The measuring sensor can be placed on the fetal back before or after rupture of membranes. METHODS: One hundred adult women who had completed 32 weeks of gestation and had an anticipated duration of labor greater than 30 minutes were included in the study. Patients with premature rupture of membranes for 24 hours or more, low placental localization, placenta previa or abruption, vaginal bleeding, acute infection, polyhydramnios, oligohydramnios, or uterine or congenital abnormalities

ACOG practice bulletin. Antepartum fetal surveillance. Number 9, October 1999 (replaces Technical Bulletin Number 188, January 1994). Clinical management guidelines for obstetrician-gynecologists.

The goal of antepartum fetal surveillance is to prevent fetal death. Antepartum fetal surveillance techniques based on assessment of fetal heart rate patterns have been in clinical use for almost three decades. More recently, real-time ultrasonography and Doppler velocimetry have been used to evaluate fetal well-being. Antepartum fetal surveillance techniques are now routinely used to assess the risk of fetal death in pregnancies complicated by preexisting maternal conditions (e.g., type 1 diabetes mellitus) as well as those in which complications have developed (e.g., intrauterine growth rest

Antepartum predictors of fetal distress in postterm pregnancy.

The purpose of this study was to determine the efficacy of combining nonstress testing with ultrasound assessment of amniotic fluid volume for the antenatal evaluation of the postterm fetus. Postterm patients (884) were managed with amniotic fluid assessments and nonstress tests (including evaluation for variable and late decelerations) twice a week. There were no perinatal deaths or major neonatal morbidity. However, the antenatal testing sensitivity, specificity, negative, or positive predictive values were not improved by combining the two tests. Individually, amniotic fluid assessment was

[Fetal congestive heart failure due to nodal tachycardia: in utero digoxin therapy ].

The authors report one case of fetal congestive heart failure secondary to nodal tachycardia, revealed in the 26th week of gestation. Transplacental digoxin treatment stopped the tachycardia in a few days and allowed the pregnancy to continue to term, with delivery of a normal child. This report shows the value of fetal echocardiography in cases with arrhythmias, the possibility of in utero digoxin treatment of supraventricular tachycardia and the follow-up procedures.

[Electronic surveillance during delivery. A method of reading the tracings].

A method for reading the foetal heart rate tracings and a guide of management advise in each case have been established from a review of the literature and the study of 500 records. The tracings are schematically divided into three categories according to the presumed risk of foetal distress: normal or "tolerable" tracings, alarm symptoms and danger symptoms. For each categorie, and according to the duration of the variations, a type of management is proposed. Other diagnostic means, such as capillary blood pH measurement are desirable to improve the foetal state diagnostic and make the treatm

Significant fetal bradycardia during antepartum heart rate testing.

Reviewed were 800 patients who underwent antepartum fetal heart rate testing. Significant fetal bradycardia (less than 90 beats per minute for longer than 50 seconds or a decrease of 40 beats per minute below baseline for longer than 50 seconds) was evaluated relative to its relationship to fetal outcome. Twelve fetuses exhibited significant bradycardia during monitoring. All of these babies, when delivered within a short period of time after the tracing, showed manifestations of ongoing intrauterine jeopardy. The finding of significant fetal bradycardia during antepartum testing should prompt

The prediction and prevention of intrapartum fetal asphyxia in term pregnancies.

OBJECTIVE: This study was undertaken to examine the roles of clinical risk scoring, electronic fetal heart rate monitoring, and fetal blood gas and acid-base assessment in the prediction and prevention of intrapartum fetal asphyxia in term pregnancies. STUDY DESIGN: The outcomes of 166 term pregnancies with biochemically confirmed fetal asphyxia (umbilical artery base deficit at delivery, >12 mmol/L) were examined. This population included 83 pregnancies delivered abdominally matched with 83 pregnancies delivered vaginally. Antepartum and intrapartum clinical risk factors and neonatal complic

International Expert Consensus on the Diagnosis and Clinical Management of Primary Fetal Pleural Effusion.

OBJECTIVE(S): Fetal pleural effusion is a rare condition that may significantly impact fetal and neonatal outcomes. Currently, no universal standardized approach exists for diagnosing and managing primary fetal pleural effusions. This study aimed to develop structured, expert-based clinical guidelines for the diagnosis, monitoring, management, and follow-up care of primary fetal pleural effusion. METHODS: A Delphi method was employed to achieve consensus among an international panel of experts in fetal medicine. Experts were selected based on their clinical expertise, affiliations, and releva

ACOG Committee Opinion No. 474: nonobstetric surgery during pregnancy.

The American College of Obstetricians and Gynecologists' Committee on Obstetric Practice acknowledges that the issue of nonobstetric surgery during pregnancy is an important concern for physicians who care for women. It is important for a physician to obtain an obstetric consultation before performing nonobstetric surgery and some invasive procedures (eg, cardiac catheterization or colonoscopy) because obstetricians are uniquely qualified to discuss aspects of maternal physiology and anatomy that may affect intraoperative maternal-fetal well-being. Ultimately, each case warrants a team approac
