Reverse V-shaped osteotomy for ankylosing rocker-bottom foot deformity in patients with rheumatoid arthritis: A report of three cases.

This study evaluated a reverse V-shaped osteotomy for ankylosing rocker-bottom foot deformity in patients with rheumatoid arthritis. Three feet were presented in this study: rheumatoid rocker-bottom deformities with painful and/or infectious bony prominence towards the bottom of the foot, treated with a reverse V-shaped osteotomy in the mid-hindfoot. In all three cases, significant correction was achieved with restoration of the medial longitudinal arch, and improvement in clinical scores was confirmed. Reverse V-shaped osteotomy has the potential to be a useful and definitive procedure for an

Do We Really Need to Worry About Calcaneocuboid Subluxation During Lateral Column Lengthening for Planovalgus Foot Deformity?

BACKGROUND: Although lengthening of the lateral column through an osteotomy of the anterior calcaneus is an integral component of flatfoot reconstruction in younger patients with flexible planovalgus deformities, the procedure has been implicated in iatrogenic calcaneocuboid (CC) subluxation and subsequent degenerative changes at the CC articulation. The purpose of this study is to characterize alterations at the CC joint after lateral column lengthening (LCL) and determine if Steinmann pin stabilization of the CC joint before distraction maintains a normal relationship. METHODS: Seven matche

Effects of backpack-induced fatigue on gait ground reaction force characteristics in primary school children with flat-foot deformity.

This study aimed to assess the effects of backpack carriage in different weights and muscle fatigue on the GRF components in primary school children with flat-foot deformity. The GRF components from 42 primary school children (21 with low arch, and 21 with normal) were collected before and after backpack-induced fatigue protocol during shod walking with backpacks in weight 7.5, 10, 12.5 and 15% of the child's bodyweight. The amplitudes of Fx2 of 10%BW and Fz3 of 7.5%BW in the flatfeet group were less than in the healthy group without fatigued condition. (P < 0.05). After fatiguing, the amplitu

Splay Toe after Freiberg-Köhler's Osteonecrosis: A Case Report of a Successful Operative Treatment in a Rare Multiplanar Foot Deformity.

"Splay toe" is a rare deformity of the forefoot and often causes the occurrence of metatarsalgia and dysfunction while walking or weight bearing. Since it involves a deviation in the sagittal and transversal planes, often combined with a malrotation, surgical correction can be challenging. We describe a case of splay toe deformity in the forefoot causing metatarsalgia in a 62-year-old female patient with a former avascular osteonecrosis of the 2 metatarsal head Smillie stage V of Freiberg-Köhler's disease causing a splay toe between the 2nd and the 3rd rays. There are only few reports in the l

Hallux valgus, ankle osteoarthrosis and adult acquired flatfoot deformity: a review of three common foot and ankle pathologies and their treatments.

The pathogenesis of hallux valgus deformity is multifactorial. Conservative treatment can alleviate pain but is unable to correct the deformity. Surgical treatment must be adapted to the type and severity of the deformity. Success of surgical treatment ranges from 80% to 95%, and complication rates range from 10% to 30%.Ankle osteoarthrosis most commonly occurs as a consequence of trauma. Ankle arthrodesis and total ankle replacement are the most common surgical treatments of end stage ankle osteoarthrosis. Both types of surgery result in similar clinical improvement at midterm; however, gait

Surgical Treatment Guidelines for Digital Deformity Associated With Intrinsic Muscle Spasticity (Intrinsic Plus Foot) in Adults With Cerebral Palsy.

Intrinsic plus foot deformity has primarily been associated with cerebral palsy and involves spastic contracture of the intrinsic musculature with resultant toe deformities. Digital deformity is caused by a dynamic imbalance between the intrinsic muscles in the foot and extrinsic muscles in the lower leg. Spastic contracture of the toes frequently involves curling under of the lesser digits or contracture of the hallux into valgus or plantarflexion deformity. Patients often present with associated pressure ulcers, deformed toenails, shoe or brace fitting challenges, and pain with ambulation or

Does excessive flatfoot deformity affect function? A comparison between symptomatic and asymptomatic flatfeet using the Oxford Foot Model.

Treatment of asymptomatic flexible flatfeet is a subject of great controversy. The purpose of this study was to examine foot function during walking in symptomatic (SFF) and asymptomatic (ASFF) flexible flatfeet. Thirty-five paediatric and juvenile patients with idiopathic flexible flatfeet were recruited from an orthopaedic outpatient department (14 SFF and 21 ASFF). Eleven age-matched participants with typically developing feet served as controls (TDF). To study foot function, 3D multi-segment foot kinematics and ankle joint kinetics were captured during barefoot gait analysis. Overall, alte

Evaluation of foot biomechanics following surgical interventions, including SPOTT and SPLATT procedures in the management of equinovarus deformity in cerebral palsy hemiplegia.

BACKGROUND: Cerebral Palsy (CP) is a group of disorders affecting movement and posture and is the most common physical disability in children. Surgical interventions, such as split anterior and posterior tibial tendon transfers (SPLATT and SPOTT), can be necessary to correct foot and ankle deformities in CP. This is a retrospective pre-/post-operative cohort study that aims to specifically investigate effects on lower limb and foot biomechanics in children with CP. METHODS: Fifteen children aged 8-16 years at initial assessment (10 girls, 5 boys) underwent SPLATT and SPOTT procedures as part

Evaluating radiologic outcomes of surgical techniques used in foot valgus deformity of cerebral palsy patients: a systematic review and meta-analysis.

BACKGROUND: Planovalgus is the most common deformity in children with cerebral palsy (CP). Numerous surgical interventions like Lateral Calcaneal Lengthening (LCL), Extra-articular Arthrodesis (EAA), Subtalar Arthroereisis (SA), Intra-articular Arthrodesis (IAA), and Talonavicular Arthrodesis (TNA) are available to address it. However, there is currently a lack of evidence defining the optimal surgical approach. This systematic review and meta-analysis aim to evaluate the radiological outcomes of these surgical techniques. METHOD: Databases were searched to identify relevant studies using keyw

Automated landmark identification for diagnosis of the deformity using a cascade convolutional neural network (FlatNet) on weight-bearing lateral radiographs of the foot.

Landmark detection in flatfoot radiographs is crucial in analyzing foot deformity. Here, we evaluated the accuracy and efficiency of the automated identification of flatfoot landmarks using a newly developed cascade convolutional neural network (CNN) algorithm, Flatfoot Landmarks AnnoTating Network (FlatNet). A total of 1200 consecutive weight-bearing lateral radiographs of the foot were acquired. The first 1050 radiographs were used as the training and tuning, and the following 150 radiographs were used as the test sets, respectively. An expert orthopedic surgeon (A) manually labeled ground t
