Treatment of an open medial tibiotalar dislocation with no associated fracture.

Tibiotalar dislocations without associated fractures are very uncommon in adults, and only a few studies have been published regarding this injury. More than 50% of these dislocations will be posteromedial, with a high incidence of open injuries, and 25% are pure posterior dislocations. In the present report, we discuss our experience and management of a medial tibiotalar dislocation with no associated fracture. In the present case, the patient was brought to the operating room on presentation to our facility and underwent irrigation and debridement with primary closure of his wound. He was im

Nonacute Treatment of Elbow Fracture with Persistent Ulnohumeral Dislocation or Subluxation.

➤ There are patterns of traumatic elbow instability that help a surgeon to anticipate which structures are injured.➤ Patients treated for persistent subluxation or dislocation of the elbow more than two weeks after injury regain less motion and experience more adverse events.➤ The primary goal of treatment is stable reduction of the ulnohumeral joint and functional elbow motion.➤ Motion and pain are affected by contracture and scarring of the soft tissues, malalignment of the joint, fracture malunion, damage to the articular surface, and ulnar neuropathy.➤ Biomechanical and clinical studies su

Proximal humerus head-splitting fracture associated with single-part anterior dislocation.

Fractures that split the humeral head are extremely rare, and usually, the split part is posteriorly dislocated. However, in our case, the split part was anteriorly dislocated and trapped between the anterior glenoid and the subscapularis muscle. In this case, the acquisition of preoperative CT results was vital to plan the exposure and reduction strategies. Open anatomic reduction and internal fixation should be considered as the first treatment of choice in young active adults.

Chronic transscaphoid, transcapitate perilunate fracture dislocation of the wrist: Fenton's syndrome.

The authors report about chronic Fenton's syndrome. This rare injury of the wrist is characterized by scapho-capitate fracture accompanied by 180° rotation of the head of capitate and associated perilunate dislocation. Two patients suffering from chronic Fenton's syndrome were treated with pyrocarbon capitate resurfacing prosthesis. Patients were evaluated according to the wrist range of motion, Mayo modified wrist and DASH scores. In conclusion, prosthetic surgery may achieve satisfactory results for this rare and diagnostically challenging syndrome.

A case of acute dorsal radiocarpal dislocation with radial styloid fracture.

We describe a case report of dorsal radiocarpal dislocation with fracture of the radial styloid process. A 21-year-old right-handed male was injured in a fall while snowboarding. Plain radiographic findings revealed dorsal dislocation of radiocarpal joint with fracture of the radial styloid process; we performed CT studies to examine the radius fracture in greater detail. These findings revealed a radial styloid fracture with avulsion fracture of dorsal articular margin of the distal part of the radius. Surgical treatment was performed 4 days after the injury. However, it was impossible to red

Extension-block pinning for fracture-dislocation of the proximal interphalangeal joint.

INTRODUCTION: Dorsal fracture-dislocation of the proximal interphalangeal joint is an unstable fracture that associates the anterior marginal fracture of the second phalangeal base and the dorsal dislocation of the proximal interphalangeal joint under the influence of the median band traction of the extensor apparatus. Stiffness and residual pain are frequent sequelae. Treatment involves choosing between the various methods with the objective of providing stable reduction allowing early mobilization. HYPOTHESIS: We relate our experience concerning treatment by proximal interphalangeal extensi

Two-team simultaneous open surgical treatment in bilateral shoulder fracture dislocation.

UNLABELLED: Different surgical options have been described for the treatment of bilateral shoulder fracture dislocations. We report 2 cases of bilateral shoulder fracture dislocation treated simultaneously by 2 surgical teams. One case had bilateral hemiarthroplasty performed, and the second case had 1 hemiarthroplasty and McLaughlin procedure on the other shoulder. A device to allow holding the patient in a bend chair position, with both shoulders hanging outside the operative table, allowing their free movement, was designed. At 1-year follow-up, neither of the patients had complications and…

Open glenohumeral dislocation: skeletonization of the proximal humerus without associated fracture.

Shoulder dislocations are common injuries. In the realm of high-energy trauma, enough force can be dissipated to violate the entire soft tissue envelope surrounding the shoulder girdle, generating an open injury. This article presents a case of a young man involved in a motorcycle accident in which he sustained an open glenohumeral dislocation with complete skeletonization of the proximal humerus. There were no associated fractures with his injury. Our patient underwent staged irrigation and debridement of his shoulder with delayed tendoligamentous reconstruction of the skeletonized proximal h

Avascular necrosis following fracture-dislocation of the hip and spontaneous relocation.

Injuries occurring during soccer tend to occur with lower energy transfer than in some other contact sports. Tibial and femoral shaft fractures occasionally occur, but pelvic fractures are rare. We report a case of a missed posterior acetabular rim fracture, caused by a low energy trauma playing soccer, complicated by the development of avascular necrosis (AVN) of the femoral head and subsequent osteoarthritis.

Evaluation of the value of pedicle screws for cervicothoracic fracture-dislocation.

OBJECTIVE: To explore the value of pedicle screw implantation for fracture-dislocation injuries of the cervicothoracic junction (C(6)-T(2)). METHODS: Pedicle screws were implanted into 26 patients (17 male and 9 female) with cervicothoracic fracture-dislocations from May 2001 to January 2008. Computed tomography (CT) scan was used to evaluate the accuracy of placement of the pedicle screws three days after surgery. Complications and state of neurological recovery were also recorded after the procedures. RESULTS: One hundred and four pedicle screws were implanted successfully in our group. Se

Cerebral salt wasting syndrome following atlantoaxial fracture dislocation in Down syndrome.

We describe cerebral salt wasting syndrome (CSWS) in a 5-year-old female child with Down syndrome who had acute myelopathy secondary to chronic atlantoaxial subluxation and fracture dislocation of the odontoid process. The patient developed hyponatraemia associated with excessive urine output and elevated urine sodium concentration following her injury. An administered volume-for-volume replacement of urine loss with 0.9% sodium chloride resulted in an excellent outcome. This patient illustrates the importance of ascertaining CSWS in children with spinal cord disorders, in addition to the synd…

[Arthroscopic assisted treatment of shoulder dislocation combined with greater tuberosity fracture].

OBJECTIVE: To investigate the operative method and clinical results of arthroscopic assisted treatment of shoulder dislocation combined with fracture of greater tuberosity of humerus. METHODS: From February 2006 to June 2008, 12 cases of shoulder dislocation (6 left, 6 right) combined with greater tuberosity fractures were treated. There were 4 males and 8 females with an average of 58.5 years (range 34-79 years). Eleven cases fall down and one was crushed. The time from injury to hospital averaged 2.1 hours (range 30 minutes-24 hours). X-ray films revealed greater tuberosity fractures with a

Posteromedial dislocation of the elbow with lateral condyle fracture in children.

Posteromedial dislocation of the elbow with lateral condyle fracture is a rare injury, and only a few cases have been reported in the literature. We report 3 children who had posteromedial dislocation of the elbow with Milch type II lateral condylar fracture of the distal humerus. Major complications included malunion, nonunion of the fracture, heterotophic calcification and elbow subluxation. From our experience and a review of the literature, open anatomical reduction and internal fixation of the lateral condyle fracture is the mainstay of treatment for these children. Poor reduction of the
