INTRODUCTION: Herpes simplex virus type 1 (HSV-1) causes various diseases in humans that can lead to disability and death. Current treatments are effective and relatively safe. However, the high prevalence of HSV, the necessity for long-term therapy associated with the development of drug resistance in the virus (mainly in immunocompromised patients) and severe side effects of second-line drugs complicate treatment. Obviously, there is a necessity to develop therapeutic agents with a new mechanism of action and new ways of influencing herpes infection to improve the effectiveness of therapy. T

Conventional therapy for human cytomegalovirus (CMV) relies on inhibition of the viral DNA polymerase. Ganciclovir (GCV) is the first-line therapy, but when GCV-resistant strains emerge, alternative therapies are extremely limited and are associated with significant toxicities. Combination of anti-CMV agents that act on different targets or stages of virus replication has not been well studied, mostly because of the limited number of anti-CMV agents. We report our investigation of combinations of agents that inhibit CMV by targeting the viral DNA polymerase, cellular kinases, or other cell/vir

INTRODUCTION: Untreated CMV retinitis with AIDS leads to blindness; therefore, a life-long virostatic treatment is required. It can either be administered systemically or locally, as there are different advantages and disadvantages. When treating patients we aim at therapy that preserves vision without diminishing quality of life. It should induce as few drug-induced side effects as possible and not shorten the patient's life expectation.
METHODS AND RESULTS: A total of 111 patients (150 eyes) with systemic maintenance treatment were compared retrospectively with 33 patients (62 eyes) that re

BACKGROUND: In the Foscarnet-Ganciclovir Cytomegalovirus (CMV) Retinitis Trial, time to first progression of newly diagnosed CMV retinitis was similar in the 2 treatment groups but was shorter when assessed by grading of fundus photographs at a central reading center than when assessed at the participating clinical centers. This report describes the extent and causes of this disagreement and considers the implications of the findings for clinical practice and future research.
METHODS: Clinical findings and photographic gradings were compared for extent and activity of retinitis at baseline an

The effect of a single subcutaneous injection of different dosages of phosphonoformic or phosphonoacetic acid on the developing dental tissues of the rat molar was studied. The substances were injected at different rat ages and effects on the developing teeth were analyzed by means of histochemistry of frozen sections and scanning electron microscopy. Molars of rats injected at the age of 10 or 15 days showed no pathologic changes. Administration of phosphonoacetic acid in a dosage of 10 or 20 mg P/kg b.w. had no demonstrable effect on the dental tissues in any of the animals. Phosphonoformic

PURPOSE: We sought to understand better the efficacy and risks of local therapies (direct placement of drug into the eye) for the treatment of cytomegalovirus retinopathy. This understanding can be used to design rational treatment regimens, to formulate indications for use of local therapy, and to establish criteria for assessment of future results in this area.
METHODS: We collected information about local therapies through a review of published literature.
RESULTS: Intraocular injection of ganciclovir and foscarnet and implantation of intraocular devices that slowly release ganciclovir ar

Drug-resistant cytomegalovirus (CMV) poses a major clinical challenge in transplant recipients, leading to treatment failure and increased morbidity. This study applied a next-generation sequencing (NGS) approach to identify antiviral resistance mutations (ARMs) in 71 samples from 68 CMV-positive patients who had undergone hematopoietic stem cell transplantation (HSCT) or solid organ transplantation (SOT) between 2018 and 2024. A custom nested-PCR protocol targeting six CMV genes (UL27, UL51, UL54, UL56, UL89, and UL97) was developed for enrichment prior to NGS. ARMs were detected in 23% of pa

Our case demonstrated unique cytomegalovirus (CMV) encephalitis post-haploidentical donor hematopoietic stem cell transplantation (HID-HSCT), with early findings on diffusion-weighted brain magnetic resonance imaging (MRI) in the absence of any neurologic symptoms. A 54-year-old Chinese man with acute lymphoblastic leukemia (Philadelphia chromosome-negative) underwent HID-HSCT. After HSCT, the patient developed CMV viremia and severe acute graft-versus-host disease. Recurrence of CMV viremia was observed. On day 129, brain MRI was performed to determine the cause for the intermittent fever. Di

Letermovir (LMV) is an experimental cytomegalovirus terminase inhibitor undergoing phase 3 clinical trials. Viral mutations have been described at UL56 codons 231 to 369 that confer widely variable levels of LMV resistance. In this study, 15 independent experiments propagating an exonuclease mutant viral strain in escalating LMV concentrations replicated 6 of the 7 published UL56 mutations and commonly elicited additional resistance-conferring mutations at UL56 codons 231, 236, 237, 244, 257, 261, 325, and 329. Mutations were first detected earlier in LMV (median, 3 passages) than in 8 paralle

We report the case of a 52-year-old man with AIDS and CMV bilateral retinitis, treated first with ganciclovir (cymevan) and then with cidofovir. During the treatment, the patient complained of a sharp decrease in visual acuity. Cycloplegics and topical corticosteroids were given. Then cidofovir was stopped and replaced with intravenous Foscavir because of the clinical inefficacy. The anterior uveitis resolved but the ciliary secretion has not stopped after 12 months of follow-up. The patient presented +11 hypermetropia with a choroidal edema and bilateral cataract. This case is interesting for

RATIONALE: The diagnosis of viral encephalitis by conventional and biochemical results of lumbar puncture of cerebrospinal fluid (CSF) is common in neurology, but the diagnosis of specific pathogens has benefited from the improvement of CSF diagnosis technology. Second-generation sequencing allows for more sensitive and accurate pathogen detection. A case of suid herpesvirus encephalitis was reported here.
PATIENT CONCERNS: A 35-year-old male experienced severe right head pain and intermittent right upper limb convulsions, occurring approximately 10 times daily since January 7, 2019. He was s

Acute kidney injury (AKI) occurs commonly in hospitalized patients, especially patients in intensive care units (ICUs). Medications are among the major causative factors of AKI. This narrative review addressed and updated different aspects of anti-infective-associated AKI, including amphotericin B, cidofovir, foscarnet, polymyxins, vancomycin, and aminoglycosides. There is no standard definition or operational criteria to describe anti-infective-associated AKI. Characteristically, it usually occurs during the first two weeks of treatment and is typically dose dependent. Functional resolution o

BACKGROUND: Kaposi sarcoma herpesvirus-inflammatory cytokine syndrome (KICS) is a rare, life-threatening condition associated with Kaposi sarcoma and systemic immune dysregulation induced by Human Herpesvirus 8 (HHV-8). With a mortality rate approaching 60%1 (Goncalves, Ziegelbauer, Uldrick, Yarchoan in Curr Opin HIV AIDS 12(1):47-56, 2017), KICS poses significant diagnostic and therapeutic challenges. This report examines a case of KICS in a patient with well-controlled HIV, emphasising the clinical complexities, treatment strategies, and the need for heightened awareness.

This study introduces a new procedure for antiviral resistance analysis and genetic classification of human cytomegalovirus (HCMV) using next-generation sequencing (NGS) adapted to existing methodologies, aiming for more targets due to the recent use of new antivirals. It expands the classical investigation of mutations in UL54 and UL97 genes, associated with resistance to (val)ganciclovir, foscarnet and cidofovir, to include UL27, UL56 and UL89 genes, which target newer antivirals like maribavir and letermovir. Additionally, it includes the genetic analysis of UL55 (glycoprotein B) for genoty
