Ulnar Parametacarpal Flap for Small Finger Avulsion Injury.

In difficult cases of replantation following small finger avulsion injury, in which amputation occurs at the proximal interphalangeal joint, the ulnar parametacarpal island flap, rotated 180° (propeller flap), can be used as an alternative method for covering a skin defect of the proximal phalanx. This flap can prevent metacarpophalangeal joint dysfunction and additional finger shortening. We propose the use of an ulnar parametacarpal flap for this purpose and report the outcomes of two successful cases treated with this method and followed up for 12 months.

The Pathogenesis and Treatment of the Stiff Finger.

"Stiff finger," defined as a finger with decreased range of motion in one or more joints, is commonly found after hand injury and is classified into flexion or extension deformities. Pathogenesis is due to dysfunction in one or more of the following anatomic components: (1) osseous and articular; (2) capsuloligamentous; (3) musculotendinous units; (4) soft tissue and fascia. Evaluation and treatment are based on accurate identification and correction of pathologic structures. The mainstay of treatment is directed hand therapy with exercises and splinting to mobilize stiff joints. Operative int

The results of serial casting in the treatment of proximal interphalangeal joint contractures in patients with systemic sclerosis.

BACKGROUND: Systemic sclerosis can lead to significant disability. Flexion contractures of the proximal interphalangeal joints are common in systemic sclerosis. Serial casting can be used to correct these contractures. The literature lacks data on the use of this method for this purpose. PURPOSE: To analyze the results of serial casting intervention in the treatment of proximal interphalangeal joint flexion contractures and identify the factors that may affect outcomes. STUDY DESIGN: Retrospective case series. METHODS: The data of 10 patients treated with serial casting for their proximal i

Wrapping degloved fingers with a distal-based radial forearm perforator flap: A repair method for multiple digital degloving injury.

Degloving hand injuries present challenging situations to hand surgeons as they present with difficulties in reconstructive microsurgery, particularly in multiple digital injuries. Time is a limiting factor when multiple degloving finger amputations are present. Thus, we proposed a repair method for multiple degloving finger injuries that involves coverage with a distal-based reverse forearm flap of all injured fingers in a two-stage procedure. Early vigorous physical therapy after pedicle division of the flap at postoperative third week eliminates joint stiffness problems at wrist and metacar

Experimental Evaluation of the Elson Test Efficiency Following Central Slip Injury.

PURPOSE: The purpose of this article is to explore the amount of work, quantitated by flexion and extension cycles, that is needed to obtain a positive Elson test following a central slip injury. METHODS: Thirteen frozen cadaveric fingers from individuals with an average age of 79.6 years were used. Testing was performed by imposing sinusoidal displacement of the 2 tendons, with loads ranging from 30 N to 2 N at 1 Hz. Following transection to the central slip, each finger was cycled 1,000 times using the same protocol adopted for the control. Following 100, 200, 300, and 1,000 cycles, we meas

[Arthroplasty at the proximal interphalengeal joint].

BACKGROUND: For the treatment of deformed finger middle joints that occur as a result of osteoarthritis or inflammatory and post-traumatic destruction, artificial joint replacement is an option to arthrodesis. Hereby mobility can be obtained, which is why this method generally has a higher acceptance. DISCUSSION: What clinical and subjective results can be expected after artificial finger middle joint replacement and what is the expected complication rate for the different types of joints? MATERIAL AND METHODS: We present our experiences with artificial joint replacement with various implant

Osteoarthritis and psoriatic arthritis: findings in three-dimensional biophotonics imaging.

The goal of this study is to capture the quantitative optical features of degenerative finger joints based on x-ray aided three-dimensional (3D) diffuse optical tomography (DOT). It is anticipated that the fused imaging technique can be applied to identifying the significant differences between osteoarthritis (OA) and psoriatic arthritis (PA). For a case study, total 6 subjects were selected to study the distal interphalangeal (DIP) finger joints. 2 OA patients, 2 PA patients and 2 healthy subjects were examined clinically first. Their DIP finger joints were then scanned by the multimodality i

Microvascular free on-top plasty in multidigital amputations.

Bidigital amputations proximal to the PIP joints severely impair function and cosmesis of the hand. The author presents a clinical series of five patients in which a three-fingered hand was reconstructed through microvascular transfer of one of the stumps on top of the other. Five male patients with amputation of two fingers proximal to the PIP joint were treated surgically. One of the stumps was transferred as a local free composite flap on top of the other stump. All five transfers were successful. The mean operative time was 2 h 15 min. The subjective improvement on hand function and cosmes

Axillary vein thrombosis as a manifestation of rapidly progressive neuropathic arthropathy of the shoulder associated with syringomyelia.

Neuropathic arthropathy is a destructive joint disease associated frequently with loss of proprioception. Syringomyelia (with or without Arnold-Chiari malformation) is characterized by slow progression over many years; the joints involved most frequently are the shoulders and elbows. Neuropathic arthropathy of the hand is rare, and clinical features are vasomotor problems, digital ulcers, thickening of the fingers, and painless subluxation of the finger joints. We report an unusual case of neuropathic arthropathy of the shoulder and hands, associated with syringomyelia without Arnold-Chiari ma

Modification of the extension block Kirschner wire technique for mallet fractures.

A modification of the extension block Kirschner wire technique that is used for closed reduction of mallet fractures is described. Eighteen mallet avulsion fractures of the distal phalanx treated with this modification were included in this prospective study. The fingers affected were nine small fingers, eight ring fingers, and one long finger. Surgical indications included fractures involving more than 33% of the articular surface and fractures associated with subluxation of the distal phalanx that could not be corrected by closed reduction. The average joint surface involvement was 39.8%. At

Transfer of the Fifth Finger to Replace an Amputated Thumb: A Preliminary Study.

Amputation of the thumb causes not only loss of significant functional use of the hand but also psychological and social problems. The procedures of toe-to-thumb transfer, pollicization, or metacarpal extension are recognized and well-documented options in thumb reconstruction. Although these techniques have been successfully applied, they have some disadvantages. This study aims to test the feasibility of the fifth finger for thumb reconstruction. The fifth finger was released following exploration of the digital artery, vein, nerve, and flexor and extensor tendons, including the flexor retin

Linburg-Comstock phenomenon following forearm laceration.

A 20-year-old man presented with an inability to flex the interphalangeal joint of the right thumb without simultaneous flexion of the distal interphalangeal joint of the index finger following a penetrating injury to the right forearm. With a clinical suspicion of intertendinous adhesions between the flexor pollicis longus and the flexor digitorum profundus to the index finger, surgical exploration under wide-awake anesthesia was performed. Intraoperatively, the intertendinous adhesions were identified and divided completely. Postoperatively, the patient achieved good, independent flexion of

Rupture of the central slip of the extensor hood of the finger. A test for early diagnosis.

Closed rupture of the middle slip of the extensor hood of a finger is easily missed until the late appearance of a buttonhole deformity. Early diagnosis gives the best chance of satisfactory treatment, but Boyes' test becomes positive only at a late stage. A new test is described in which, from a 90 degree flexed position over the edge of a table, the patient tries to extend the proximal interphalangeal joint of the involved finger against resistance. The absence of extension force at the proximal joint and fixed extension at the distal joint are immediate signs of complete rupture of the cent

Entrapment of the flexor digitorum superficials in the radius fracture site.

Entrapment of the flexor tendon after fracture of both forearm bones is very rare. This report describes entrapment of the middle finger flexor digitorum superficialis (FDS) tendon alone in the radius fracture site. A 13-year-old boy fractured both forearms and had closed reduction and a long-arm cast. After the cast was removed he had full middle finger motion with the wrist flexed but was unable to extend the proximal interphalangeal or metacarpophalangeal joints and could extend only the distal interphalangeal joint of the middle finger with the wrist in the neutral or extension position. Concomitant injuries affect prognosis in patients with central slip tear.

Central slip tears often occur with concomitant hand injuries. However, the outcome of a central slip tear and the effect of concomitant injuries are rarely reported. We evaluated 67 fingers in 63 patients with central slip tears who underwent primary surgery in our hospital between April 2009 and June 2017. We performed multivariate analyses, with proximal interphalangeal (PIP) joint active range of motion (AROM) and existing extension lag greater than 10° as dependent variables and age, existence of concomitant fractures, skin defects, collateral ligament injuries, ruptured lateral bands, ru

Modification of the Zancolli Lasso Procedure for Simultaneous Correction of Wartenberg's Sign.

Ulnar nerve injury initiates an imbalance between the intrinsic muscles and extrinsic extensors of the ring and small fingers, which leads to the characteristic hyperextension of the metacarpophalangeal (MP) joints and flexion of the proximal interphalangeal joints of these 2 digits-commonly referred to as the ulnar claw hand. In addition to these changes in the static posture of the hand, ulnar nerve palsy severely impairs grasp due to deficient active MP joint flexion. In most cases, motor balance can be restored by preventing MP joint hyperextension and augmenting MP joint flexion using the

Activation of Superficial and Deep Finger Flexors Through Transcutaneous Nerve Stimulation.

OBJECTIVE: Functional electrical stimulation (FES) is a common technique to elicit muscle contraction and help improve muscle strength. Traditional FES over the muscle belly typically only activates superficial muscle regions. In the case of hand FES, this prevents the activation of the deeper flexor muscles which control the distal finger joints. Here, we evaluated whether an alternative transcutaneous nerve-bundle stimulation approach can activate both superficial and deep extrinsic finger flexors using a high-density stimulation grid. METHODS: Transverse ultrasound of the forearm muscles w

Pressing fixation of mallet finger fractures with the end of a K-wire (a new fixation technique for mallet fractures).

AIM: The aim of this study was to describe and evaluate a surgical technique for the treatment of mallet finger fractures using a K-wire stabilization of the distal interphalangeal (DIP) joint and another K-wire pressing the bone fragment. METHODS: Between June 2007 and March 2014, 41 patients (28 men, 13 women) with isolated closed mallet finger fracture were treated using two K-wires. In the cohort, the mean joint surface involvement was 44.3% (range: 28-62%). With a mean period of 23.6 months, patient follow-up lasted 13-34 months. The fingers were evaluated for loss of extension and flexi
