Outcomes of patients born with single-ventricle physiology and aortic arch obstruction: the 26-year Melbourne experience.

BACKGROUND: To review the long-term outcomes of patients born with single-ventricle physiology and aortic arch obstruction. METHODS: Follow-up of 70 consecutive neonates undergoing single-ventricle palliation and arch repair, excluding hypoplastic left heart syndrome, between 1983 and 2008, was reviewed. Dominant arch anomalies were coarctation (n = 48), interrupted arch (n = 10), and hypoplastic arch alone (n = 12). Neonatal Damus procedure with arch repair and shunt became the dominant approach, being performed in 1 (10%) of 10 in 1983 to 1989, 9 (32%) of 28 in 1990 to 1999, and 23 (72%) of

Safe Anesthetic Management of One-Lung Ventilation in an Adolescent Patient With Fontan Circulation: A Case Report.

We present a case report on anesthesia management for a 16-year-old male with Fontan circulation undergoing pacemaker implantation, necessitating one-lung ventilation. The patient had a complex congenital heart condition and had previously undergone multiple surgeries, including a Fontan procedure. Due to his unique cardiovascular anatomy, a left thoracotomy was chosen for pacemaker implantation, which required one-lung ventilation for surgical approach and visualization. This procedure can pose significant risks in patients with Fontan circulation due to potential intraoperative increases in …

Association of Pre-Fontan Hemodynamics With Long-Term Outcomes After Fontan Palliation: A Study From the Pediatric Cardiac Care Consortium.

BACKGROUND: Long-term outcomes after Fontan vary widely. Although pre-Fontan hemodynamics predict early failure, their association with long-term outcomes remains unclear. We hypothesized that pre-Fontan hemodynamics predict long-term risk of death or transplantation. METHODS: We analyzed data from the Pediatric Cardiac Care Consortium, a US-based multicenter registry including patients undergoing first-time Fontan with pre-Fontan catheterization and long-term follow-up. Patients undergoing a Fontan procedure before 18 years of age at any time between 1982 and 2011 were included in the study.…

Performance of the Occlutech® Atrial Flow Regulator Device in Patients With Either Failing Fontan Circulations or Irreversible Pulmonary Hypertension.

BACKGROUND: The presence of an atrial level right to left shunt may be desirable in patients with severe pulmonary hypertension (PH) or failing Fontan circulation. The Occlutech Atrial Flow Regulator (AFR) was originally designed to create an atrial shunt for left heart failure and is currently being used off-label in patients with PH or failing Fontan circulation. There is limited evidence supporting AFR use for these indications. AIMS AND METHODS: We performed a retrospective review of consecutive patients with Fontan circulation or severe irreversible PH who underwent AFR implantation. Com

Balanced Hepatic Venous Flow with Hepatic to Azygos Vein Shunt Following the Kawashima Procedure.

Objectives: Ensuring balanced hepatic venous (HV) flow to both lungs is vital for preventing pulmonary arteriovenous malformations (PAVMs) after the Kawashima procedure in patients with polysplenia and interrupted inferior vena cava. This study investigated the long-term distribution of HV flow following two Fontan completion methods: HV-to-azygos vein shunt (HVAZ) and HV-to-pulmonary artery shunt (HVPA). Methods: We retrospectively reviewed 20 patients who underwent Fontan completion after the Kawashima procedure between 2004 and 2021 (HVAZ, n = 10; HVPA, n = 10). HV flow distribution was ass

Virtual surgical planning, flow simulation, and 3-dimensional electrospinning of patient-specific grafts to optimize Fontan hemodynamics.

BACKGROUND: Despite advances in the Fontan procedure, there is an unmet clinical need for patient-specific graft designs that are optimized for variations in patient anatomy. The objective of this study is to design and produce patient-specific Fontan geometries, with the goal of improving hepatic flow distribution (HFD) and reducing power loss (Ploss), and manufacturing these designs by electrospinning. METHODS: Cardiac magnetic resonance imaging data from patients who previously underwent a Fontan procedure (n = 2) was used to create 3-dimensional models of their native Fontan geometry usin…

Electrocardiographic ST-segment depression in children with fontan circulation.

Despite successful operation, many patients palliated with a Fontan-type procedure continue to show effort limitation. We previously observed that these children showed electrocardiographic ST depression during exercise tests. The purpose of the study was to investigate whether electrocardiographic ST depression is a common finding in children with Fontan circulation. Forty-two children in two groups were examined: group A consisted of 14 patients who had all undergone a modified Fontan procedure, and group B consisted of 28 children with a structurally normal heart, matched for length, weight…

New developments in the treatment of hypoplastic left heart syndrome.

Hypoplastic left heart syndrome (HLHS) was once a uniformly fatal disease, accounting the majority of neonatal deaths due to congenital heart defects. Twenty-five years of advances in the surgical and medical management of this disease have resulted in dramatic improvements in survival for these children. The goal for patients with HLHS should be survival of near 100% with a good quality of life. The advances described in this manuscript detail some of the new techniques used in the surgical and medical management. For infants undergoing staged reconstruction, the Norwood procedure is performe

Contemporary outcomes of the Fontan operation: An Australian and New Zealand Fontan Registry study.

OBJECTIVE: We sought to review the impact of extracardiac conduit size and risk factors on the long-term outcomes of the contemporary Fontan procedure: lateral tunnel and extracardiac conduits. METHODS: Data were collected from a binational registry. Composite outcome of late adverse events included death, transplant, Fontan failure, supraventricular tachycardia (SVT), thromboembolism, or pacemaker implantation. RESULTS: Transplant-free survival and freedom from Fontan failure was 85% (95% CI, 80%-89%) and 75% (95% CI, 70%-80%) at 30 years, respectively. Extracardiac conduit sizes were avail…

Incidence and implications of systemic to pulmonary collaterals after bidirectional cavopulmonary anastomosis.

BACKGROUND: Systemic to pulmonary arterial collaterals often develop after bidirectional cavopulmonary anastomosis (BCPA). It has been proposed that such collaterals may be related to perioperative outcome and duration of effusions after the modified Fontan procedure. However, the incidence and significance of collaterals after BCPA remain uncertain. METHODS: To evaluate risk factors for, and significance of, such collaterals, we reviewed angiographic and clinical data for all 76 patients who underwent BCPA between January 1990 and June 1996 and had follow-up catheterization during or before …

Untargeted Serum Proteomics in the Fontan Circulation Reveals Three Distinct Molecular Signatures of Fontan Physiology with CYB5R3 Among Key Proteins.

The total cavopulmonary anastomosis (Fontan procedure), a palliative procedure for single-ventricle congenital heart disease, improves survival but is associated with progressive multiorgan complications and high long-term morbidity. Prior blood-based proteomic studies in adults have been limited to targeted antibody-based panels or focused on methodological comparisons. Systemic molecular alterations in younger, clinically heterogeneous patients, particularly in untargeted pathways, remain incompletely characterized. Serum samples from 48 Fontan patients and 48 age- and sex-matched healthy co…

Hemoptysis in a single lung Fontan: percutaneous approach.

BACKGROUND: The Fontan procedure is sometimes the last palliative surgery for complex congenital heart disease with single-ventricle physiology. According to some authors, atrial-baffle fenestrations with right-to-left shunts have improved the surgical outcome of the Fontan operation. Percutaneous fenestration closure at a later stage may increase saturations and reduce the risk of embolic complications. A case could be made that the fenestration should be left patent if the patient, having lost the continuity of the pulmonary artery to one lung, only has a single functional lung. CASE REPORT…

Assessment of functional capacity among students with univentricular heart.

A cross-sectional study was done in students with univentricular heart after undergoing total cavopulmonary connection (Fontan procedure) to establish their baseline functional capacity based on the six-minute walk test and identify potential determining factors. Thirty patients were included; their median age was 12 years old. Eighteen patients were males. The median distance walked was 551.3 meters, 84 % of the theoretical distance for a healthy pediatric population. Height, pre-test systolic blood pressure, and resting arterial oxygen saturation showed a significant association with the dis…

Heart beat but not respiration is the main driving force of the systemic venous return in the Fontan circulation.

The Fontan procedure provides relief from cyanosis in patients with univentricular hearts. A major clinical unmet need is to understand whether the venous flow patterns of the Fontan circulation lead to the development of congestive hepatopathy and other life-threatening complications. Currently, there is no consensus on whether heart beat or respiration is the main driving force of venous return and which one affects the periodic flow changes for the most (i. e., pulsatility). The present study, for the first time, quantified respiratory and cardiac components of the venous flow in the inferi…

Pathophysiology of endothelial dysfunction in Fontan circulation: from bench to bedside and back again.

The Fontan procedure is a definitive surgical approach for complex cardiac malformations, redirecting systemic venous blood into the pulmonary circulation through a staged repair that separates systemic and pulmonary venous returns in the absence of a subpulmonary ventricle. The ensuing unique hemodynamic conditions compromise the endothelial function both in the pulmonary and the systemic circulation. The underlying pathophysiological mechanisms, although distinct within each vascular bed, are interrelated and may collectively contribute to progressive end-organ dysfunction, ultimately accoun…

Temporal progression of P wave abnormality in a patient with classical or atriopulmonary Fontan.

We present the electrocardiographic findings in a 36-year-old female with tricuspid atresia with double-outlet right ventricle and malposition of great arteries who underwent classical or "atriopulmonary" Fontan procedure in childhood. Her electrocardiograms have consistently shown marked intra-atrial delay with an initial positive P wave deflection and terminal negative P deflection in all leads with progressive increase in P wave duration with time. She has had frequent episodes of intra-atrial tachycardia, atrial fibrillation and sinus and post ectopic pauses over the past few years. The fi…

Iatrogenic atrioventricular reentrant tachycardia following Bjork/Fontan palliation of tricuspid atresia: Electro-anatomic mapping, ablation, review and possible mechanism.

This case report describes the successful ablation of an iatrogenic accessory pathway in a Fontan patient. A 15-year-old girl with tricuspid atresia was palliated with a Bjork modification Fontan procedure; six years later she developed supraventricular tachycardia. Electro-anatomic mapping during electrophysiology study localized a concealed iatrogenic atrioventricular accessory pathway and facilitated successful ablation using radiofrequency energy. This report reviews the reported cases of anomalous atrioventricular conduction (antegrade only, retrograde only [Hager, et al., J Thorac Cardio…
