Fine needle aspiration of sclerosing lymphocytic lobulitis of the breast. A report of two cases.

BACKGROUND: Sclerosing lymphocytic lobulitis (SLL) was described in 1948 by Soler and Khardori as fibrotic disease of the breast with histologic features similar to those of Hashimoto's thyroiditis. Associations of this process with thyroiditis, artropathy and diabetes mellitus have been seen. CASES: We report the fine needle aspiration biopsy (FNAB) findings of two cases of SLL seen in our service. Both patients showed a breast nodule suspicious for malignancy. The cytologic changes vary according to the stage of the disease, from abundant lymphocytes and scanty fibrosis (case 1) to the pres

A single mycobacterial ligand organizes multi-receptor signaling to reprogram macrophage lipid metabolism.

How innate immune receptors integrate signals from complex microbial ligands remains poorly understood, yet this integration may offer new avenues for host-directed therapies. Here, we show that the architecture of a single pathogen-derived component can organize the coordinated engagement of multiple pattern-recognition receptors to reprogram host cell behavior. We find that the mycobacterial lipoglycan mannose-capped lipoarabinomannan (ManLAM) uses distinct structural features to engage two pattern-recognition receptors, Toll-like receptor 2 (TLR2) and Dectin-2, thereby driving macrophage li

An unusual histologic variant of necrobiotic xanthogranuloma.

Necrobiotic xanthogranuloma (NXG) usually shows a stereotypical histopathologic presentation. However, few unusual cases have been published. We present a patient with NXG showing exceptional histopathologic features. NXG in our patient presents with exclusively dermal granulomatous inflammation mimicking interstitial granuloma annulare. Not only subcutaneous involvement, but also, evident zones of degenerated collagen, foam cells, and cholesterol clefts were missing. Moreover, the case shows overlaps with recently published granulomatous scleromyxedema. Some common clinical and histopathologi…

[Reverse cholesterol transport processes and their role in artherosclerosis regression].

Atherosclerosis represents dynamic process with periodic tendency to reverse. Although animal experiments and human studies have provided considerable evidence of atherosclerosis regression, our understanding of this phenomenon remains still incomplete. Atherosclerosis regression depends on removal of cholesterol deposits from atherosclerotic plague in the process of reverse cholesterol transport and possibly other mechanisms including migration of macrophages and foam cells, inhibition of inflammation and endothelial regeneration. Our paper reviews available data on the process of atheroscler…

Loss of Vacuole Membrane Protein 1 protects against acetaminophen-induced liver injury by modulating inflammation and hepatocyte proliferation.

Acetaminophen (APAP) overdose is a leading cause of severe liver injury and acute liver failure (ALF). Increased hepatic macrophages promote … and glutathione (GSH) levels were similar between genotypes, but increased lipid-laden macrophages (foam cells) were present in Vmp1LKO livers. Interestingly, APAP protein adduct formation decreased in Vmp1LKO mouse livers, but mitochondrial damage was similar in both phenotypes. Peroxynitrite formation was observed in APAP-treated Vmp1LWT hepatocytes but wa …

APOL1 renal risk variants promote cholesterol accumulation in tissues and cultured macrophages from APOL1 transgenic mice.

Apolipoprotein L1 (APOL1) genetic variants G1 and G2, compared to the common allele G0, are major risk factors for non-diabetic kidney disease in African descent populations. APOL1 is a minor protein component of HDL, as well as being expressed in podocytes and vascular cells. Reverse cholesterol transport involves the transport of cholesterol to HDL by cellular ATP-binding cassette; ABCA1 and ABCG1 with subsequent delivery from peripheral tissues to the liver. With impaired reverse cholesterol transport, lipid accumulation occurs and macrophages morphologically transform into foam cells, rele…

Adult Xanthogranulomatous Disease of the Orbit: Clinical Presentations, Evaluation, and Management.

Adult xanthogranulomatous disease of the orbit refers to a heterogeneous group of clinical syndromes with differing degrees of systemic involvement and distinct prognoses. The different syndromes all present clinically with progressively enlarging, yellowish lesions of the orbit. Histologically, the lesions are characterized by an inflammatory infiltrate of foam cells and Touton-type multinucleated giant cells. The xanthomatized histiocytes are CD68+, S100-, and CD1a-. There are 4 clinical forms of xanthogranulomatous disease of the orbit: adult xanthogranulomatous disease of the orbit, adult …

Early diagnosis in Niemann-Pick disease.

Hepatosplenomegaly, observed on routine physical examination of a 3-month-old French-Canadian infant, was the first evidence for the possibility of Niemann-Pick disease. Vacuolated foam cells filled with phospholipid material were found in liver and bone marrow biopsy material. The absence of sphingomyelinase activity in isolated peripheral leukocytes and cultured skin fibroblasts confirmed the diagnosis. The parents' leukocytes displayed significantly less activity than was found in control cells. Exact and early confirmation of the diagnosis of Niemann-Pick disease is of prime importance for…

β-Endorphin Mediates the Development and Instability of Atherosclerotic Plaques.

β-Endorphin, an endogenous opioid peptide, and its μ-opioid receptor are expressed in brain, liver, and peripheral tissues. β-Endorphin induces endothelial dysfunction and is related to insulin resistance. We clarified the effects of β-endorphin on atherosclerosis. We assessed the effects of β-endorphin on the inflammatory response and monocyte adhesion in human umbilical vein endothelial cells (HUVECs), foam cell formation, and the inflammatory phenotype in THP-1 monocyte-derived macrophages, and migration and proliferation of human aortic smooth muscle cells (HASMCs) in vitro. We also assess

Size-selective uptake of colloidal low density lipoprotein aggregates by cultured white blood cells.

This paper illustrates how principles of colloid science are useful in studying atherosclerosis. Accumulation of foam cells in the arterial intima is a key step in atherogenesis. The extent of foam cell formation is enhanced by low density lipoprotein (LDL) aggregates, and we have previously shown that the size of sphingomyelinase (Smase)-hydrolysis-induced aggregates depends directly on the concentration of ceramide generated in the LDL phospholipid monolayer, mediated by the hydrophobic effect. Here, we focus on the effect of LDL aggregate particle sizes on their subsequent uptake by macroph…

Electron microscopy of chlorphentermine lung.

Smith, P., Heath, D., and Hasleton, P. S. (1973).Thorax, 28, 559-566. Electron microscopy of chlorphentermine lung. Electron microscopy of lung tissue from rats treated with chlorphentermine shows an intense hyperplasia in the alveolar walls of granular pneumocytes which remain attached and do not migrate into the alveolar spaces. Numerous `foam cells' are found free in the alveoli and are histiocytes which have ingested osmiophilic lamellar bodies. Similar laminated inclusions are found in granular pneumocytes, membranous pneumocytes, bronchiolar Clara cells, pulmonary macrophages, and endoth…

Vascular smooth muscle cell metabolic reprogramming and phenotypic remodeling in atherosclerosis.

Metabolic reprogramming of vascular smooth muscle cells (VSMC) is emerging as a central driver of atherosclerotic plaque heterogeneity and instability. VSMCs undergo phenotypic remodeling into osteogenic, macrophage-like, foam cell-like, or pro-inflammatory states through metabolic reprogramming, which actively drives vascular calcification, lipid accumulation, and extracellular matrix degradation. In this review, we summarize the various phenotypes of VSMCs observed during AS development and describe potential molecular pathways linking metabolic reprogramming to phenotypic remodeling. We hig

Developing a Biomimetic 3D Neointimal Layer as a Prothrombotic Substrate for a Humanized In Vitro Model of Atherothrombosis.

Acute cardiovascular events result from clots caused by the rupture and erosion of atherosclerotic plaques. This paper aimed to produce a functional biomimetic hydrogel of the neointimal layer of the atherosclerotic plaque that can support thrombogenesis upon exposure to human blood. A biomimetic hydrogel of the neointima was produced by culturing THP-1-derived foam cells within 3D collagen hydrogels in the presence or absence of atorvastatin. Prothrombin time and platelet aggregation onset were measured after exposure of the neointimal models to platelet-poor plasma and washed platelet suspen…

Addressing dyslipidemic risk beyond LDL-cholesterol.

Despite the success of LDL-lowering drugs in reducing cardiovascular disease (CVD), there remains a large burden of residual disease due in … ll formation and inflammation. Low levels of HDL may associate with reduced cholesterol efflux from foam cells, aggravating atherosclerosis. While fibrates and fish oils reduce TRL, they have not been uniformly successful in reducing CVD, and there is a large unmet need for new approaches to reduce remnants and CVD. Rare genetic variants that lower triglycer …

[Contemporary views on the etiology and pathogenesis of atherosclerosis].

Atherosclerosis is caused by a large range of exogenous and endogenous risk factors and does not result simply from the hypercholesterolemia and the accumulation of lipids. Atherogenesis is a dynamic multifactor and multi-stage inflammatory process, where different cells, cytokines and growth factors interacte as results of endothelial cell damage. This process is characterized by a focal subintimal deposition of lipids and lipoproteins in "foam cells" with a further development of chronic focal inflammation and expanditure of fibrosis, stenosis of the arterial lumen and development of further…
